Book excerpt: In patients with hepatobiliary and pancreatic cancers, surgery is the only potential curative treatment for resection candidates (Benson et al., 2009; Freelove & Walling, 2006). A five-year research experience in patients undergoing surgery for colorectal cancer found that prehabilitation led to faster return to preoperative walking capacity than rehabilitation (Minnella et al., 2017). There has been no studies thus far evaluating the impact of a trimodal prehabilitation program on functional recovery in patients undergoing hepatobiliary or pancreatic cancer surgery. Our goal was to determine the impact of prehabilitation on the functional exercise capacity of these patients. We performed a randomized controlled pilot trial comparing the impact of a prehabilitation program begun four weeks before surgery, to the same program (rehabilitation) initiated right after surgery. The program was maintained by both groups until eight weeks postoperatively. We hypothesized our prehabilitation program would improve 6-minute walk test performance preoperatively. Furthermore, at eight weeks post-surgery, we hypothesized greater 6-minute walk performance in the prehabilitation group compared to the rehabilitation group. Thirty-five participants were randomized to receive prehabilitation or rehabilitation. We found that preoperatively, the prehabilitation group demonstrated a clinically meaningful improvement in 6-minute walk distance. At four-weeks after surgery, the rehabilitation group experienced a statistically and clinically significant decrease in mean 6-minute walk distance from baseline, whereas the prehabilitation group was able to maintain its baseline walking capacity. Our findings suggest that a prehabilitation program in hepatobiliary and pancreatic cancer patients can deliver meaningful changes in pre- and postoperative functional exercise capacity.

Book excerpt: "Objective: My doctoral work aims to investigate the effects of multimodal prehabilitation on functional capacity in lung cancer patients undergoing surgery, with a specific interest on the impact of preoperative nutrition.Study 1: The objective was to determine whether a multimodal prehabilitation program enhances post-operative functional recovery, measured by the 6-minute walk test (6MWT), 8 weeks after surgery compared to multimodal rehabilitation. Both interventions were entirely home-based and consisted of aerobic and resistance training, nutritional consultations with whey protein supplementation and relaxation strategies, commenced either before (prehabilitation, n=52) or after (rehabilitation, n=43) surgery. There was no difference in functional capacity at any time point during the perioperative period between the 2 multimodal programs. By 8 weeks after surgery, both groups returned to baseline 6MWT and a similar proportion of patients (over 75%) in both groups had recovered to their baseline. It was concluded that both interventions were as effective in recovering functional capacity.Study 2: A systematic review was conducted to determine the effect of preoperative nutrition and multimodal prehabilitation on clinical and functional outcomes. Only 5 studies were included, 4 of which were multimodal prehabilitation interventions and 1 was a preoperative nutrition-only intervention. Limited research has been performed on preoperative nutritional supplementation in lung cancer patients however, multimodal prehabilitation programs that combine nutrition and exercise may have beneficial effects on various physical function outcomes. Study 3: The goal of this secondary analysis study was to characterize the presence of malnutrition, examine the association between malnutrition and baseline functional capacity and examine the extent to which patients benefit from preoperative multimodal prehabilitation. Data from 162 participants enrolled in multimodal prehabilitation or control before lung cancer surgery were analyzed. Malnutrition was measured using the Patient-Generated Subjective Global Assessment (PG-SGA) according to triage levels. A total of 83 (51.2%) patients were considered at low nutrition risk, 61 (37.7%) were at moderate nutrition risk and 18 (11.1%) were at high nutrition risk. Results confirmed that patients at high nutrition risk had significantly lower baseline functional capacity compared to patients at low nutrition risk [mean baseline 6MWT: 416 m (SD 90) vs. 484 m (SD 88)], but experienced a significant improvement in preoperative functional capacity of 58.9 m (16.7-101.2) upon receiving multimodal prehabilitation.Study 4: The aim was to investigate the feasibility of delivering a novel 4-week multimodal prehabilitation intervention compared to standard of care (control). Results showed that within a preoperative time-frame, it was feasible to deliver this novel multimodal prehabilitation intervention as recruitment rate was 58.6% and adherence to the a) prescribed intensity of the supervised exercise program was 84.1% (SD 23.1), b) home-based exercise program was 88.2 % (SD 21), c) nutritional supplement was 93.2% (SD 14.2) and d) patients’ preoperative assessment was 82% and 88% in the prehabilitation and control group, respectively.
