Book excerpt: The primary purpose of this study was to examine how clients' self-reported adult attachment pattern and their attachment to the counselor are associated with working alliance and premature termination. A total of 65 clients at a large southeastern university counseling center were included in data analysis. Clients in this study completed survey packets including the Experiences in Close Relationships Scale (Brennan, Clark, & Shaver, 1998), the Working Alliance Inventory (Horvath & Greenberg, 1989), the Client Attachment to Therapist Scale (Mallinckrodt, Gantt, & Coble, 1995), the Outcome Questionnaire 45 and 30 items (Lambert et al. 1996), and the Therapeutic Distance Inventory (Mallinckrodt, 2011) at four different time points: (a) pretest, (b) after the 3rd session, (c) after the 5th session, and (d) at termination. The Therapeutic Distance scale is composed of four dimensions, Too Close, Too Distant, Growing Engagement, and Growing Autonomy. Results suggested that interactions between adult attachment (anxiety or avoidance) and therapeutic distance were not significantly associated with working alliance or premature termination. However, therapeutic distance subscales were correlated as direct effects with working alliance and premature termination. Other findings suggested adult attachment did not change over the course of therapy. The Client Attachment to Therapist (CATS) subscales at session 5 and at termination were significantly correlated with premature termination. In addition, working alliance at termination was significantly negatively associated with premature termination. Finally, the CATS-Avoidant-Fearful subscale at session 3 was associated with an increase in symptoms, and working alliance at session 3 was associated with a decrease in symptoms. Implications for theory, psychotherapy, and future research are discussed.

Book excerpt: The current study examined whether attachment style, interpersonal problems, and treatment expectations predict outcome in psychotherapy and whether the working alliance may act as mediator for this prediction. Participants were 226 clients at an outpatient psychotherapy training clinic housed within a medium-sized Midwestern university. Clients completed measures of attachment, interpersonal problems, and treatment expectations prior to beginning therapy. Clients were administered the Outcome Questionnaire-45 at intake and session 3 and Working Alliance Inventory (Short, Revised) at session 2. Using structural equation modeling, it was found that both more secure adult attachment and higher treatment expectations were predictive of a stronger working alliance, though neither were predictive of outcome. Results were consistent across models whether measuring outcome at session 3 or termination (median treatment length = 8 sessions). However, interpersonal problems were not found to be predictive of either working alliance or outcome in therapy. These findings indicate that both the client's attachment security and attitudes regarding treatment both play a role in predicting the working alliance. Implications and ways of utilizing these findings to better affect the course of psychotherapy are discussed.

Book excerpt: This thesis explores the associations between therapist and client attachment styles and the working alliance. It is presented as three papers: a literature review, a report of the empirical research study, and a critical reflection of the research process. In the first paper, the author provides a systematic review of studies that have investigated the association between therapist attachment and the working alliance and therapist attachment and clinical outcome. A total of nine research studies met the inclusion criteria and were included in the final review. The studies varied considerably in terms of their design and methodology, as well as, the different tools used to measure the therapeutic alliance, attachment style and outcome. The findings were then organized in terms of the relationship between therapist attachment and client-rated therapeutic alliance; the relationship between therapist attachment and therapist-rated alliance; the relationship between therapist attachment and alliance as rated by an observer and finally, the relationship between therapist attachment and outcome. The key findings were that therapist attachment security was associated with client-rated and therapist-rated working alliance. However, the overall association between therapist attachment and alliance was not straight forward. There was evidence to suggest that the attachment style of therapist and the client interact to produce a combined effect on perceptions of the working alliance and outcome. The empirical research paper examined the extent to which client and therapist self-reported attachment styles were related to the working alliance. The study also investigated any role for client attachment to therapist and psychological mindedness. Both these variables were included as exploratory. It was hypothesised that psychological mindedness may be related to secure attachment and the alliance. Specifically, we investigated the extent to which self-reported attachment styles were related to the working alliance and assessed the relative contribution of psychological mindedness to attachment security and the quality of the working alliance. Thirty clients and 42 therapists from primary care services were recruited. Participants completed measures of anxiety and depression, attachment style, working alliance and psychological mindedness. Therapist attachment security was not related to the alliance. No significant relationships were found between client attachment style and the working alliance. However, therapists and clients with oppositional attachment styles had more favourable alliances. The clinical implications and future research directions is then discussed. The final paper provides a critical reflection of the research process. It begins by outlining the rationale for the development of the literature review and the empirical research paper, and goes on to discuss some of the methodological considerations of the research paper. The implications for therapeutic practice are then suggested, followed by the wider service-related issues. Attachment theory is then critiqued, and the clinical implications discussed. Finally recommendations are made for future research.

Book excerpt: Premature termination in psychotherapy is associated with negative consequences for clients, therapists, and the institutions that employ them, in the form of poorer mental health, diminished confidence, and lost revenue, respectively. Missed appointments in the form of late cancellations and no-shows exert similarly negative effects on client outcomes and treatment providers (Berrigan & Garfield 1981; Edlund et al., 2002; LaGanga & Lawrence, 2007; Oldham, Kellett, Miles, & Sheeran, 2012). Approximately one out of every five patient/clients in treatment will drop out of therapy before goals are met, regardless of treatment settings or intervention (Swift & Greenberg, 2012). The current study sought to explore relational variables - the therapeutic alliance and adult attachment style - as they relate to premature termination and missed appointments. A total of 110 case files, collected at a university training clinic over a period of 38 months, were consulted to determine rates of premature termination and missed appointments, as well as the relationships of these dispositions with alliance and attachment. The effects of attachment and alliance on premature termination could not be adequately tested due to missing data; missed appointment rate was analyzed instead. The results of regression analyses indicated that individuals who missed more sessions were more likely to prematurely terminate. Client-reported adult attachment style did not predict missed appointment rate; effects of client-reported alliance were confounded with a low sample size and positively skewed distribution and as such results were not conclusive. Implications and recommendations are suggested for future research.

Book excerpt: There is some evidence from individual psychotherapy that the ability to build therapeutic alliances with clients may be related to the therapist's adult attachment style. These are patterns of expectations, needs, emotions and social behaviour, learn from our history of attachment experiences with our caregivers (Fraley and Shaver in Mikulincer and Shaver, 2007). There are no published empirical studies of the association between therapist's attachment style and therapeutic alliance in family therapy. Aim The overall aim was to investigate family therapist's attachment styles and explore the association between the therapist's attachment style and therapeutic alliance in family therapy. Method Participants were registered UKCP family therapists. The research comprised three linked studies. Study 1 was a survey using two different self-report attachment measures, the Relationship Questionnaire (Bartholomew and Horowitz, 1991) and the Experiences in Close Relationships questionnaire (Brennan et al., 1998) to examine the variety of therapists' attachment styles; Study 2 sought to examine therapists' and families' perceptions of the therapeutic alliance using the System for Observing Family Therapy (SOFTA, Friedlander et al., 2006) and to examine their association with the therapists' attachment styles; and Study 3 explored therapists' (of different attachment styles) views on alliance and their therapeutic work using semi-structured interviews. Quantitative data derived from the survey of therapists' attachment styles (Study 1) and the survey of therapists' difficulties with Study 2 were analysed using descriptive, and where possible, inferential statistics. Open ended questions for Study 2 were analysed thematically. Digital recordings of the interviews with therapists (Study 3) were analysed using Interpretive Phenomenological Analysis (Smith, 1999) and a template analysis based on a conceptual model of the therapeutic alliance in family therapy, the System for Observing the Therapeutic Alliance (SOFTA, Friedlander et al., 2006). In addition, observational field notes and self- reflections on the interview process were analysed using the method of Interpretative Observational Analysis (Dallos and Vetere, 2005). Results Study 1 showed that therapists (N=82) tended to self-report as having a 'secure' attachment style (74.4%) when assessed using the RQ, a more obvious measure. However, only just over a quarter of them were assessed as having a 'secure' attachment style on the, less transparent, ECR measure. Attachment style, as measured by the ECR, was not associated with therapists' prior profession, 3. gender and preference for a model of therapy. The attempt to investigate the association between therapists' attachment styles and family members' and therapist's perceptions of the therapeutic alliance in family therapy (Study 2) was unsuccessful. In a follow up questionnaire, therapists (N = 13) who had agreed to participate reported that they were unable to carry this through for a number of reasons. These explanations included a feeling that they were overloaded with new paperwork, that they had no time beyond their regular work, and that they had no suitable new cases. The 'failure' of Study 2 showed the difficulty of engaging practitioners in research on their practice. The interviews with therapists (Study 3) showed differences between the 'secure' and 'insecure' therapists (N=13) in alliance building according to their attachment styles. The 'secure' therapists were able to be more reflexive and used richer explanations to guide their therapeutic work than their 'insecure' counterparts. In two cases there was a discrepancy between the therapists self- reported assessments of themselves as 'secure' and their interview presentations. Discussion and Conclusions It is assumed in the literature that effective family therapy requires a 'secure' base/strong therapeutic alliance and that family therapists should have a 'secure' adult attachment style. However this study indicates: 1. On what is probably the more reliable measure, the ECR, around three- quarters of family therapists in this study appeared to have 'insecure' attachment styles. 2. There are differences between participating 'secure' and 'insecure' family therapists in their accounts of therapeutic alliance in family therapy, suggesting that participating 'insecure' therapists have difficulty in some of the key elements of therapeutic alliance building. Nonetheless, this research cannot say for certain whether therapists' attachment styles influenced the formation of therapeutic alliance because this part of the study was unsuccessful. Consequently, it would be desirable to make another attempt to investigate the possible association and to explore whether it was associated with the outcomes of therapy. 4. Given that it would appear that a high proportion of family therapists in this study manifested 'insecure' attachment styles and may have unresolved attachment issues arising from family of origin experiences, personal development/therapy for family therapists would seem to be desirable, both as part of training and subsequently.
