It is an operation for the formation of an artificial or bi-mucous fistula between the stomach and some point in the small intestine. The same operation may be extended to some part of the colon. It was first performed by Wolfler of Vienna, in 1881, who began an operation for pylorectomy, and being unable to execute it from local conditions, ended the scene with a gastro-enterostomy. Since that time Billroth, Lauenstein, Senn, Rydygier, Barker, Rockowitz, Stamm, Ranshoff, Page, Monastryski, Czerny, Hahn, Winslow, Postowski, and others, have practiced and advocated the operation. The records of some 105 operations for gastro-enterostomy are extant, but I surmise some of these are repeated reports of the same cases. The object of the operation is to overcome obstruction of the pylorus by directing the faecal circulation through another channel. Gastro-enterostomy is a rival and a substitute for pylorectomy, and is fast displacing the latter operation. Being done in conjunction with an active surgical practice, it naturally partakes of the practical viewpoint. The first volume discusses the more abstract problems, while the second volume has to do with the practical aspects. This being a research I have not felt obligated to treat all chapters equally and in the same way. In those chapters in which my own work is chiefly represented, there was in some instances little literature to consider. In others, for instance, that on Tuberculosis, where my own material was limited, the literature was reviewed extensively. On the whole, my reasons for the selection of the literature quoted are purely personal, and no attempt has been made to judge the permanency of the value of the papers selected for the bibliography.
