With this vacuum of knowledge, various "guidelines" have proliferated that are either misleading or difficult to practice. In this manual, we present 16 MAPs that have been extensively reviewed by the cardiologists at MD Anderson. These MAPs should be considered our best practices rather than "guidelines." These MAPs will be updated frequently to reflect advances in the field. This manual con¬sists of MAPs, figures, and tables.
