Book excerpt: Biomarkers are of critical medical importance for oncologists, allowing them to predict and detect disease and to determine the best course of action for cancer patient care. Prognostic markers are used to evaluate a patient’s outcome and cancer recurrence probability after initial interventions such as surgery or drug treatments and, hence, to select follow-up and further treatment strategies. On the other hand, predictive markers are increasingly being used to evaluate the probability of benefit from clinical intervention(s), driving personalized medicine. Evolving technologies and the increasing availability of “multiomics” data are leading to the selection of numerous potential biomarkers, based on DNA, RNA, miRNA, protein, and metabolic alterations within cancer cells or tumor microenvironment, that may be combined with clinical and pathological data to greatly improve the prediction of both cancer progression and therapeutic treatment responses. However, in recent years, few biomarkers have progressed from discovery to become validated tools to be used in clinical practice. This Special Issue comprises eight review articles and five original studies on novel potential prognostic and predictive markers for different cancer types.

Available in PDF, EPUB and Kindle. Book excerpt: Carcinoma of the prostate increasingly dominates the attention of urologists for both scientific and clinical reasons. The search for an explanation and the prediction of the variable behaviour of the malignant prostatic cell continues unabated. The search for more precise tumour staging and more effective treatment is equally vigorous. Editors Andrew Bruce and John Trachtenberg have assembled acknowledged leaders in prostate cancer to present those areas of direct interest to the clinician. There are a number of other topics that might have been considered but most of these, such as experimental tumour models or biochemical factors affecting cell growth, still lack immediate application for the clinician. Carcinoma of the prostate continues to have its highest incidence in the western world, and the difference in comparison with the incidence in the Far East appears to be real and not masked by diagnostic or other factors. A number of other epidemiological aspects need careful analysis: Is the incidence increasing? Is the survival improving? Is the prognosis worse in the younger patient? Epidemiological data are easily misused and misinterpreted so that a precise analysis of the known facts makes an important opening chapter to this book.

Available in PDF, EPUB and Kindle. Book excerpt: Prostate cancer is by far the most common cancer in men and the second leading cause of death due to cancer. It comprises a mixed group of tumours displaying varying clinical behaviour: while some have a very aggressive course, others are rather indolent. Prevention of prostate cancer and discrimination between aggressive and indolent forms are important clinical goals and the acquisition of significant new evidence on means of achieving these aims makes this book particularly timely. A wide range of topics are covered by leading authorities in the field. The biology and natural history of prostate cancer are reviewed and the role of lifestyle and dietary factors, assessed. Detailed attention is paid to risk prediction biomarkers and to the role of novel high-throughput nucleic acid-based technologies in improving risk prediction and thereby allowing tailored approaches to cancer prevention. Potential means of chemoprevention of prostate cancer are also reviewed in depth, covering the very positive new data on the impact of aspirin as well as evidence regarding 5α-reductase inhibitors, DFMO and lycopene. Guidance is provided on the differentiation of aggressive from indolent disease and the policy and research implications of recent findings are examined. This book will be of interest to both clinicians and researchers.

Available in PDF, EPUB and Kindle. Book excerpt: This volume, with chapters written by experts in the field of cancerous tumors, details the key factors associated with liquid biopsies in solid tumors: blood-based diagnostics; circulating tumor cells; enumeration and molecular analysis (association with breast cancer); epithelialmesenchymal transition; detection and monitoring; circulating-free tumor DNA; CTCs and ctDNA; and the exosome. The field of blood-based diagnostics is rapidly evolving demonstrating the possibility of real-time molecular analysis of cancer cells and their phenotype and genotype. Circulating Tumor Cell (CTCs) have demonstrated prognostic and predictive value in advanced cancer and represents a source of tumor cells for transcriptome and genomic analysis. Most recently, the detection of genomic abnormalities in the peripheral blood by sensitive and selective PCR methods (liquid biopsy) opened to the option of a comprehensive blood-based tumor analysis. Similar information can be obtained by analysis of exosome, a natural packaging and messaging system being explored in advanced malignancies. The final frontier is the evaluation of immune cells determinant of innate and adaptive immunity.

Available in PDF, EPUB and Kindle. Book excerpt: Prostate cancer (PCa) is the most common newly diagnosed cancer among men in the United States today. With the advent of the prostate-specific antigen (PSA) test, the number of newly diagnosed cases has increased tremendously. The rates of PCa have increased so dramatically over the last decade that the age-adjusted incidence rate of PCa is no greater than that for any other cancer among men in the United States. Although PCa rates have risen steadily since 1973, there has been a dramatic acceleration in the late 1980s which has been associated with the introduction and use of PSA for screening and early detection. There is now some evidence that the rates may be levelling off and even decreasing in some areas. After lung cancer, PCa is the leading cause of death due to cancer in men in the United States. Although PCa can occur in younger men, it is essentially a cancer of elderly men. The highest rates of PCa in the world occur among African-American men in the United States. African-Americans have higher rates than Caucasians at all age levels in the United States, and adjusting for social-economic status does not appear to account for this difference to any appreciable extent. There is no clear reason why PCa rates are so much greater among African-Americans compared with Caucasians in the United States. The reported rates in Africans are substantially lower than those of an African-American, suggesting that environmental factors have an influence on PCa. In spite of this substantial impact on our society. PCa remains a relatively understudied disease, with an essentially unknown etiology. The reviews contained in this book are by no means exhaustive. We have, however, attempted to provide information regarding the pathology of prostate cancer, the status of diagnostic and prognostic markers, as well as a discussion of our understanding of the molecular basis for the disease. The biology of prostate cancer is covered with a discussion on the role of apoptosis in prostate cancer and the suggestion of it being the target for new therapeutic development, as well as the role played by integrins and angiogenesis. The status of current therapies, both hormonal and chemotherapy, are also discussed. This volume should provide a useful background for individuals wishing to commence an in-depth understanding of prostate cancer.

Book excerpt: Of the thousands of biomarkers that are currently being discovered, relatively few are being validated for further applications, and the potential of a biomarker can be quite difficult to evaluate. To aid in this imperative research, Dr. Kewal K. Jain’s Handbook of Biomarkers thoroughly describes many different types of biomarkers and their discovery using various "-omics" technologies, such as proteomics and metabolomics, along with the background information needed for the evaluation of biomarkers as well as the essential procedures for their validation and use in clinical trials. With biomarkers described first according to technologies and then according to various diseases, this detailed book features the key correlations between diseases and classifications of biomarkers, which provides the reader with a guide to sort out current and future biomarkers. Comprehensive and cutting-edge, The Handbook of Biomarkers serves as a vital guide to furthering our understanding of biomarkers, which, by facilitating the combination of therapeutics with diagnostics, promise to play an important role in the development of personalized medicine, one of the most important emerging trends in healthcare today.
