Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. Book excerpt: Based on extensive use of primary sources, this book provides an analysis of Solidarity, from its ideological origins in the Polish "new left," through the dramatic revolutionary months of 1980-81, and up to the union?s remarkable resurgence in 1988-89, when it sat down with the government to negotiate Poland?s future. David Ost focuses on what Solidarity is trying to accomplish and why it is likely that the movement will succeed. He traces the conflict between the ruling Communist Party and the opposition, Solidarity?s response to it, and the resulting reforms. Noting that Poland is the one country in the world where "radicals of ?68" came to be in a position to negotiate with a government about the nature of the political system, Ost asks what Poland tells us about the possibility for realizing a "new left" theory of democracy in the modern world. As a Fulbright Fellow at Warsaw University and Polish correspondent for the weekly newspaper In These Times during the Solidarity uprising and a frequent visitor to Poland since then, David Ost has had access to a great deal of unpublished material on the labor movement. Many of the prerequisites for establishing an MTBF have already been satisfied in Poland. The annual budget has a high degree of credibility; macroeconomic projections are generally accurate; and fiscal rules on debt, expenditure, and the deficit provide sufficient guidance to fiscal policy in the medium term. However, some weaknesses remain that should be addressed either in parallel or as part of the MTBF reform. The design of the MTBF should be carefully considered to meet the government’s fiscal objectives while operating within current capabilities.

Book excerpt: This paper highlights the fact that to address the challenge of declining working age population, policies should not only aim to support fertility, but also to increase labor force participation rate, to improve labor allocation across sectors and to encourage net immigration. However, with some recent measures going in the opposite direction, even such mitigating policies may not fully offset the demographic headwinds. Poland faces profound demographic changes. The decline of fertility rate and growing life expectancy, mostly driven by longevity of older cohorts, will significantly change the demographic landscape. Demographics may soon pose a barrier for growth. The historical expansion of working age population appears to have been only partly utilized to boost potential growth, with increased unemployment and declined labor force participation rates dampening potential gains. Net migration was also a drag on the labor supply in the past, but this has changed recently. In addition to a steady outflow of permanent migrants, there was a sharp pickup in temporary migration after EU accession.

Book excerpt: Abstract: This empirical paper sheds light on a significant element of the debate of whether infrastructure services have a strong impact on economic development by exploring the impact of innovative road freight services on downstream business users. The paper uses a new and purpose-specific survey of 165 logistics service providers and 493 user enterprises in food processing, food distribution, and the automotive industry in the Czech Republic, Hungary, and Poland. The main findings are that there are substantial downstream benefits from innovations in road freight services, both dampening cost increases and raising sales revenues of business users. The additional finding that increased intensity of competition in road freight services is significantly associated with the provision of innovative services suggests that easing any remaining barriers to competition in upstream business sectors should be a priority.
