Les pleurs prolongés et inconsolables sont un facteur identifié de la survenue du syndrome du bébé secoué. Les parents se sentent insuffisamment écoutés et informés par les professionnels de santé sur cette thématique alors que la réassurance parentale est préconisée en première intention en cas de pleurs prolongés du nourrisson. Déterminer les représentations, le ressenti et les besoins parentaux pour les anticiper et délivrer des conseils adaptés lors des consultations de médecine générale, ceci dans un souci de prévention. Etude qualitative non participante de discussions sur trois forums virtuels de langue française. Les représentations des pleurs étaient multiples et identifier leur cause était primordial. De leur interprétation dépendait leur gestion et leurs conséquences. Les parents rapportaient leur vécu difficile et les conséquences néfastes des pleurs. Les mères témoignaient de la particularité de leur rôle. Elles étaient souvent seules responsables. Se sentant délaissés par le corps médical, les parents exprimaient leurs besoins sur les forums. Le besoin d'identification aux autres et le besoin de soutien étaient prédominants. L'approche centrée patient requiert de comprendre le vécu et les représentations des parents. Notre étude propose des éléments pour mieux les accompagner. Les parents ont besoin d'être encouragés à se confier. Valoriser les pleurs comme un signal d'attachement pourrait permettre d'orienter leurs représentations et d'améliorer leur vécu. Les mères nécessitent un accompagnement particulier et le père devrait être intégré pour un trinôme harmonieux. Il est bénéfique pour les parents d'échanger avec d'autres parents vivant la même situation.

Concernant cette thématique, les parents sont en demande d'information fiables et accessibles. En consultation, le médecin généraliste manque parfois de temps et de connaissance pour pouvoir aborder ce sujet. L'objectif principal était de créer un outil vidéo pédagogique destiné aux parents, détaillant les notions essentielles concernant les coliques infantiles, les signes d'alerte, la prévention du syndrome du bébé secoué ainsi que les ressources professionnelles disponibles. L'objectif secondaire était de réaliser une première évaluation de cette vidéo auprès d'un échantillon de parents. La méthodologie ADDIE a été suivie pour réaliser cet outil pédagogique. Nous avons fait appel à des professionnels de l'audio-visuel. Nous nous sommes appuyées sur des ressources bibliographiques validées. Notre public cible représentait des parents de nourrissons. L'outil a été évaluée grâce à une étude qualitative, en utilisant la méthode du focus group, sur un échantillon de parents. Une vidéo en Motion Design a été réalisée, d'une durée de 3 à 5 minutes, pouvant être visualisée sur YouTube. Notre évaluation a montré que cet outil semble représenter une réponse appropriée aux parents en demande d'informations. Des pistes d'améliorations ont été proposées. Cet outil pédagogique propose des réponses à certaines questions que se posent les parents de nourrisson, et peut permettre de pallier au manque de connaissance et de temps de certains médecins généralistes. Avant d'envisager la diffusion plus large de cette vidéo, il serait intéressant d'envisager une étude d'évaluation en la montrant aux parents à la maternité, avant l'accouchement, puis d'évaluer les connaissances et les pratiques des parents après la visualisation de la vidéo, après la naissance du nouveau-né.

Nous avons réalisé une étude transversale prospective, en envoyant, à 7473 assistantes maternelles du département de Loire Atlantique, un questionnaire en ligne portant sur leurs connaissances et leur prise en charge des pleurs. Son objectif principal était d'évaluer les connaissances théoriques et pratiques des AM concernant les pleurs prolongés des nourrissons. L'objectif secondaire était de déterminer les facteurs réduisant ou favorisant la survenue de trois attitudes inadaptées face aux pleurs: le sentiment d'être persécutée par l'enfant, la peur de réagir avec violence et l'émission de paroles déplacées. Le niveau de connaissance des 1074 assistantes maternelles inclues était correct et corrélé à leur niveau d'études initiales.
