Book excerpt: This case study illustrates the features of a common type of myasthenia gravis, which is observed in older people and is called the oculobulbar form because it principally involves the muscles of the eye. Abnormalities of the thymus are not observed in older patients and thymectomy does not help.

Available in PDF, EPUB and Kindle. Book excerpt: Treatment of myasthenia gravis is still a rather difficult task, since there is no single tactic to use different drugs (corticosteroids, rituximab, immunoglobulins), especially since it is associated with a number of side effects. They are not able to remove the accumulating autoantibodies and immune complexes, the large size of which does not allow them to be excreted by the kidneys as well. Special problems of treatment arise when myasthenic crises develop associated with respiratory failure requiring artificial lungs ventilation. Plasmapheresis can help to solve this for it is possible to remove antibodies and other pathological metabolites. In addition, regular plasmapheresis is able not only to prevent exacerbations but also to reduce doses of the maintenance therapy with less risk of their side effects, which is confirmed by our own experience.

Available in PDF, EPUB and Kindle. Book excerpt: Autoimmune myasthenia gravis (MG) is a classical autoimmune disease, for which the target antigen, nicotinic acetylcholine receptor, has been cloned, sequenced and biochemically characterized. Antibodies to acetylcholine receptors destroy acetylcholine receptor at the neuromuscular junction, thus leading to defective neuromuscular transmission, muscle fatigue, and weakness. In the last few years, rapid advances have been made in unraveling the cellular and molecular mechanisms involved in the pathogenesis of MG, both in the animal model, experimental autoimmune MG (EAMG), and in human MG. Significant advances are being made in characterizing the cells and molecules involved in the autoimmune response to the acetylcholine receptor (AChR). These advances are leading to the development of specific methods of immunointervention in EAMG. Further understanding of the intricate involvement of the major histocompatibility complex (MHC) and non-MHC genes, T cell receptors (TCR), costimulator molecules, and specific cytokines in the afferent and efferent autoimmune response of AChR should pave the way to future antigen/clone-specific therapy of MG. This book is the outcome of the MG workshop proceedings in Mysore, India, 1998. The majority of the chapters in this book are contributed by world-renowned authors and their students. The book not only contains a timely review of specific topics, but also up-to-date findings. Immunologists and neurologists will find, in this volume, the latest in MG/EAMG cutting-edge research. Clinicians will be interested in the applications of the various immunointervention strategies into clinical trials in MG patients. Finally, students will not only be interested in reading the latest in EAMG/MG research, but will also find information to help them develop a future strategy to unravel the precise mechanism of disease. To summarize, in this book, the readers should find up-to-date information related to immunological mechanisms involved in MG pathogenesis and various modalities for possible approaches to immunointervention to treat MG.

Book excerpt: The neuromuscular diseases comprise the disorders of peripheral nerves, the neuromuscular junction, and muscle. Both pre and post-synaptic elements of the neuromuscular junction may become targets of autoimmune attack in different diseases. This book addresses the immunologically mediated neuromuscular diseases, including Guillain--Barré syndrome and other autoimmune neuropathies, the Lambert--Eaton myasthenic syndrome, myasthenia gravis, and the autoimmune diseases of muscle. Two chapters are devoted to the vasculitic and HIV-mediated neuromuscular diseases. The experimental models of neuritis and myasthenia gravis are addressed in separate chapters. An introductory chapter provides a general background to autoimmunity. This book is a useful overview for all neurologists, rheumatologists and immunologists both in research and clinical practice.

Book excerpt: Advances in the study and understanding of myasthenia gravis have led to the need for the publication of this important new edition. The goal of Myasthenia Gravis and Related Disorders, Second Edition is identical to the first -- to provide the clinician and the scientist with a common resource for understanding this complex disorder. This new edition begins with discussions of neuromuscular junction structure and function and follows with updated chapters covering a wide range of topics, such as the acetylcholine receptor, clinical presentation, diagnostic evaluation, and treatment. Importantly, new supplemental chapters have been added; these discuss rigorous clinical assessments of patients for research trials and the epidemiology and genetics of myasthenia gravis. The discussion of the most challenging aspects of myasthenia gravis, its impact on patients’ psychological make-up, has been expanded as well. Myasthenia Gravis and Related Disorders, Second Edition retains the “personal approach” of the authors regarding treatment and is a valuable resource for meeting the many and varied needs of patients with myasthenia gravis.

Available in PDF, EPUB and Kindle. Book excerpt: This volume contains papers and discussions of the Vlth Dialyse-Arzte Workshop, which was held in Bernried at Lake Starnberg near Munich the 5th and 6th of March 1980. Generous ly sponsored by Travenol, Munich, the Dialyse-Arzte meetings now have a tradition spanning 16 years. According to the con stitution of these meetings, the topics of earlier years had to cover dialysis and related fields. Thus the sponsor requested that this year also one lecture - incorporated here as part - should deal with the state of art of dialysis, thereby hopefully linking this Workshop to the previous meetings. Dialysis techniques of the 1960s, pioneered by many of attend ing speakers and panelists (see List of Contributors), have never come to a standstill. Indeed, vascular access and extra corporeal circulation have become routine for the nephrologist and have made possible the introductimn of new approaches, such as hemofiltration and hemoperfusion. Also today new membrane technologies provide us with a potentially even more effective therapeutic tool, namely plasma separation.

Available in PDF, EPUB and Kindle. Book excerpt: Myasthenia gravis is presently an incurable antibody-mediated autoimmune disorder characterized by generalized voluntary skeletal muscle weakness. The cause of the weakness is a defect at the neuromuscular junction level, in which autoimmune antibodies block the receptors responsible for initiating muscular contraction. Literally translated from its Latin and Greek etymological roots, myasthenia gravis means "grave muscle weakness". Fortunately, advances in modern medicine have resulted in a reduction of the truly "grave" outcomes for those inflicted but, without a cure, the gravity surrounding the disease remains
