Book excerpt: BACKGROUND: Demineralisation around orthodontic brackets is a considerable risk of orthodontic treatment. It can occur very early during treatment and involve a large number of teeth with aesthetic and dental health consequences. Early detection of demineralisation is very important in terms of treatment and prevention. The presence of bacteria is one of the requirements for demineralisation to take place and it is unclear whether the type of bacteria in the plaque or the quantity of certain types of bacteria are significant influences on demineralisation. AIM: To identify any general changes in the bacterial composition of dental plaque in adolescents undergoing fixed orthodontic appliance therapy. A secondary aim is to determine the incidence of white spot lesion development and if this was related to the identified red fluorescent plaque. STUDY DESIGN: Prospective longitudinal cohort study. METHODS: Fourteen 11 to 23 year old consecutive patients attending the Orthodontic Department of Liverpool University Dental Hospital were recruited for this study. ToothcareTM and QLF were used to identify red fluorescent plaque and enamel demineralisation on the labial surfaces of the anterior teeth before and after the placement of fixed orthodontic appliances. The bacterial composition of the red fluorescent supragingival plaque was determined by DNA extraction, polymerase chain reaction amplification of the 16r rRNA gene and denaturing gradient gel electrophoresis. 13 RESULTS: The incidence of white spot lesions was recorded as 4.2% of the total surfaces of the teeth included as detected with QLF, the development of white spot lesions was not associated with the presence of red fluorescent plaque. There were differences in the bacterial composition of red fluorescent supragingival plaque in terms of P. gingivalis, S. mutans and S. gordonii between different participants, although changes in the plaque composition between visits for the same participant was not significantly associated with the development of white spot lesions in adolescents. CONCLUSIONS: With the limitations of this study, the placement of fixed orthodontic brackets in adolescents does not significantly change the bacterial composition of red fluorescent supragingival plaque around the brackets and that the development of white spot lesions cannot be correlated to red fluorescent plaque alone.

Subject and methods: 22 orthodontic patients were selected. The study group received an application of Duraphat varnish. Plaque samples were collected at baseline and 30 and 60 days after fluoride varnish application. The total cultivable microflora were counted on blood agar. Mitis-Salivarius agar served to enumerate different oral streptococci and, Rogosa SL agar plates to count lactobacilli. Results: S. mutans counts were statistically lower in the control group than in the experimental groups. Conclusion: Fluoride varnish helps to reduce the bacterial flora in plaque immediately after application but the long-term cariostatic effect of fluoride varnish probably is caused by the remineralization of early carious lesions.

Book excerpt: Increased plaque, calculus, and tartar build-up are some of the worst consequences of braces and orthodontic treatment. Orthodontic brackets and wires act as inadvertent traps that collect food and plaque where bacteria accumulate, and subsequently cause heavy inflammation of the gums, cause pain, and generate subsequent bone loss. Current supplemental oral hygiene aids include floss threaders, "super-floss," interproximal brushes, and most importantly reinforcing proper oral hygiene habits at every orthodontic visit. However, implementing these strategies often render ineffective due to the increased cost of additional appliances and due to patients' non-compliance at such a young age and their unwillingness to change their oral hygiene habits. Prior investigations within the last decade have recently identified arginine, an amino acid, as an active ingredient in oral dentifrices that help prevent dental sensitivity and dental decay. These recent investigations and advancements of amino acids in oral health care products have led to questions of whether they can also help in decreasing plaque that subsequently surround braces once orthodontic treatment starts. A recent study at UCLA (2015) has shown in-vitro bacterial plaque and biofilm that is specifically formed from an orthodontic bracket (Braces). Additionally, different combinations of amino acids were added to evaluate its efficacy in reducing this specific biofilm. Resultant data showed that a combination of the D-forms of arginine, alanine, and lysine proved to be the most effective of any combination tested towards the decrease of the bracket-derived plaque. This previous study in-vitro study and previous notable associations of amino acids in its positive outcomes in oral health care have led us to investigate whether amino-acids (more specifically arginine, alanine, and lysine) in toothpaste can help prevent and reduce plaque in patients with braces. This will be conducted via two randomized, double-blind clinical trials and its analyses using both objective and subjective measures.

Book excerpt: Hydrogen peroxide rinses have been reported to help prevent pathological colonization of filaments, fusobacteria, motile and curved rods, and spirochetes in developing plaque, while allowing an increse in the number of coccoid organisms. The antibacterial and mechanical action of carbamide peroxide may provide a cleansing mechanism that could be beneficial to suppression of plaque accumulation on the surface of ceramic orthodontic brackets. The purpose of this double-blind crossover study was to evaluate the microbial flora on the surface of ceramic orthodontic brackets when using carbamide peroxide (experimental) or glycerin (control) as a brushing agent. Thirty-one patients in a graduate orthodontic program were randomly assigned to experimental or control groups. Each patient received a prophylaxis prior to bonding orthodontic brackets. After 3 months, one of the six maxillary anterior brackets was randomly selected for debonding. After a one month rest period, the subjects received a prophylaxis, six new brackets were bonded, and the groups were crossed-over. Three months later, the same tooth was debonded and the microbial population on the debonded braskets was evaluated with scanning electron microscopy. Of the 24 pairs of brackets available at the end of the study, 10 demonstrated no change, 7 exhibited younger plaque morphotypes, and 7 exhibited more mature plaque morphotypes when using the experimental agent. Carbamide peroxide showed no significant difference from the control agent in preventing plaque maturation and colonization at 3 months in patients with ceramic brackets.
