Book excerpt: Introduction : la maladie rénale chronique est un problème de santé publique, avec une prévalence et une incidence en constante augmentation, dont l’évolution est l’insuffisance rénale terminale avec nécessité de traitement de suppléance. L’objectif de notre travail est de déterminer les différentes ressources auxquelles les patients ont eu recours pour choisir leur traitement de suppléance, la fréquence des consultations médicales, et le degré de confiance des patients dans ces différentes ressources. Matériel et méthode : il s’agit d’une étude rétrospective multicentrique incluant des sujets atteints de maladie rénale chronique sous traitement de suppléance depuis moins de cinq ans, avec recueil des données via un questionnaire à paramètres quantitatifs et qualitatifs. Résultats : 101 sujets furent inclus, dont 54 sujets hémodialysés, 15 sujets dialysés péritonéaux et 32 sujets greffés, avec un âge moyen de 62,4 ans. La ressource la plus sollicitée était le néphrologue à 52,5%. Plus d’un tiers des sujets (35,6%) n’ont cherché d’information nulle part. Internet est une ressource plus sollicitée que le médecin généraliste (17,8% contre 8,9%). Malgré le peu d’informations demandées au médecin généraliste, il reste l’une des ressources en qui les sujets ont le plus confiance (74,3%). Les sujets ayant choisi la dialyse péritonéale ou la transplantation avaient vu plus fréquemment leur néphrologue durant l’année pré-suppléance (p = 0,001). Conclusion : une amélioration de la coordination et de la communication entre néphrologue et médecin généraliste semble essentielle pour l’amélioration du parcours de soins des insuffisants rénaux, en particulier au moment du choix du traitement de suppléance.

Book excerpt: La réforme ma santé 2022 prévoit un financement pour le parcours de la maladie rénale chronique (MRC) pré-terminale avec des objectifs de pertinence et d'efficience impliquant une approche plus globale basée sur les besoins des patients. La période de transition au cours de laquelle le patient doit choisir une des techniques de suppléance rénale est une étape particulièrement délicate. Le but de cette étude était d'évaluer l'impact réel du parcours de soins MRC pré-terminale sur le choix de traitement de suppléance et les indicateurs de qualité de prise en charge. Nous avons réalisé une étude observationnelle rétrospective monocentrique. Au total, 483 patients ayant reçu une information sur les techniques de suppléance rénale entre 2014 et 2018 dans le service de néphrologie de l'hôpital Édouard Herriot ont été inclus. Les données médicales, sociales, les paramètres de suivi néphrologique et la mortalité ont été recueillis sur une période de suivi minimale de 2 ans. Lors de l'information pré-suppléance, 35 patients présentaient une insuffisance rénale aigue (IRA) et 448 présentaient une IRC. Parmi les patients en IRC, 352 patients (79%) avaient bénéficié d'un suivi néphrologique préalable de plus de 3 mois. Les patients ayant suivi un parcours de soins néphrologique optimal avaient été suivis de façon prolongée (médiane de 34 mois (EI :15-53)) et rapprochée avec une médiane de 2 consultations dans l'année avant l'information (EI :1-3) mais près d'un tiers d'entre eux avaient été hospitalisés en néphrologie au moins une fois cette année-là. Le choix prédominant réalisé par les patients à l'issue de la première consultation d'information pré-suppléance était l'hémodialyse (50%). La dialyse péritonéale et le traitement conservateur n'ont été choisis que par respectivement 24% et 10% des patients et 16% des patients sont resté indécis. Les paramètres de prise en charge néphrologique ne semblaient pas influencer vers la réalisation d'un choix en particulier. Malgré le suivi d'un parcours de soins optimal, parmi les patients ayant initié un traitement de suppléance, seulement 17% ont débuté la dialyse péritonéale, 20% ont débuté la dialyse en urgence, 36% ont débuté l'hémodialyse sur cathéter et 20% étaient inscrits sur liste de greffe à l'initiation de la dialyse. Au total, 32% des patients sont décédés pendant la période de suivi. La durée médiane entre information pré-suppléance et décès était de 20 mois (EI :10-38). Nous confirmons que le suivi d'un parcours de soins néphrologique optimal offre de meilleurs indicateurs de prise en charge. Ceux-ci restent cependant perfectibles. La prise en compte de la littératie du patient pour adapter la stratégie d'information pré-suppléance semble être indispensable. L'amélioration de la connaissance qu'ont les patients de leur maladie et de sa prise en charge pourrait accroitre le choix de techniques de domicile et diminuer le nombre d'initiations de la dialyse en urgence. La réalisation d'études qualitatives centrées sur l'expérience des patients et de leurs aidants dans le processus décisionnel pourrait permettre d'améliorer notre prise en charge.

La iatrogénie constitue un problème majeur dans la prise en charge de ces patients, auquel les médecins généralistes sont confrontés au quotidien. Objectif : L'objectif principal de l'étude était de décrire les sources d'informations utilisées par les médecins généralistes pour l'adaptation des traitements selon la fonction rénale de leurs patients. Les pratiques et les difficultés des médecins dans ce domaine étaient également étudiées. Méthode : Il s'agit une étude épidémiologique descriptive observationnelle transversale, réalisée par le biais d'un questionnaire transmis entre Mars et Juillet 2017 aux médecins généralistes installés en Savoie. Résultats : Quatre-vingt-cinq réponses ont été recueillies, dont quatre-vingt étaient analysables. La source d'information la plus utilisée par les médecins était le Vidal® (78,8%). Etaient ensuite cités la Base Claude Bernard® (20%) et le néphrologue (16,3%). 96,3% des médecins rencontraient des difficultés lors de la prescription chez les patients insuffisants rénaux chroniques, pour des médicaments fréquemment rencontrés en médecine générale. Conclusion : Malgré des sources d'informations multiples, la prescription de médicaments chez les patients insuffisants rénaux chroniques reste encore un problème très fréquent pour les médecins généralistes. Les perspectives d'améliorations passent notamment par la formation, l'information et une optimisation du contrôle et de l'aide à la prescription.

Book excerpt: La maladie rénale chronique (MRC) correspond à des dommages structurels des reins qui conduisent à une réduction progressive et anormale des fonctions rénales. Ces fonctions d'épuration, de régulation et de sécrétion d'hormones sont vitales et leurs dégradations peuvent être fatales. Cette maladie s'aggrave de façon inéluctable à travers cinq stades et est irréversible. Une fois atteint le stade 5 ou « terminal » de la MRC (insuffisance rénale terminale, IRT), il est nécessaire de mettre en place un traitement de suppléance : la dialyse ou la transplantation d'un nouveau rein. On parle alors d'IRT traité (IRTT). Alors que la MRC à différents stades pourrait toucher 5 à 10% de la population française adulte, et comprenait près de 90 000 patients en IRTT en 2018, cette maladie est rare au sein de la population pédiatrique, avec environ 930 enfants en IRTT en 2018. Cependant, ses conséquences peuvent être majeures puisqu'elle impacte le développement physique et psychosocial de l'enfant (retard de croissance, dénutrition, problèmes cognitifs...). Ainsi, les objectifs des soignants, personnels de santé et chercheurs sont de ralentir la progression de la maladie vers le stade terminal, et une fois atteint, d'optimiser les traitements de suppléance pour assurer la meilleure prise en charge possible et une meilleure qualité de vie aux patients. Dans ce contexte, étudier les inégalités sociales de santé peut se révéler pertinent pour aider à l'élaboration d'interventions mieux ciblées à chaque population. Cependant, chez l'enfant, le lien entre des facteurs sociaux comme le revenu, le niveau d'éducation ou encore le contexte familial et la MRC n'est que très peu exploré. A notre connaissance, aucune étude n'a été réalisée en Europe sur l'impact de la défavorisation sociale sur le parcours de soins, les modalités et les résultats de prise en charge à différents stades de la MRC pédiatrique. Or, en Europe et en France plus particulièrement, le système de santé mis en place mais également les aides sociales proposées ne sont pas les mêmes qu'aux Etats-Unis ou dans les pays à ressources limitées dans lesquels des associations ont été mises en évidence. Il est donc important d'évaluer si les résultats observés dans ces pays sont superposables à la France pour adapter la prise en charge des enfants et adolescents. L'objectif de cette thèse est donc d'étudier les inégalités sociales de santé dans la MRC pédiatrique et plus précisément d'évaluer l'association entre la défavorisation et l'évolution de l'IRTT chez des enfants et adolescents. Pour répondre à cet objectif, nous avons utilisé des données de patients âgés de moins de 20 ans entre 2002 et 2015, issues du registre national REIN, recueillant les informations de tous les patients initiant un traitement de suppléance par dialyse ou greffe rénale. En l'absence d'indicateurs individuels, la défavorisation sociale a été estimée à l'aide d'un indicateur écologique appliqué à l'adresse de résidence des enfants, l'European Deprivation Index, score continu catégorisable en cinq groupes : le quintile 1 correspondant aux zones géographiques les plus favorisées et le quintile 5 aux zones les plus défavorisées du pays. Nous avons ainsi étudié l'association entre l'EDI et 1) l'incidence de l'IRTT pédiatrique en France, 2) des indicateurs de prise en charge et conditions cliniques à l'initiation de la dialyse, puis 3) la survie du greffon et du patient chez les jeunes transplantés rénaux. Les résultats principaux de cette thèse sont que, chez les patients issus de milieux les plus défavorisés, l'incidence de l'IRTT était plus élevée, le traitement de suppléance était plus tardif, plus souvent effectué en urgence et en hémodialyse et que l'échec de transplantation rénale état plus fréquent. Nous pensons que l'étude de cette association avec différents critères de l'IRTT est importante pour proposer des prises en charge ciblées pour cette population.

Despite their growing importance, semi-authoritarian regimes have not received systematic attention. Marina Ottaway examines five countries (Egypt, Azerbaijan, Venezuela, Croatia, and Senegal) which highlight the distinctive features of semi-authoritarianism and the special challenge each poses to policymakers. She explains why the dominant approach to democracy promotion isn't effective in these countries and concludes by suggesting alternative policies. Marina Ottaway is senior associate and codirector of the Democracy and Rule of Law Project at the Carnegie Endowment.

According to Burton, Victorian and Edwardian feminists such as Josephine Butler, Millicent Garrett Fawcett, and Mary Carpenter believed that the native women of colonial India constituted a special 'white woman's burden.' Although there were a number of prominent Indian women in Britain as well as in India working toward some of the same goals of equality, British feminists relied on images of an enslaved and primitive 'Oriental womanhood' in need of liberation at the hands of their emancipated British 'sisters.' Burton argues that this unquestioning acceptance of Britain's imperial status and of Anglo-Saxon racial superiority created a set of imperial feminist ideologies, the legacy of which must be recognized and understood by contemporary feminists.
