Qu'apporte alors l'hospitalisation en urgence en service psychiatrique adulte ? Après avoir repris le processus adolescent et les principaux troubles psychiatriques, nous abordons le parcours de soins de l'adolescent en décrivant les différents lieux d'accueil existants pour répondre à la nécessité de prise en charge en urgence. La dernière partie de notre travail reprend les résultats d'une étude rétrospective réalisée sur le secteur adulte 75G13 de l'Hôpital Sainte Anne décrivant les caractéristiques d'une population d'adolescents hospitalisés sur l'année 2015, âgés de 15 ans et demi à 18 ans, adressés par les services d'urgence. Nous discuterons l'intérêt de l'hospitalisation en milieu adulte, souvent préconisée par défaut en l'absence de disponibilité de structures spécifiques en urgence. La mise en place du projet de soins individualisé du jeune, en concertation avec son milieu (familial ou éducatif) apparaît alors essentielle afin de maintenir une continuité de la prise en charge, favoriser la rémission et l'insertion sociale et scolaire ou professionnelle.

Méthodes : dans une étude menée du 1er Mai au 31 juillet 2018, au sein du Pôle de psychiatrie centre est Vaucluse, les caractéristiques socio-démographiques et le parcours de soins des patients hospitalisés en urgence, ainsi que l'offre de soins ont été relevé. Résultats : les principaux facteurs associés à une hospitalisation en urgence en psychiatrie sont le statut civil, un âge supérieur à 42 ans, l'isolement, l'absence d'activité professionnelle, les antécédents d'hospitalisation psychiatrique et de passage à l'acte, la présence d'un trouble psychiatrique avéré, une demande initiale du patient, l'horaire et la durée de la consultation et le nombre de places disponibles sur l'hôpital. Discussion : ainsi, la décision d'hospitaliser dépend, autant, de la nature des symptômes présentés par le patient et de ses caractéristiques socio-démographiques, que de facteurs imputables au psychiatre examinateur et à l'offre de soins.

Il est central de déterminer la gravité de cette crise et de mettre en place les modalités de prise en charge adaptées. C'est pourquoi les adolescents doivent avoir accès à un service de consultation spécialisé réactif. Les données actuelles montrent que de nombreux systèmes de santé sont en difficulté face à cette situation et les mesures prises en réponse restent variables et peu consensuelles. Dans un contexte de virage ambulatoire global dans le système de soin, le dispositif départemental réactif pour adolescents (DDRA31) a été mis en place progressivement depuis 2017 en Haute-Garonne. Il a pour vocation de permettre aux adolescents en crise un accès rapide à une consultation spécialisée et un accompagnement de sortie de crise d'une durée inférieure à trois mois. Dans ce contexte, un projet d'évaluation mixte quantitative-qualitative de ce dispositif a été élaboré. Nous en présentons ici la partie quantitative. L'objectif de cette évaluation était de mettre en évidence un effet de la mise en place du dispositif sur les consultations et hospitalisations d'adolescents, pour motif ou diagnostic psychiatrique, aux urgences du CHU de Toulouse. Méthodes : notre étude consistait en une analyse rétrospective selon un schéma d'étude quasi-expérimentale comparant des données avant/après issues des services d'urgences adulte et enfant du CHU de Toulouse. L'extraction des données a permis de caractériser les passages aux urgences des adolescents âgés de 12 à 17 ans révolus, résidant en Haute-Garonne, sur la période allant de 2014 à 2019. Le critère d'évaluation principal analysé était le taux d'incidence annuelle de passage aux urgences pour motif ou diagnostic psychiatrique. Les critères secondaires étaient le taux d'incidence annuelle d'hospitalisation en service psychiatrique et l'étude des motifs, diagnostics, et modes de sortie associés aux passages aux urgences des adolescents. L'analyse était descriptive dans un premier temps puis s'attachait à caractériser l'impact du dispositif étudié par une étude des taux d'incidence annuelle de passage aux urgences et d'hospitalisation ainsi que par une modélisation du risque d'hospitalisation par régression logistique à effets mixtes. Résultats : 6686 passages aux urgences pour motif ou diagnostic psychiatrique, réalisés par 4245 adolescents ont été étudiés entre 2014 et 2019. Le taux d'incidence annuelle de passage aux urgences pour motif ou diagnostic psychiatrique est resté stable au cours de l'étude, passant de 14,0 (IC 95 % : 13,2-14,9) pour cent adolescents, en 2014, à 13,9 (IC 95 % : 13,2-14,7), pour cent adolescents, en 2019. Les filles étaient sur-représentées dans notre échantillon (60,0%), de même que pour les adolescents âgés de 15 à 17 ans (62,7%). L'analyse des taux d'incidence annuelle d'hospitalisation en service psychiatrique après passage aux urgences pour motif ou diagnostic psychiatrique montrait une diminution de ce dernier, à partir de l'année 2017 inclue, passant de 38,3 (IC 95% : 34,1-42,8) pour cent adolescents, en 2014 à 24,2 (IC 95% : 21,2-27,4) pour cent adolescents, en 2019. La modélisation du risque d'hospitalisation en service psychiatrique après passage aux urgences pour motif ou diagnostic psychiatrique a mis en évidence un effet protecteur de l'année à partir de 2017 inclue avec un rapport de côtes passant de 0,63 (IC 95 % : 0,49-0,80), en 2017 à 0,52 (IC 95 % : 0,41-0,67), en 2019. Conclusion : aucun effet du dispositif n'a été mis en évidence sur l'incidence annuelle de passage aux urgences pour motif ou diagnostic psychiatrique, entre 2014 et 2019. En revanche, notre étude a montré un impact possible de l'implantation du DDRA31 sur la réduction de 37 à 48% du risque d'hospitalisation en psychiatrie d'adolescents. Une prolongation de l'étude pourrait être intéressante pour disposer de plus de temps de recul post mise en place du dispositif.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
