Book excerpt: The Patient Protection and Affordable Care Act (ACA) includes two provisions with significant implications for the adequacy of the supply of primary care physicians (PCP) serving Medicaid enrollees. First, the ACA extends Medicaid eligibility to nearly everyone under age 65 up to 133% of the federal poverty level ($14,484 for an individual or $29,726 for a family of four in 2011). The Congressional Budget Office has estimated that 16 million people, mostly adults, will gain Medicaid coverage as a result of this expansion; another 16 million people are projected to gain private coverage through insurance exchanges established by the new law. Second, for 2013 and 2014, the ACA raises Medicaid payment rates for primary care services delivered by PCPs, to Medicare's payment levels for the same services. While access to primary care among adult Medicaid beneficiaries is generally good, the entry of 32 million newly insured people into the health care system will intensify competition among patients and payers for scarce primary care resources. To help inform policy to ensure adequate access to primary care for Medicaid enrollees, this study provides an assessment of which PCPs are most likely to respond to the changes under health reform by serving additional Medicaid beneficiaries, and it profiles important aspects of their practices.

Given the unwillingness of many primary care physicians (PCPs) to treat new Medicaid patients, policy makers and others are concerned about adequate primary care capacity to meet the increased demand. States with the smallest number of PCPs per capita overall - generally in the South and Mountain West - potentially will see the largest percentage increases in Medicaid enrollment, according to a new national study by the Center for Studying Health System Change (HSC). In contrast, states with the largest number of PCPs per capita - primarily in the Northeast - will see more modest increases in Medicaid enrollment. Moreover, geographic differences in PCP acceptance of new Medicaid patients reflect differences in overall PCP supply, not geographic differences in PCPs' willingness to treat Medicaid patients.

Book excerpt: Medicaid plays a vital role in the U.S. health care system and the importance of office-based physician practices to the care of Americans served cannot be overstated. Despite increases in program enrollment, a number of sources report that the proportion of U.S. office-based physicians willing to accept new Medicaid patients is decreasing. This discovery is significant. This managerial accounting study utilizes a national survey of practicing physicians to explore this conundrum from the physicians' perspective. Specifically, the impact of organizational-related determinants including (1) practice characteristics, (2) compensation and (3) performance measures is examined on physicians' propensity to accept new Medicaid patients. With an empirical analysis using the cumulative odds ordinal logit model, this study breaks new ground in several significant venues. Only a limited number of studies have yet examined physician practice characteristics relative to physician acceptance of Medicaid, but these were far more limited in scope. In addition, this study offers a first look at how financial and non-financial incentives collectively impact patients' access to Medicaid. Finally, it is important to point out that this study provides for a comprehensive look at understanding what organizational characteristics help to determine the probability of physician acceptance of new Medicaid patients.The most significant factors found to be associated with diminished propensity for Medicaid access include: operating as a primary care doctor, physician ownership in the practice, practice size, low relative geographic reimbursement, supportive clinical IT and gender. Knowing what factors are associated with physicians' decisions should provide opportunities for potential public policy solutions.
