Using a behavioral experiment with a representative sample of German primary-care physicians, we study the incentive effect of performance pay on service provision and quality of care. To explain heterogeneity in quality of care, we link administrative data on practice characteristics and survey data on physicians' attitudes with experimental data. We find that under performance pay quality increases by about 7pp compared to baseline capitation. While the effect increases with the severity of illness, the bonus level does not significantly affect the quality of care. Data linkage indicates that primary-care physicians in high-profit practices are most responsive to incentives and provide low quality of care, a relationship which seems to be driven by physicians practicing in suburban areas. Physicians' general altruism and regard for the health benefit of patients are significant drivers of a high quality of care.

The fate of P4P is not yet certain, but we can learn a number of lessons from experiences with P4P to date, and ways to improve the designs of P4P programs are becoming apparent. Book excerpt: To test the contention that the capitation system, coupled with "bonus" payments for reduced hospitalization, would lead to less use of hospitals, CHEPA researchers conducted a comparison betwen Ontario general practioners in Health Service Organizations (HSO) (paid on this capitation basis) and those remunerated under the traditional fee-for-service system. Results from the investigation showed no differences in the rates of hospitalizations for the two groups. In the study, the rates at which 39 HCO physicians admitted patients to hospital was compared with that of 77 doctors being paid on a fee-for-service basis. The two groups of doctors were matched as closely as possible on a number of variables such as age, sex, years in practice, and geographic location. Hospitalization rates for patients were compared at three time periods a total of five years apart. The study found there were no statistically significant differences between the rates at which the two groups of doctors admitted patients to hospital during any of the three time periods. Between the first time period measured and the last, the mean rate of hospital separations per 100 population (a statistical measure of hospital use) fell by 9.2 per 1000 in capitated practices and 10 per 1000 in fee-for-service practices.
