I have just mentioned that in two cases lately, death followed vaccination, apparently from pyaemia. A case of death following a hypo dermic injection is just recorded. A lady who had fallen into the habit of injecting morphine to relieve sickness to which she was very lia ble, was seized with tetanus and died. It transpired that she was scarred with marks of old injections, as well as bearing marks Of recent origin and in a wardrobe were found a number of phials Of solution of morphine, an injection-syringe, and several rusty steel needles. The use of the latter led to the un fortunate consequences. This case teaches another lesson, viz., the spreading habit of secretly resorting to the hypodermic use of morphia, -the most dangerous Of all forms of narcotic indulgence. This gentleman, who deservedly holds a promi nent reputation, took charge Of her case, and for ten months she submitted to a series Of Operations productive Of no good result whatever. He admitted, when pressed, sev eral attacks Of gonorrhoea in earlier days, and also admitting a sound into his urethra a breakwater was encountered two inches from the meatus, and another four inches farther on. These passed, navigation was uninterrupted. He, of course, had diffi culty in urinating, and after proper ex planation submitted to dilatation Of the strictures. Eleven months after curing the gentleman the imaginary cervical strie ture Of his estimable wife was thoroughly dilated by a fine boy, who is to-day well and growing nicely. Joffroy's thesis upon this subject, which has been analyzed by Le Progres for September 14, we extract the following facts. After a physiological introduction, including an account of the part played by the skin in regulating the bodily heat, the author studies the effects upon the circulation of cutaneous stimulation. These effects may be local or general, the former consisting in vaso-motor phenomena, constriction and dilatation of the vessels at the point excited. They are due to a reflex action of the special vaso motor centres, and probably also to an action of the periph eral nervous ganglia, as yet not perfectly demonstrated. The general effects are more complex, and the results of different investigations are contradictory in some points. The following, however, may be asserted. The nature of the stimulus makes but little difference; its extent and inten sity, however, are important. After feeble stimulation, acceleration of the general circulation, together with a manifest con striction of the arteries, and even of the veins, is observed. So soon as the excita tion is withdrawn, the vessels return rapidly to their normal condition. When a feeble stimulation is continued for a long time, constriction of the arteri oles is induced which may last for hours. (such is the effect of local applications of moderate cold, astringents, etc., etc.) Energetic stimulation produces at first acceleration of the blood-current, with marked constriction in the calibre of the vessels: these phenomena rapidly giving place to slacking in circulation, with dila tation of the vessels. After very violent stimulation, the preliminary stage of con traction is not noticed, dilatation of the vessels being produced instantaneously and becoming excessive. Such are the re sults of experiments upon animals. With man, M. Joffroy thinks, the phenomena are more simple, and are confined to con traction Of the vessels and increased rapid ity of circulation.
