Book excerpt: Background and AimsPatent ductus arteriosus (PDA) is a potentially harmful condition in the youngest preterm infants. Ibuprofen is often used to treat PDA. The optimal dosing regimen of ibuprofen remains debatable since information on target exposure and pharmacology in this population is limited. This study aims to develop a population pharmacokinetic model for ibuprofen to improve the treatment of PDA in preterm infants. MethodSeventy-three neonates born after 24 to 32 weeks gestation that received ibuprofen treatment for PDA closure were included in a prospective study. Median gestational age (GA) was 26.1 weeks (24.0-30.1) and median postnatal age (PNA) at treatment initiation was 3 days (1-12). 218 plasma samples were collected in which either R-ibuprofen, S-ibuprofen or both could be quantified. Using NONMEM software, a model was developed to describe the pharmacokinetics of ibuprofen. ResultsOn the basis of combined R and S ibuprofen data a one-compartment model for total ibuprofen was developed. GA and PNA were found to be the best predictors for clearance maturation (p
