Les conséquences obstétricales (avortement spontané, retard de croissance intra-utérin) et les complications médicales (anémie, infections : maladies sexuellement transmissibles, hépatite B et C, VIH...) sont des problèmes fréquemment rencontrés chez la mère dépendante. L'héroïne ne semble pas tératogène, mais traverse la barrière placentaire et entraine chez le foetus un phénomène de dépendance. Des études ont montré que le sevrage aux opiacés s'avérait être une contre-indication majeure chez la femme gravide parcequ'il entraine un risque de menace d'accouchement prématuré chez la mère et une souffrance voire une mort foetale. Il apparait donc qu'un traitement de substitution s'impose, la méthadone semblerait être actuellement le traitement le plus approprié pour mener à son terme ce type de grossesse dans les conditions optimales de sécurité. Toutefois ce traitement est indissociable d'une prise en charge médico-psycho-sociale.
