Book excerpt: Once considered reserved for life-threatening disease or illness, emergency departments in the United States are now described as the primary care clinic and the social work department for many Americans (Grumback, Keane & Bindman, 1993). A 1992 study found 2 million children in the United States routinely used the emergency department as their primary source of care (Halfon, Newacheck, Wood & Peter, 1996). Studies have shown 50-90% of all patients presenting to emergency departments have minor complaints which could easily be treated in primary care settings (Dowling & Dudley, 1995; Grumbach et al., 1993). The non-urgent use of emergency departments has led to overcrowding, long waiting times and patient dissatisfaction (Grumbach et al., 1993; Derlet & Nisho, 1990, Liggins, 1993). Overcrowding and inappropriate use of the emergency department also interferes with the care of critically ill patients (Dowling & Dudley, 1995). Civilian hospitals have developed various strategies to address patients with minor complaints varying from outright refusal of care, to triaging to off-site clinics, developing non-urgent care clinics and establishing Fast Track Clinics also known as Minor Emergency Areas (MEAs) within existing emergency departments. These Fast Track clinics have been staffed with a variety of non-physician, mid-level health care providers including nurse practitioners.

Emergency department nurses work in this fast-paced, high-stress environment and are expected to know a vast amount of knowledge to treat injuries and manage illness for any age group. Emergency department nurses are frequently faced with unique challenges that requires utilization of other members of the healthcare team to provide high quality care. Educating the public to prevent injury and promote wellness is a key role for emergency departments that must be supported by interprofessional collaborative practice team members such as nurses, physicians, pharmacists, social workers, emergency medical services, respiratory therapists, physical therapists, behavioral health, and athletic trainers. Understanding and respecting each professions’ scope of practice may promote communication that may translate into value-based outcomes such as higher satisfaction scores and quality-driven transition of care. Interprofessional collaborative practice is essential for various healthcare providers to communicate their perspectives when delivering care to individual patients, families, and communities. The purpose of this Doctor of Nursing Project was two-fold. First to assess the emergency department nurse’s knowledge and perception of the role of athletic trainers for sports-related injuries through an on-line survey of more than 300 emergency nurses which demonstrated that knowledge deficits exist. The second purpose championed athletic trainers as valued members of the interprofessional healthcare team for enhanced sports injury care and seamless transition of care for both admission and discharge from the emergency department.

There were limited studies dealing with the practice of teamwork between nurses and physicians in the Emergence Departments (EDs). Aims: Descriptive aim of the study was to examine differences between staff in the Interventional and Control Groups on perception of staff teamwork. The exploratory aim was to examine staff perception of job satisfaction, work environment, autonomy, and control over practice. Design: The Interventional Group comprised four EDs that participated in teamwork training and operationalized its principles in their EDs. Control Group EDs comprised four EDs which did not participate in the training. Survey questionnaires were used for data collection. Setting and Participants: Staff from four Interventional and four Control Group EDs throughout California participated in the study. There were 191 participants from the Interventional Group EDs and 307 from the Control Group EDs. Main Outcome Measures: Differences between staff who worked in the Interventional Group EDs and staff who worked in the Control Group EDs on perception of teamwork, job satisfaction, work environment, autonomy, and control over practice were assessed.
