Book excerpt: It has been a consistent finding in the literature on patients with schizophrenia that poor medication adherence precipitates relapse of psychiatric symptoms. One of the most consistent findings in the research is that patients diagnosed with schizophrenic disorders who have close ties to family members that are critical of them tend to have much higher rates of relapse. The question of how affectionate and/or supportive family relationships impact psychiatric stability remains. This dissertation examined the connection between perception of family relationships and medication adherence among patients with schizophrenia. Utilizing the Rutgers Hospital and Community Survey, this study looked at the follow up surveys of 182 patients who were on oral anti-psychotic medications and who reported at least one family relationship. Data were analyzed using the patients' responses to questions about how they perceive their family relationships as well as their responses about their medication adherence in the previous two weeks and the previous three months. Chi-square and logistic regression were performed, while controlling for age, race, education, gender, Brief Psychiatric Rating Scale scores, Global Assessment Scale scores, side effects, substance abuse, therapeutic alliance, housing status, and objective family involvement scores (which measured the family's involvement behaviors while the patient had been hospitalized). The main hypothesis, that patients who perceive their families to be affectionate and/or high in instrumental support would have higher rates of adherence, was not supported. An objective measure of family involvement (which measured family behaviors such as visitation and participation in treatment when the patient was in the hospital) also did not correlate with medication adherence. This analysis did find that having a substance abuse diagnosis was a predictor of poor medication adherence and that a positive therapeutic alliance, when measured at the three month follow up, was a predictor of medication adherence. Limitations of this follow up treatment study include questionable validity and reliability of self report on both family relationships and medication adherence, loss of statistical power as variables were added into the regression, and possible memory problems related to the diagnosis of schizophrenia since both the illness and possibly the medication regimen may impact memory. However, it is possible, since the objective family involvement score also did not yield a relationship to medication adherence, that positive family relationships do not translate into medication taking behavior.

Individuals who suffer from schizophrenia struggle with adhering to their medication treatment plan. Non-adherence to their prescribed medication may cause individuals to decompensate; their symptoms may worsen and hospitalization may be needed. Sources of Data Three databases were searched to evaluate studies on the effectiveness of individual and family group psychoeducation on medication adherence. PsycINFO, CINAHL, and PubMed were used for the systematic review of literature. The search terms used were: schizophrenia, psychoeducation, caregiver support, family support, and medication adherence. Seven articles were retrieved from the literature search, and met the inclusion criteria for the systematic review. Inclusion criteria consisted of the studies testing the effectiveness of psychoeducation with families and patients. Individuals diagnosed with schizophrenia, schizoaffective disorder, or unspecified psychotic disorder was included. The studies selected were randomized control trials, systematic reviews and meta-analyses. Conclusions Reached Studies support patient and family psychoeducation as an effective intervention to improve the patient's adherence to the medication adherence. The goal of psychoeducation is to help families identify negative and positive symptoms of schizophrenia, encourage the patient to seek psychiatric treatment, and adhere to the prescribed medication regimen. In addition, the family and the patient learn how to cope with the symptoms of schizophrenia and have greater understanding of the patient's illness. Overall, family and patient participation in psychoeducation improves understanding of the disease process and ultimately the patient's quality of life.

Book excerpt: Medication non-adherence for individuals with schizophrenia spectrum disorders is a problem with serious consequences to personal health, family relationships, and the community at large. Occupational therapists in mental health facilitate medication management strategies and need to be informed in order to determine best practice approaches. This study utilized a qualitative multiple case-study approach to examine the lived experiences of four consumers and four families relative to psychiatric medication adherence, social support, and recovery. In-depth, semi-structured interviews and brief recovery surveys were completed. Results suggest that consumers in this study incorporated medications as a process that illustrated development of self-awareness. Consumers' desire for greater autonomy in early adulthood emerged as a powerful influence on their sense of liminality. Parents articulated that medications were necessary for their adult children's illnesses, and that decisions about how to support their adult children were complicated. The findings of this study have important implications for occupational therapists in the delivery of client-centered mental health care to support individuals and their families with medication adherence.

Book excerpt: Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

Book excerpt: Several contributions in our first book about schizo affective disorders (Marneros and Tsuang, Schizoaffective Psychoses, Springer-Verlag, 1986) supported the assumption that schizoaffective disorders differ in relevant ways from schizophrenic disorders. The classification of schizo affective disorders as a subgroup of schizophrenia has also been criticized, and empirical research in clinical, genetic, therapeutic, and prognostic areas supports the idea that there are some strong similari ties between schizo affective and affective disorders. Of course, there are not only similarities between these two groups, but also differences just as there are between schizo affective and schizophrenic disorders. It is precisely the existence of similarities and differences between schizo affective disorders and the other two so-called typical mental disorders, i.e., schizophrenia and affective disorders, which makes them a challenge in psychiatric research, a challenge to the traditional dichotomy in the classification of disorders which originated with Kraepelin. This challenge is certainly proving fruitful in psychiatric research. These "cases in between" may well demonstrate that sep arating, dividing, and limiting is not always meaningful. Sometimes it can be more meaningful to unify; to unify in the sense of building bridges between typical groups. It is here that the assumption of a "psychotic continuum" can become relevant, and the investigation of schizoaffective disorders is of prime importance in research on a possible continuum of psychosis.

The mean total Q8 scores were 4.83 u00b1 3.1. The KAST total score was correlated with the Q8 (p = 0.001). The nonadherence rate was 43.8%. The medication-adherent patients had Higher score of Q8 (p = 0.0001) and higher KAST subscore of cause (p = 0.006). These two variables explained 38.9% of the variance in adherence behavior, with Q8 being the mediator. uf076tConclusion: This study highlights the importance of patientsu2019 understanding of etiology of the illness and its relationship with insight and medication adherence.
