Available in PDF, EPUB and Kindle. Book excerpt: Over the past 15 years, a series of empirical studies in different countries have shown that our increasing genetic knowledge leads to new forms of exclusion, disadvantaging and stigmatization. The spectrum of this "genetic discrimination" ranges from disadvantages at work, via problems with insurance policies, to difficulties with adoption agencies. The empirical studies on the problem of genetic discrimination have not gone unnoticed. Since the beginning of the 1990s, a series of legislative initiatives and statements, both on the national level and on the part of international and supranational organizations and commissions, have been put forward as ways of protecting people from genetic discrimination. This is the first book to critically evaluate the empirical evidence and the theoretical usefulness of the concept of "genetic discrimination." It discusses the advantages and limitations of adopting the concept, and offers a more complex account distinguishing between several dimensions and forms of genetic discrimination.

Available in PDF, EPUB and Kindle. Book excerpt: This volume offers interdisciplinary perspectives on contemporary biomedicine as a cultural practice. It brings together leading scholars from cultural anthropology, sociology, history, and science studies to conduct a critical dialogue on the culture(s) of biomedical practice, discussing its epistemic, material, and social implications. The essays look at the ways new biomedical knowledge is constructed within hospitals and academic settings and at how this knowledge changes perceptions, material arrangements, and social relations, not only within clinics and scientific communities, but especially once it is diffused into a broader cultural context.

Book excerpt: This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

Book excerpt: This handbook covers Electronic Medical Record (EMR) systems, which enable the storage, management, and sharing of massive amounts of demographic, diagnosis, medication, and genomic information. It presents privacy-preserving methods for medical data, ranging from laboratory test results to doctors’ comments. The reuse of EMR data can greatly benefit medical science and practice, but must be performed in a privacy-preserving way according to data sharing policies and regulations. Written by world-renowned leaders in this field, each chapter offers a survey of a research direction or a solution to problems in established and emerging research areas. The authors explore scenarios and techniques for facilitating the anonymization of different types of medical data, as well as various data mining tasks. Other chapters present methods for emerging data privacy applications and medical text de-identification, including detailed surveys of deployed systems. A part of the book is devoted to legislative and policy issues, reporting on the US and EU privacy legislation and the cost of privacy breaches in the healthcare domain. This reference is intended for professionals, researchers and advanced-level students interested in safeguarding medical data.

Available in PDF, EPUB and Kindle. Book excerpt: Featuring a timely presentation of total survey error (TSE), this edited volume introduces valuable tools for understanding and improving survey data quality in the context of evolving large-scale data sets This book provides an overview of the TSE framework and current TSE research as related to survey design, data collection, estimation, and analysis. It recognizes that survey data affects many public policy and business decisions and thus focuses on the framework for understanding and improving survey data quality. The book also addresses issues with data quality in official statistics and in social, opinion, and market research as these fields continue to evolve, leading to larger and messier data sets. This perspective challenges survey organizations to find ways to collect and process data more efficiently without sacrificing quality. The volume consists of the most up-to-date research and reporting from over 70 contributors representing the best academics and researchers from a range of fields. The chapters are broken out into five main sections: The Concept of TSE and the TSE Paradigm, Implications for Survey Design, Data Collection and Data Processing Applications, Evaluation and Improvement, and Estimation and Analysis. Each chapter introduces and examines multiple error sources, such as sampling error, measurement error, and nonresponse error, which often offer the greatest risks to data quality, while also encouraging readers not to lose sight of the less commonly studied error sources, such as coverage error, processing error, and specification error. The book also notes the relationships between errors and the ways in which efforts to reduce one type can increase another, resulting in an estimate with larger total error. This book: • Features various error sources, and the complex relationships between them, in 25 high-quality chapters on the most up-to-date research in the field of TSE • Provides comprehensive reviews of the literature on error sources as well as data collection approaches and estimation methods to reduce their effects • Presents examples of recent international events that demonstrate the effects of data error, the importance of survey data quality, and the real-world issues that arise from these errors • Spans the four pillars of the total survey error paradigm (design, data collection, evaluation and analysis) to address key data quality issues in official statistics and survey research Total Survey Error in Practice is a reference for survey researchers and data scientists in research areas that include social science, public opinion, public policy, and business. It can also be used as a textbook or supplementary material for a graduate-level course in survey research methods.

Book excerpt: A new eye-opener on how we can make better decisions—by the author of Gut Feelings In this age of big data we often trust that expert analysis—whether it’s about next year’s stock market or a person’s risk of getting cancer—is accurate. But, as risk expert Gerd Gigerenzer reveals in his latest book, Risk Savvy, most of us, including doctors, lawyers, and financial advisors, often misunderstand statistics, leaving us misinformed and vulnerable to exploitation. Yet there’s hope. In Risk Savvy, Gigerenzer gives us an essential guide to the science of good decision making, showing how ordinary people can make better decisions for their money, their health, and their families. Here, Gigerenzer delivers the surprising conclusion that the best results often come from considering less information and listening to your gut.

Book excerpt: Anthropologists have written a great deal about the coastal adaptations and seafaring traditions of Pacific Islanders, but have had much less to say about the significance of rivers for Pacific island culture, livelihood and identity. The authors of this collection seek to fill that gap in the ethnographic record by drawing attention to the deep historical attachments of island communities to rivers, and the ways in which those attachments are changing in response to various forms of economic development and social change. In addition to making a unique contribution to Pacific island ethnography, the authors of this volume speak to a global set of issues of immense importance to a world in which water scarcity, conflict, pollution and the degradation of riparian environments afflict growing numbers of people. Several authors take a political ecology approach to their topic, but the emphasis here is less on hydro-politics than on the cultural meaning of rivers to the communities we describe. How has the cultural significance of rivers shifted as a result of colonisation, development and nation-building? How do people whose identities are fundamentally rooted in their relationship to a particular river renegotiate that relationship when the river is dammed to generate hydro-power or polluted by mining activities? How do blockages in the flow of rivers and underground springs interrupt the intergenerational transmission of local ecological knowledge and hence the ability of local communities to construct collective identities rooted in a sense of place?

A special focus on percutaneous coronary interventions is evident, from chapters on pharmacotherapy, its relevance to 30-day mortality and the CANADA score, besides the treatment of patients with saphenous vein graft disease. Other treatment protocols addressed include therapeutic hypothermia and the drug-eluting balloon, cardiac postconditioning to limit cell death following myocardial infarction and rotablation in the drug-eluting Syent era. There is also a chapter on using contrast medium-induced nephropathy gram-iodine/GFR ratio to predict CIN.

Book excerpt: Drawing on decades of experience and wisdom, Dr. Ugo Fisch created this classic text/atlas on microsurgical procedures for the skull base. The authors have labored not to popularize skull base surgery, but to provide the details of each operation, giving surgeons the valuable information they need. Each chapter of the text discusses a specific procedure and is divided into two sections, covering both the general surgical steps of the procedure and also its application. Within each chapter you'll find: general considerations; surgical techniques; applications; tips and pratfalls; imaging scans of illustrative cases; and color plates of pre- and post-operative preparation and instrumentation. The final two chapters discuss anesthesia issues and the impact of neuroradiology on skull base surgery. All of the cases used in the book reflect actual procedures, not hypothetical situations. All neurosurgeons and otolaryngologists involved in skull base surgery will benefit from having this classic text in their professional library. It demonstrates the procedures that have proven to be both safe and reliable through the years.
