In 1999 the Institute of Medicine estimated that as many as 98,000 hospitalized Americans die each year as a result of errors in their care. More people die from medical mistakes each year than from highway accidents, breast cancer, or AIDS. There are limited studies examining the relationship of a patient safety program and the associated outcomes of improved safety culture and errors. This quantitative, descriptive study retrospectively examined the results of the Safety Attitudes Questionnaire (SAQ) before and after the implementation of a patient safety program. The relationship between the measured safety culture and the number of medication errors was also examined to determine if there was a decrease in overall medication errors as safety culture improved. The interdisciplinary staff of a 25 bed pediatric intensive care unit completed this survey in April, 2007 and March 2008. From 2007 to 2008 there was an improvement in SAQ scores for the patient safety-related domains of job satisfaction, teamwork climate, safety climate, and perceptions of management (see SAQ results graph). There was a decrease in SAQ scores for the domains of stress recognition and working conditions. There was only a statistical improvement in the domain of job satisfaction (z= -3.27, p-value 0.001). In addition, there was a statistically significant increase in the number of reported medication errors from 2007 and 2008 (z= -3.12, p-value 0.002).
