This report gives an overview of the impact of discharge instructions on outcomes, and discusses the specifics of inpatient discharge summaries and outpatient after-visit summaries. The report also contains case studies illustrating different approaches to improving discharge instructions.

Book excerpt: Upon discharge from an emergency department (ED), patient education is a crucial component to ensuring adequate self-care of not only acute but chronic medical conditions. According to the CDC, there are approximately 130 million emergency department visits annually, with only 16.2 million resulting in hospital admissions. This leaves about 114 million opportunities for healthcare providers to educate patients at the time of discharge from the emergency department (ED). Research suggests more often than not, patients have difficulty understanding and comprehending the information they are given resulting in non-compliance with their treatment plans and appropriate follow-up. Although there are a variety of factors that can play a role in non-compliance with discharge instructions including language, healthcare literacy, stressful situations, a heavy workload for the healthcare provider, and an overall chaotic environment, there is a need for intervention. The purpose of this future research study is to evaluate if the use of pictographs written at a third-grade reading level improves compliance, comprehension, and satisfaction of discharge instructions as opposed to the standard written/verbal instructions commonly used in EDs across the nation.
