Available in PDF, EPUB and Kindle. Book excerpt: Therapeutic doses of X-rays to the thyroid region of children have been followed after some years by the development of thyroid neoplasms. Whereas the percent of cases of malignant neoplasms is small, the proportion of persons irradiated who develop nodular thyroid disease can be extremely high. The incidence of radiation-induced thyroid disease is strongly dose dependent above 100 rads (thyroid dose). The shape of the response curve below 100 rads is unknown. X-rays are probably as effective if not more so than iodine 131 in producing thyroid lesions for equal, average absorbed doses delivered to the gland at similar rates. An apparent greater effectiveness of X-ray irradiation may be due to the higher dose rate used. Whereas it was formerly believed that the induction of thyroid tumors was enhanced by irradiation of tissues other than the thyroid itself, it now seems possible to explain variability in tumor induction in children on the basis of whether or not the gland was in the primary X-ray beam. Radioactive iodine in amounts sufficient to deliver several hundred rads to the thyroid of the infant or young child has been shown to produce a high incidence of thyroid nodules. Radioactive iodine has been shown to be carcinogenic in some animals. No case of thyroid cancer clearly ascribable to it has been reported in man. (Author).

Book excerpt: During the 1950s, with the Cold War looming, military planners sought to know more about how to keep fighting forces fit and capable in the harsh Alaskan environment. In 1956 and 1957, the U.S. Air Force's former Arctic Aeromedical Laboratory conducted a study of the role of the thyroid in human acclimatization to cold. To measure thyroid function under various conditions, the researchers administered a radioactive medical trace, Iodine-131, to Alaska Natives and white military personnel; based on the study results, the researchers determined that the thyroid did not play a significant role in human acclimatization to cold. When this study of thyroid function was revisited at a 1993 conference on the Cold War legacy in the Arctic, serious questions were raised about the appropriateness of the activityâ€"whether it posed risks to the people involved and whether the research had been conducted within the bounds of accepted guidelines for research using human participants. In particular, there was concern over the relatively large proportion of Alaska Natives used as subjects and whether they understood the nature of the study. This book evaluates the research in detail, looking at both the possible health effects of Iodine-131 administration in humans and the ethics of human subjects research. This book presents conclusions and recommendations and is a significant addition to the nation's current reevaluation of human radiation experiments conducted during the Cold War.

The report says that potassium iodide pills should be available to everyone age 40 or youngerâ€"especially children and pregnant and lactating womenâ€"living near a nuclear power plant. States and municipalities should decide how to stockpile, distribute, and administer potassium iodide tablets, and federal agencies should keep a backup supply of tablets and be prepared to distribute them to affected areas.

The discussions and conclusions regarding terminology and morphologic criteria 1, 2 from the meeting were summarized in publications by Baloch et al. and form the framework for this atlas. The atlas is organized by the general categories of “Nondiagnostic,” “Benign,” “Follicular Neoplasm/Suspicious for a Follicular Neoplasm”, “Suspicious for Malignancy,” and “Malignant,” and it includes the defi- tions and morphologic criteria of these categories as set forth by Baloch et al. The majority of the conference participants also agreed on a category of “undetermined significance,” which is incorporated in this atlas (Chap. 4). It is critical that the cytopathologist communicate thyroid FNA interpretations to the referring physician in terms that are succinct, unambiguous, and helpful clinically. We recognize that the terminology used here is a flexible framework that can be modified by individual laboratories to meet the needs of their providers and the patients they serve.
