Suite à la publication des recommandations de l’ANAES de 1999 sur le suivi des patients diabétiques, plusieurs études ont été menées par les services de l’Assurance Maladie dans diverses régions françaises, afin d’évaluer la prise en charge bucco-dentaire de ces patients. Le constat est identique : les recommandations de l’ANAES sont loin d’être respectées, les patients diabétiques consultent moins souvent que la population générale. Une enquête personnelle, réalisée auprès des médecins généralistes, des chirurgiens-dentistes et des patients diabétiques aboutit au même constat et incite à faire des efforts pour une meilleure application de ces recommandations.

Le diabète de type 2 est la conséquence d'une résistance des tissus cibles à l'action de l'insuline. Il est à l'origine de nombreuses complications. Pourtant, il demeure mal connu et moins bien étudié que le diabète de type 1. L'existence d'altérations de l'état parodontal associées au diabète de type 2 a souvent été discutée dans la littérature sans jamais faire la lumière sur les mécanismes exacts de cette association. Après avoir décrit la pathogénie du diabète de type 2 et des parodontites, nous nous intéresserons aux effets sur les tissus parodontaux de l'hyperglycémie et de l'hyperlipidémie propres aux sujets diabétiques de type 2. Nous verrons ainsi les différents mécanismes moléculaires du développement des parodontites chez le diabétique. Dans un deuxième temps, nous observerons les interrelations thérapeutiques existant entre ces deux maladies en examinant les effets des thérapeutiques parodontales sur le contrôle métabolique du diabète de type 2 ainsi que l'incidence de l'équilibration glycémique sur le traitement des parodontites. Enfin, à la lumière des dernières publications scientifiques et cliniques, nous envisagerons les nouvelles pistes de la thérapeutique parodontale du patient diabétique de type 2.
