For some decades the knowl edge acquired 40years or more ago was not improved appreciably, though the fre quency ofthe disease started to increase in occidental countries. This has recently changed, and the progress ofthe medical sciences has spread to the pancreas. Although the surgical or medical prognosis of the most frequent form of pancreatic cancer, exocrine adenocarcinoma, remains very bad, recent studies have shown the multiplicityofits pathological forms, some being less severe so that curative surgery is possible. New experimental models, particularly in the hamster, and the use of carcinogenic drugs allow experimental studies on lesions similar to those in man. Oncologic immunology is still at its beginnings but shows promise for diagnosis and treatment. Though modem techniques of imaging sonography, aspirative cytology, CT scan, endoscopic catheterism, arteriography, and maybe in the future nuclear magnetic resonance - have not yet significantly in fluenced prognosis, they have made the diagnosis easierand more precocious. Yet in a diseasethat diffuses so rapidly to deep lymph nodes, it has not been proved whether early diagnosis can improve prognosis.

Book excerpt: Malignant neoplasms occurring in the biliary tract and pancreas remain a therap- tic challenge. The mechanism of carcinogenesis as well as the growth and spread of these tumors is still poorly understood, making the development of rational tre- ment strategies difficult. In order to improve the clinical results achieved by sur- cal or other medical treatment of such malignant tumors, the establishment of an experimental animal model is critical. For this purpose, attempts were made to induce carcinoma experimentally in the biliary tree and finally an animal model using the hamster was established in 1994 at our laboratory. Because the tumor in this model mimicked the characteristics of human tumors, a series of experimental investigations were conducted to clarify the pathological characteristics of biliary carcinoma, the genetic alterations during biliary carcinogenesis, and the relationship between biliary inflammation and c- cinogenesis. The chemopreventive effects on the occurrence of biliary carcinoma were also successfully examined. In addition, in vitro studies led to the establi- ment of transplantable biliary cancer cell lines and biliary epithelial cell lines by utilizing the hamster model. This monograph represents the collective efforts in hepato-biliary and pancreatic disease research over the past 20 years. I hope that this monograph will be a source of useful knowledge for basic researchers as well as for clinicians involved in the care of patients with hepato-biliary and pancreatic neoplasms. Book excerpt: Carcinoma of the pancreas has markedly increased in incidence over the past several decades, and now ranks as the fourth leading cause of cancer death. Cancer of the pancreas is abnormal cell growth in the tissue of the pancreas which is a small organ (about six inches long) located next to the small intestine, behind the stomach. The pancreas has two main jobs in the body: to produce juices that help digest (break down) food and to produce hormones, such as insulin and glucagon, that help control blood sugar levels. Both of these hormones help the body use and store the energy it gets from food. The majority of pancreatic cancers are adenocarcinomas. Adenocarcinomas are tumours that arise from the exocrine portion of the pancreas. The exocrine portion of the pancrease produces digestive fluids. A small number of pancreatic cancers begin in the endocrine pancreas. The endocrine portion produces hormones (such as insulin). Despite the high mortality rate associated with pancreatic cancer, its aetiology is poorly understood, it is rarely curable and has an overall survival rate of less than 4%. Research on changing the survival rates of this stubborn disease is highlighted in this new book.
