Book excerpt: The Federal Railroad Administration's (FRA) rail-safety oversight framework relies on inspections to ensure railroads comply with federal safety regulations. FRA inspects railroad infrastructure and operations, identifies safety defects, and may, if warranted, cite the railroads for violations of federal safety regulations. This book examines the overall framework that FRA, the states, and the railroads use to ensure rail safety; the extent to which FRA and the railroads assess safety risks and allocate resources to address those risks; and what challenges, if any, exist to FRA's current safety framework, and what ongoing and emerging issues FRA faces.

It's the lifeblood of job creation and economic growth. We are at a critical period in the Nation's long and storied railroad history. There are severe consequences to failures in safety and reliability. We've seen them in Metro-North, the busiest railroad in the country, and the series of incidents have caused not only delays in convenience, economic harm, but also injuries and fatalities. The American public also may not realize the severe environmental consequences and economic costs that could result from repetitions of these failures in safety and reliability. It is the responsibility of railroads to make investments and improve policies and practices; change their cultures; and install new leadership, because the record has to be improved. Federal agencies also have a responsibility to oversee and scrutinize those practices and policies of railroads, and the leadership and management that is coming from our railroads.

Book excerpt: " Railroad accidents pose significant safety risks to railroads, their employees, passengers, and the public. FRA oversees safety of the nation's railroads. In light of three high profile accidents in 2012 involving fatalities or hazardous materials, GAO was asked to review FRA's oversight processes and the challenges to railroad safety. This report examines (1) the overall framework that FRA, the states, and the railroads use to ensure rail safety; (2) the extent to which FRA and the railroads assess safety risks and allocate resources to address those risks; and (3) what challenges, if any, exist to FRA's current safety framework, and what ongoing and emerging issues FRA faces. GAO analyzed FRA accident and incident data, reviewed the analytical models FRA uses to incorporate risk into its inspection program, and interviewed FRA headquarters and field safety staff, officials from the 7 largest freight railroads and 11 smaller railroads, industry associations and 7 rail labor organizations. "

Book excerpt: The Rail Safety Improvement Act of 2008 (RSIA08) requires implementation of positive train control (PTC) on railroads which carry passengers or have high-volume freight traffic with toxic- or poisonous-by-inhalation hazardous materials. Chapters 1 and 2 discuss passenger railroads' PTC progress and FRA's steps to assist them, and how passenger railroads and FRA plan to approach the 2018 and 2020 deadlines. Chapter 3 reports on the FRA's passenger equipment safety standards using a performance-based approach to adopt new and modified requirements governing the construction of conventional and high-speed passenger rail equipment. Chapter 4 examines key characteristics of FRA's and FTA's rail safety oversight programs and strengths and limitations of FRA's and FTA's rail safety oversight programs. Crashes at highway-rail grade crossings are one of the leading causes of railroad-related deaths. Chapter 5 examines the focus of FRA's grade-crossing-safety research, how states select and implement grade-crossing projects and what data are available from FRA to inform their decisions, and the challenges states reported in implementing and assessing projects and the extent to which FHWA assesses the program's effectiveness. When a train is not moving but its engines are running, it can present risks and disruptions for the surrounding community as reported in chapter 6. Railroad bridges carry heavy and potentially dangerous loads over busy roadways and important waterways. Many of these bridges are a century old or more. However, unlike road bridges, which are the responsibility of public entities, railroad bridges are the responsibility of the private railroad companies that own or operate them. Congress has acted in recent years to improve oversight of railroad bridge safety, but incidents have prompted concerns about whether enough is being done to protect the public as discussed in chapter 7. Train derailments or collisions are often well-publicized events and receive significant attention from policymakers seeking to reduce their reoccurrence. Less attention has been devoted to trespassing, although it is a much greater cause of rail-related fatalities than derailments and collisions combined. As reported in chapter 8, over three-fifths of deaths in rail incidents have been pedestrian trespassers, and vehicle-train accidents at railroad grade crossings account for nearly one-third Congress created the Railroad Rehabilitation and Improvement Financing (RRIF) program to offer long-term, low-cost loans to railroad operators, with particular attention to small freight railroads, to help them finance improvements to infrastructure and investments in equipment. This program is the focus of chapter 9. Chapter 10 examines how WMATA spent its capital funds from fiscal years 2011 through 2017, how WMATA's new capital planning process addresses weaknesses it identified in the prior process, and WMATA's progress toward its track preventive maintenance program's goals and how the program aligns with leading program management practices.

Book excerpt: The United States railroad system consists of a vast network of operations that includes over 780 railroads with more than 230,000 employees and 200,000 miles of track in operation. The freight railroad industry is dominated by the seven largest freight railroads, referred to as Class I railroads, which collectively operate over 1.7 trillion ton-miles and accounted for more than 90 percent of annual railroad-freight revenues in 2012. There are also 10 Class II freight railroads that typically operate over 350 to 900 miles per railroad, and about 750 smaller Class III freight railroads with less than 350 miles of track that typically perform point to point service over short distances. In addition, Amtrak and 29 commuter railroads collectively carry an average of about 670 million passengers a year over 23 billion miles.

Book excerpt: Positive train control (PTC) is a communications-based train control system designed to prevent some serious train accidents. Federal law requires passenger and major freight railroads to install PTC on most major routes by the end of 2015. Railroads must address other risks by implementing other technologies. The FRA oversees implementation of these technologies and must report to Congress on progress in implementing PTC. This report discusses railroads' progress in developing PTC and the remaining steps to implement it, the benefits of and challenges in implementing other safety technologies, and the extent of FRA's efforts to fulfill the PTC mandate. Charts and tables. This is a print on demand edition of an important, hard-to-find report.

Book excerpt: "The evolution of railroad safety, Aldrich argues, involved the interplay of market forces, science and technology, and legal and public pressures. He considers the railroad as a system in its entirety: operational realities, technical constraints, economic history, internal politics, and labor management. Aldrich shows that economics initially encouraged American carriers to build and operate cheap and dangerous lines. Only over time did the trade-off between safety and output - shaped by labor markets and public policy - motivate carriers to develop technological improvements that enhanced both productivity and safety."--BOOK JACKET.
