Book excerpt: Diligent application of the Hazard Analysis and Critical Control Point (HACCP) approach is believed by many to provide a comprehensive system for food safety management. This belief, however, is not shared uniformly by all stakeholders. Because of the dichotomy of opinion surrounding food safety, there is a compelling need for additional dialogue and consensus on this issue. The aim of this expert survey instrument is to identify the gaps and the areas of agreement among various stakeholders. Two hundred and thirty-one survey instruments were received out of three hundred and sixty sent (64.2% response rate) to food safety professionals in academia, industry, federal and state government, and consumer protection groups. The survey consisted of four parts: I. A series of statements across the food chain describing the extent to which respondents agree with each statement; II. Rank order priority ratings and degree of satisfaction/dissatisfaction with 14 issues; III. Weighted priority rankings for 10 issues; and IV. Verbatim comments. The data suggests broad support for: 1. More consistent application of HACCP by industry and government; 2. Development of a comprehensive K-12 food safety education program by the federal government and industry; 3. Increased federal funding for microbiological food safety research; 4. Increased focus on on-farm practices for controlling pathogens; and 5. Increased focus on the microbiological safety of imported food products. There is little support for: 1. Increased investment to control microbial pollutants in U.S. surface and ground water supplies; 2. Increased federal food safety inspection programs; 3. Increased use of finished product microbiological testing; and 4. Development of a comprehensive global foodborne disease surveillance network. Significant differences exist among stakeholder groups on selected food safety priorities. Continued dialogue is needed to understand the basis for these differences and to develop potential approaches to addressing them.

The U.S. Food and Drug Administration's abilities to discover potential threats to food safety and prevent outbreaks of foodborne illness are hampered by impediments to efficient use of its limited resources and a piecemeal approach to gathering and using information on risks. Enhancing Food Safety: The Role of the Food and Drug Administration, a new book from the Institute of Medicine and the National Research Council, responds to a congressional request for recommendations on how to close gaps in FDA's food safety systems. Enhancing Food Safety begins with a brief review of the Food Protection Plan (FPP), FDA's food safety philosophy developed in 2007. The lack of sufficient detail and specific strategies in the FPP renders it ineffectual. The book stresses the need for FPP to evolve and be supported by the type of strategic planning described in these pages. It also explores the development and implementation of a stronger, more effective food safety system built on a risk-based approach to food safety management. Conclusions and recommendations include adopting a risk-based decision-making approach to food safety; creating a data surveillance and research infrastructure; integrating federal, state, and local government food safety programs; enhancing efficiency of inspections; and more. Although food safety is the responsibility of everyone, from producers to consumers, the FDA and other regulatory agencies have an essential role. While we enjoy one of the safest food supplies in the world, it requires constant monitoring. We need to be able to rapidly identify and address risks to our food supply as a result of new disease agents, new food technologies, changes in U.S. demographic and dietary patterns, and an abundance of food imports resulting from an increasingly globalized food supply. In recent years, consumers and industry alike have been impacted by illnesses associated with food products, such as ground beef, peppers, peanut butter, spinach, shell eggs, and cookie dough, among others. While regulatory and industry efforts have over time improved food safety considerably, the Centers for Disease Control and Prevention has recently estimated that 1 in 6 Americans suffers from foodborne illness annually, resulting in 128,000 hospitalizations and 3,000 deaths per year, most of which are preventable. The public health consequences of foodborne illness remain significant. The impact reaches well beyond the number of cases. Foodborne illnesses result in billions of dollars in medical costs, as well as significant economic losses to the food industry when illness outbreaks and contamination incidents undermine consumer confidence in affected commodities, require large recalls, and diminish demand. American consumers have high expectations for the safety of the food supply. While all risks will never be eliminated, the public rightfully expects that the government and the food industry do everything that can reasonably be done to prevent food safety problems. The federal government has taken important steps to protect an increasingly complex food supply. In March 2009, President Obama created the Food Safety Working Group (FSWG), a central coordinating mechanism for the federal government's food safety activities that is led by the Department of Health and Human Services (HHS) and the U.S. Department of Agriculture (USDA). The FSWG's core mission is to strengthen federal efforts and develop short-term and long-term strategies to improve food safety. Partner agencies include HHS's Food and Drug Administration (FDA), USDA's Food Safety and Inspection Service (FSIS), and HHS's Centers for Disease Control and Prevention (CDC), as well as the Environmental Protection Agency (EPA), Department of Homeland Security, Department of Commerce, Department of State, and the Office of the United States Trade Representative. The White House Domestic Policy Council convenes the FSWG. The premise underlying the FSWG's creation is that coordination among federal agencies is essential to protect consumers in our highly diverse, global food system. Coordination and cooperation are necessary through every stage of the production and consumption process-"from farm to table." FSWG member departments and agencies thus share information and experience about all aspects of food safety. Such coordination strengthens the scientific and technical infrastructure to support a modern food safety system.

In 2013, the U.S. Dept. of Agriculture (USDA) estimated the cost of foodborne illness in the U.S. to be $14.1 billion per year. Three major trends also create food safety challenges: (1) a substantial and increasing portion of the U.S. food supply is imported; (2) consumers are eating more raw and minimally processed foods; (3) segments of the population particularly susceptible to foodborne illnesses, such as older adults and immune-compromised individuals, are growing.
