Book excerpt: Many people think of bones as simple, solid structures that make up the skeletal system. In truth, bones are complex, living tissues that go through a constant process of building up and tearing down. This process, called "bone remodeling," rebuilds the bones as people grow and age. One of the main components of bone is calcium. The skeletal system contains 99 percent of the body's calcium. In osteoporosis, which means "porous bones," excessive bone loss results in a depletion of calcium. The gradual weakening of the bones over time makes them more susceptible to fractures and can lead to disfigurement and pain. Since the loss occurs over time, the effect may not be noticed until substantial bone loss has occurred, often signaled by an unexpected fracture. Osteoporosis cannot be detected by x-ray until the bone loss has reached 30-50 percent of bone mass, by which time the calcium depletion cannot be reversed. Because of this, prevention and early diagnosis are critical. Osteoporosis Sourcebook, Third Edition offers information to help readers identify key risk factors for osteoporosis and understand effective prevention strategies. Osteoporosis across various ages and genders is discussed in detail and information about the health conditions related to osteoporosis is also provided. It offers insights on calcium intake, other dietary needs, medications, and surgical options. It also suggests coping strategies for those suffering from the disease. The book concludes with a glossary of related terms and a directory of resources. Part 1: Osteoporosis: An Overview begins with an introduction to osteoporosis gives a brief insight basic structure and functions of the bone and highlights the factors that influence/affect bone health. It also distinguishes osteoporosis from arthritis and emphasizes the importance of proper diagnosis. Part 2: Osteoporosis across Age and Gender outlines the impact of osteoporosis on various segments of the population such as women, children, men, and older adults. Part 3: Osteoporosis and Related Conditions provides information about diseases and conditions that lead to or aggravate osteoporosis or that have symptoms similar to those produced by osteoporosis. The conditions include dripping fibrous dysplasia, Gaucher disease, otosclerosis, hypercalcemia, hypocalcemia, hyperparathyroidism, hypophosphatasia, inflammatory bowel disease (IBD), multiple myeloma, oral health, osteopetrosis, osteogenesis imperfecta, and Paget disease. Part 4: Risk Factors and Prevention Strategies discusses in detail about the factors that lead to osteoporosis and suggests healthy life-style choices to minimize those factors. Part 5: Diagnosing and Treating Osteoporosis traces the process of diagnosing osteoporosis as early as possible and treating the condition effectively. It reviews the various options in drug and surgical therapies. It also highlights some of the complementary and alternative medicines available for osteoporosis. Part 6: Living with Osteoporosis deals with the impact to osteoporosis on an individual. It explains how lifestyle changes can help patients with osteoporosis lead a healthy life. The part also deals with the strategies to cope up with the chronic pain caused by the disease. Part 7: Additional Help and Information provides a glossary of osteoporosis-related terminology and a list of resources for patients with osteoporosis or related conditions.

Available in PDF, EPUB and Kindle. Book excerpt: René Rizzoli Menopause is the time in a woman’s life when reproductive capacity ends. Ovaries decrease their activity and the production of sex hormones ceases. This period may be associated with a large variety of symptoms affecting the cardiovascular and urogenital systems, as well as skin, hair and bone. Bone capital is accumulated by the end of the second decade and remains more or less constant up to the time of menopause. Sex hormone deficiency leads to accelerated bone turnover, a negative balance and microarchitectural deterioration, which compromises bone strength, thereby increasing bone fragility and, thus, fracture risk. By the age of 80, it is estimated that 50% of trab- ular bone will have been lost. Natural menopause occurs between the ages of 45 and 54 years all over the world. This age does not appear to have changed significantly over the centuries. In contrast, since the middle of the 19th century, life expectancy, particularly in women, has increased considerably, with most women living to the age of 80 years or more in many regions of the world. This means that at the age of 50 years, a woman will live for more than 30 years without bone protection by sex hormones. This r- resents more than one-third of a woman’s life. At the age of 50 years, the lifetime risk to experience a fracture is about 50% (ie, one out of two women will have a fracture during this period).

In this new edition, the authors point out the enormous scale of the problem in terms of human suffering, morbidity and mortality on the one hand, and the astronomical national costs on the other. Written in an easy-to-read style, this book updates physicians on the current knowledge about bone structure, physiology and pathology, with an emphasis on the diagnosis, therapy and prevention of osteoporosis. Its purpose is to educate members of the medical profession about this widespread and potentially disabling disease, while providing information on related current evidence-based medicine. The book covers everything from basic physiology to osteoporosis diagnosis, including utility of specialized tests and current treatment recommendations, and will provide the primary care physician with sound medical knowledge in the prevention and management of osteoporosis. It is up-beat and optimistic, with a touch of humor; it is scientifically based but provides easy-to-follow guidelines for lifelong maintenance of skeletal structure and function. Osteoporosis is preventable if doctors want it to be, and every doctor can contribute. Bone is every doctor's and every body's business.

Book excerpt: Completely revised and updated, and utilizing the most current evidence and practice guidelines for the treatment of osteoporosis, this comprehensive third edition discusses the basic aspects of bone metabolism, the pathophysiology of osteoporosis, current diagnostic techniques and medical treatment strategies. Osteoporosis is a common disorder that is prevalent in over 20 million Americans over the age of 60. One and a half million osteoporotic fractures occur in the United States every year, including 300,000 hip fractures. Mortality rates after hip fracture approach 25%, with another 50% of patients experiencing a major decrease in their prior level of independence and quality of life. Despite the wide prevalence and severe consequences of osteoporosis, it remains a disorder that is severely under-diagnosed and treated. In this context, specialists and primary care physicians alike are having increasing difficulty keeping up with the rapid changes to the field and incorporating these advances to clinical care. In the years since the last edition of this book was published, the osteoporosis field has changed drastically. In addition to revising and updating existing chapters and removing a few that are no longer as relevant, new chapters discuss an advanced understanding of the cellular and molecular mechanisms underlying the disorder, the introduction of new diagnostic imaging techniques, a more nuanced appreciation of the risks and benefits of osteoporosis therapies, and the introduction of two new classes of osteoporosis medications. Following the format of the second edition, and including helpful key points at the opening of each chapter, this text will present a comprehensive overview of both the basic and clinical concepts relating to each topic, when appropriate. Chapter authors were chosen based on their high level of expertise and leadership in the field. Taken together, this text should thus be of great interest to physicians of multiple specialties, allied health professionals, as well as basic and clinical researchers.
