Book excerpt: Osteoporosis prevention education interventions intended to increase the osteoporosis preventive behaviors of weight-bearing physical activity and calcium consumption in young individuals have been found to be ineffective. An osteoporosis prevention education intervention was developed and modeled after an effective health threat prevention education intervention based on the health belief model, which emphasized the health threat's visible severity and proximal time of onset. To test its effectiveness, it was experimentally researched in a sample of 109 college women who were students in an undergraduate health education course, and were randomly assigned to either the treatment or a control group to receive the osteoporosis prevention education intervention or a stress management intervention, respectively. The treatment group did not positively alter their osteoporosis health beliefs or increase self-reported weight-bearing physical activity and calcium consumption compared to the control group. And the control group who received the stress management intervention showed a significant increase in health motivation while the treatment group who received the osteoporosis prevention education intervention did not. A probable reason is that due to the distal time of onset of osteoporosis, young individuals may not be concerned with modifying their behaviors to prevent the disease. Recommendations for future research and effective ways to promote weight-bearing physical activity and calcium consumption are provided.

Education to increase awareness is identified as being paramount in helping to prevent this disease. Present study examined the effectiveness of a three-month osteoporosis educational prevention program based on the Health Beliefs Model on knowledge, health beliefs, and practice about calcium intake, among young Iranian female. Participants in the intervention group were exposed to a three-month multi-component educational program including a group lecture, self study materials, individual face to face consultation, emails and phone reminders. Osteoporosis knowledge, beliefs, self-efficacy, and calcium intake, were assessed at baseline and at the end of three months follow up. This study showed poor levels of knowledge regarding osteoporosis among Iranian young female students at baseline. The intervention program was effective in improving knowledge, beliefs, and self-efficacy to take action about calcium intake as a lifestyle factor for the prevention of osteoporosis in young women.

Participants in the intervention group were exposed to a three months educational program. Osteoporosis knowledge, health beliefs, and total time (min) and frequency of weight-bearing exercise in the past seven days were assessed at baseline, and at the end of three months follow up. This book indicates that an effectiveness osteoporosis educational program can help to improve Iranian young female knowledge, beliefs and regular exercise as a health prevention action for osteoporosis.

Book excerpt: The three aims of the study investigated the females' age 18-25 perception, knowledge, and health beliefs, barriers and strategies to overcoming barriers to prevent osteoporosis. The mixed method explanatory design of research was implemented. Phase one used a survey to gather demographic data, past and current behaviors, the completion of the Osteoporosis Health Behavior Scale (OHBS) questionnaire, and Short Calcium Intake List (SCaIL). Four hundred forty-nine quality survey responses were obtained. The second phase, the focus group (n=23), discussed questions designed to answer the three aims of the study. Results- Our analysis showed 90% of our 447 survey participants did not ingest the RDA of 1000mg of calcium. Those that drank milk growing up and ate four or more family meals were more likely to consume the RDA of calcium as a young adult. Also those consuming 2 or more glasses of milk per day as a young adult were more likely to meet calcium RDA. As participants were more health motivated they saw fewer barriers to exercise and calcium intake. There was a positive relationship between if participants believed they were more susceptible to and viewed osteoporosis as serious they were more likely to believe there were more barriers to helping themselves. Conclusion- These results suggest a need for nutrition education using hands-on teaching methods, and a national marketing/social media platform focused towards the young adult female pertaining to dietary consumption of calcium, vitamin D and physical activity as it relates to bone health/prevention of osteoporosis. This age group are the future parents and leaders of the next generation and current and past research indicates education of healthy meals, childhood milk consumption and family meals equate to healthier future outcomes of the next generation.

Abstract: Osteoporosis is a serious health problem worldwide and most commonly affects postmenopausal women (World Health Organization, 1994). Of the variety of complications associated with osteoporosis in postmenopausal women, osteoporotic fracture is the most prevalent and serious one. It affects their quality of life and puts heavy burden on the healthcare system in Hong Kong. Since osteoporosis exercise program can help to reduce bone loss, it seems to be an effective strategy to prevent and manage osteoporosis in postmenopausal women. The purpose of this study is to develop an evidence-based osteoporosis exercise education program for postmenopausal women, based on the best available evidence, with an aim of helping them to prevent and manage osteoporosis through the prevention and reduction of bone loss and its related fractures. A search of literature from four databases, including Pubmed (1997 to August 2011), CINAHL Plus (1937 to August 2011), Cochrane Library (1996 to Issue 8 of 2011) and EMBASE Classic + EMBASE (1947 to August 2011), was done to identify relevant randomized controlled trials (RCTs). The methodology checklist for RCTs developed by the Scottish Intercollegiate Guidelines Network (SIGN, 2008) was used to evaluate the quality of the selected studies. A total of nine studies met the inclusion criteria and did not violate the exclusion criteria. The findings of these studies showed that exercise intervention or program significantly reduced postmenopausal women's likelihood of having fracture and also improved their bone mass density slightly. Among the reviewed studies, programs were most commonly implemented within a 12-month period and two 60-minute supervised exercise sessions were delivered per week. The potential of implementing the proposed program, based on the evidence from the literature review, to local practice was assessed. Based on the assessment, the proposed program is necessary and beneficial for postmenopausal women. An evidence-based osteoporosis exercise education program is developed based on the best available evidence. The SIGN grading system was utilized to provide grades for the recommendations in the guideline. An effective and adequate communication with potential stakeholders to gain their support and cooperation for the proposed program is essential. In addition, a tailor-made pilot test is also crucial for ensuring the successful implementation of the proposed program. The pilot study will be conducted in a local practice and the targeted participants are 124 postmenopausal women. Data analysis will be done by the paired t-test. The proposed evidence-based osteoporosis exercise education program will be considered as effective if (1) there is a reduction of bone loss and an increase in physical activity level in postmenopausal women, and (2) there is an increase of confidence and self-perceived skill on the prevention of management of osteoporosis in nurses.
