Il s’agit d’une pathologie des os du squelette caractérisée par une diminution de la masse osseuse pouvant entraîner des fractures. Les maladies parodontales sont des pathologies inflammatoires d’origine bactérienne, des tissus de soutien de la dent pouvant conduire à une destruction de l’os alvéolaire. Ces deux pathologies, dont l’incidence est importante au sein de la population mondiale, présentent des similitudes. En présence d’ostéoporose ou de maladies parodontales avancées, on retrouve le même processus physiopathologique, c'est-à-dire un déséquilibre de la balance formation/dégradation osseuse, qui normalement permet l’homéostasie osseuse. Ainsi, existe-t-il une association entre ostéoporose et maladies parodontales ? Afin de répondre à cette question, nous aborderons dans la première partie, les généralités sur l’ostéoporose et les maladies parodontales, puis, nous nous intéresserons aux différentes études concernant cette possible association. Nous verrons que la radiographie panoramique des maxillaires peut jouer un rôle dans le dépistage précoce de l’ostéoporose. Enfin, nous essaierons de dresser une prise en charge adaptée du patient ostéoporotique en parodontologie et dans d’autres disciplines odontologiques.

L’hypothèse aujourd’hui retenue serait que la carence en oestrogènes à la ménopause serait également responsable de la résorption de l’os alvéolaire observée dans les maladies parodontales. La ménopause et l’ostéoporose ne seraient donc pas le facteur déclenchant mais potentialiseraient les effets destructeurs des parodontites sur l’os alvéolaire. Par ailleurs, l’Académie Américaine de Parodontologie reconnaît aujourd’hui que l’ostéoporose est un facteur de risque de maladie parodontale.

La maladie parodontale est une pathologie inflammatoire d’origine infectieuse pouvant entraîner la destruction des tissus de soutien de la dent et la perte dentaire. De nombreuses études expérimentales et cliniques supposent que ces deux pathologies pourraient être liées. Certaines ont montré que la diminution de la masse des os du squelette était corrélée avec celle des os de la cavité buccale, et ceci indépendamment de la présence d’une pathologie parodontale. D’autres se sont focalisées sur l’impact de l’ostéoporose sur les indicateurs de la maladie parodontale, ainsi que sur les effets des traitements de l’ostéoporose sur la cavité buccale, en présence et en l’absence de pathologie parodontale. Ces études ont présenté des résultats souvent contradictoires. Ceci peut s’expliquer par le fait que la plupart de ces études sont transversales et/ou réalisées sur des échantillons de petite taille, et que certaines variables de confusion ne sont pas toujours contrôlées. La connaissance de cette relation et la compréhension des mécanismes liant ces deux pathologies, représenteraient une aide certaine aux professionnels de santé, à la fois dans le domaine de la prévention et du traitement de ces pathologies
