Des stratégies adaptatives doivent être recherchées afin de rendre les tissus, en particulier la dentine sclérotique, plus réceptifs en matière de rétention micromécanique et d'adhésion. Différentes stratégies ont été suggérées dans la littérature pour améliorer l'adhésion à la dentine sclérotique, mais actuellement aucun protocole ne fait consensus. L'objectif de cette revue systématique de la littérature était d'analyser les études visant à évaluer des protocoles pour optimiser l'adhésion à la dentine sclérotique. La recherche a été réalisée en suivant la méthode PRISMA, à partir de la base de données Pubmed. 45 articles répondant aux critères d'inclusion ont été retenus pour l'analyse finale. Parmi ces articles, la nature de l'échantillon, le protocole évalué et les résultats obtenus ont été extraits et classés. Ce travail a permis d'élaborer un protocole de collage optimal incluant l'isolation, le pré-traitement et le choix de l'adhésif. Malgré les résultats obtenus, davantage d'études cliniques sont nécessaires pour déterminer le protocole optimal pour améliorer les valeurs d'adhérence à la dentine sclérotique.

Le tissu hyperminéralisé interfère avec l'obtention d'une adhésion prévisible et durable en empêchant la diffusion des différents adhésifs. Pour améliorer l'adhésion sur ces tissus pathologiques il faut optimiser toutes les étapes du collage (émail périphérique, choix du système adhésif et du composite de collage, préparation de la prothèse, digue) et différents pré-traitements adaptés sont à l'étude (fraisage léger, désinfection, laser, mordançage accentué, application d'EDTA). Les traitements de l'usure dentaire, particulièrement en présence de tissu sclérotique, ne sont pas aussi bien codifiés que ceux décrits pour d'autres pathologies dentaires. Il faut donc prendre en compte tous les éléments décris ci-dessus pour envisager un plan de traitement adapté et personnalisé

Malgré les progrès notables en termes d'adhésion, les protocoles adhésifs sont établis pour la structure dentaire saine or de nombreuses altérations peuvent venir interférer et impacter négativement l'adhésion. Parmi ces altéra-tions nous retrouvons notamment : l'amélogénèse imparfaite, la fluorose dentaire, l'érosion, la sclérose dentinaire, la maladie carieuse, la MIH ou encore l'altération suite à l'exposition aux amalgames. Il convient donc au praticien de trouver un moyen d'améliorer celle-ci sur les substrats altérés. Ce travail a pour vocation de présenter, avec l'aide des données de la littérature, les différentes possibilités qui s'offrent à nous pour optimiser les protocoles d'adhésion sur les substrats altérés. Après avoir effectué quelques rappels sur l'altération des substrats et les moyens de les diagnostiquer nous analy-serons les résultats de la littérature scientifique pour comprendre les pistes de réflexions proposées en fonction de chaque substrat. Enfin, dans une partie dédiée nous verrons quels moyens employer pour améliorer l'adhésion. Pour la majorité des substrats, le système M&R sera le système de choix pour le praticien permettant une meilleure adhésion au substrat altéré. Le choix de l'adhésif revêt un caractère important mais n'est pas le seul critère permettant d'améliorer l'adhésion, en effet il est également possible pour le praticien d'opter pour une modification de la phase de mordançage (en doublant celle-ci par exemple) ou l'adjonction d'un traitement de surface du substrat (préparation supplémentaire, utilisation de NaClO/EDTA) permettant d'améliorer l'adhésion.

Book excerpt: Evaluates the carcinogenic risks to humans posed by the use of four antiretroviral agents four DNA topoisomerase II inhibitors used in the treatment of cancer and an additional three pharmaceutical agents (hydroxyures phenolphthalein and vitamin K substances). The volume marks the first IARC evaluation of nucleoside analogs that act as antiviral agents. The evaluation responds in part to recent findings that zidovudine (AZT) an effective antiretroviral agent now being given to pregnant HIV-infected women to prevent maternal-to-fetal transmission of the virus is a transplacental carcinogen in mice. The opening monograph evaluates the carcinogenicity to humans of the antiretroviral nucleoside analogs zidovudine (AZT) zalcitabine (ddC) and didanosine (ddI) and the antiherpesvirus drug aciclovir. Of these aciclovir and didanosine could not be classified on the basis of available data. For zidovudine transplacental administration to mice resulted in an increased incidence and multiplicity of lung and liver tumours and in an increased incidence of female reproductive tract tumours in one study but not in another involving treatment at a lower dose. Despite observation of toxic effects in some studies of humans human carcinogenicity data were judged to provide inadequate evidence of carcinogenicity in humans. Zidovudine was classified as possibly carcinogenic to humans. Similar weaknesses in human carcinogenicity data for zalcitabine which consistently induces thymic lymphomas in mice resulted in its classification as possibly carcinogenic to humans. The second monograph evaluates four DNA topoisomerase II inhibitors: etoposide teniposide mitoxantrone and amsacrine. Of these etoposide - one of the most widely used and effective cytotoxic drugs in combination therapy - was classified as probably carcinogenic to humans and etoposide in combination with cisplatin and bleomycin was judged to be carcinogenic to humans. Teniposide was classified as probably carcinogenic to humans and mitoxantrone and amsacrine were classified as possibly carcinogenic to humans. Of the three pharmaceutical agents evaluated in the final monograph hydroxyurea which is widely used in cancer treatment and increasingly in combination with didanosine in HIV infection could not be classified. Phenolphthalein a widely used laxative now being withdrawn from the market in many countries because of toxicological concerns was classified as possibly carcinogenic. Vitamin K substances could not be classified on the basis of available evidence.
