Book excerpt: Consider the problem of determining the optimal operating policy of a two terminal shuttle with fixed capacity q or = infinity. The passengers arrive at each terminal according to Poisson processes and are transported by a single carrier operating between the terminals. The interterminal travel time is a positive random variable with finite expectation. Under a fairly general cost structure, it is shown that the policy which minimizes the expected total discounted cost over infinite time horizon has the following form: Suppose the carrier is at one of the terminals with x number of waiting passengers and suppose that y number of passengers are waiting at the other terminal. Then the optimal policy is to dispatch the carrier if and only if x or = G(y), where G(y) is a monotone decreasing control function. Furthermore, G(y) is always less than or equal to the carrier capacity Q. This control function can be approximated by the linear functions G(y) = K - Beta y.

Book excerpt: We study the dynamic ambulance dispatch problem in emergency medical services, in which a decision-maker sequentially observes call arrivals and decides which unit to dispatch to service the call, where the service times follow independent and heterogeneous distributions for each station-location pair. This problem is formulated as a Markov decision process, which is intractable due to the curse of dimensionality. To solve this problem, we develop two easy-to-implement heuristics based on a deterministic linear program (LP) and the Lagrangian relaxation. We show that these policies have provably near-optimal performances as the number of units and arrival rates increase. We also show that the deterministic LP-based policy is still asymptotically optimal for general service time distributions. Finally, we examine the performance of these policies and the bounds on numerical experiments, including a case study based on real data from St. Paul, Minnesota.

Book excerpt: In this thesis, we propose novel optimization and spatial queueing models that expand the currently existing methods by allowing multiple servers to be located at the same station and multiple servers to be dispatched to a single call. In particular, a mixed integer linear programming (MILP) model is introduced that determines how to locate and dispatch ambulances such that the coverage level is maximized. The model allows multiple servers to be located at the same station and balances the workload among them while maintaining contiguous first priority response districts. We also propose an extension to the approximate Hypercube queueing model by allowing multi-server dispatches. Computational results suggest that both models are effective in optimizing and analyzing the emergency systems. We also introduce the M[G]/M/s/s queueing model as an extension to the M/M/s/s model which allows for multiple servers to be assigned to a single customer.
