Book excerpt: This is the first of four volumes that together offer an authoritative, in-depth reference guide covering all aspects of the management of oral cancer from a multidisciplinary perspective and on the basis of a strong scientific foundation. This volume, on tumor biology, epidemiology, etiology, emerging role of cancer stem cells and prevention of oral cancer, opens by discussing oral carcinogenesis in general and the role of particular carcinogens and human papillomavirus. Global epidemiology and changes in disease prevalence are then addressed. Up-to-date information is provided on emerging cancer biomarkers, and the biologic basis of personalized therapy is explained. Histopathological features of malignant and premalignant neoplasms and their relevance to management are described. Further chapters focus on the current status of chemoprevention, the management of oral submucous fibrosis, and the value of various diagnostic adjuncts. The volume concludes by critically evaluating the efficacy of oral screening methods. Volume 1: Biology, Epidemiology, Etiology, and Prevention Volume 2: Diagnosis and Management Volume 3: Oral and Maxillofacial Reconstructive Surgery Volume 4: Rehabilitation and Supportive Care

Book excerpt: This monograph equips clinicians with the knowledge required to detect oral cancer at the earliest possible stage while simultaneously inspiring researchers to work on novel methods of detection. All the methods employed in the oral cancer context are considered, from simple ones like oral screening to more complex emerging optical methods and biomarker identification strategies. Individual chapters focus on conventional oral screening and application of vital stains, optical methods like white light based fluorescence-reflectance imaging, narrow band imaging, direct-oral-microscopy, and more advanced methods like optical coherence tomography, an in-vivo optical biopsy technique, and photo-acoustic imaging that allows visualization of deeper tissue changes. Novel electrical methods like bio-impedance assessment, occult biophysical methods like crystallization test, and the most promising salivary biomarkers and point-of-care opportunities are covered. Helpful information is also provided on essential topics including, oral potentially malignant disorders, biological aspects and molecular mechanisms underlying oral cancer progression, global epidemiology, concept of diagnostic delays, traditional imaging, and classic histopathology and microscopic features. The newer techniques are currently of active research interest, and can soon become powerful chair-side tools with potential to reduce diagnostic delays and improve survival.

Available in PDF, EPUB and Kindle. Book excerpt: Oral squamous cell carcinoma (SCC) is the 13th commonest cancer worldwide, and the most common cancer in the Asian subcontinent due to the widespread habit of tobacco and betel nut chewing. Despite many advances in diagnosis and treatment, the survival statistics have only marginally improved. However our understanding of the disease process and transformation from pre-cancerous lesions of the oral mucosa to an invasive SCC cancer and their progression has expanded exponentially. There are many conditions of the oral mucosa that can progress to an invasive malignancy. A thorough understanding of these conditions is a prerequisite for all those involved in the management of the diseases of the oral mucosa and head and neck region. The recognition and timely treatment of potentially pre-malignant conditions of the oral cavity can minimize the change to an overt malignancy in many patients through patient education, appropriate treatment and surveillance. In this book we cover relevant anatomy, biology, diagnosis and latest management strategies for pre-cancerous conditions that affect the oral mucosa. The respective chapters are written by expert contributors from around the world, lending the book a global perspective and making it an essential guide for all those involved in the management of pre-malignant lesions arising in this challenging anatomical region.

Book excerpt: A working group of sixteen experts from seven countries re-evaluated the evidence of the carcinogenicity of betel-quid and areca-nut chewing and some areca-nut related nitrosamines. Betel-quid and areca-nut chewing are widely practised in many parts of Asia and in Asian-migrant communities elsewhere in the world. There are hundreds of millions of users worldwide. They evaluated betel quid with tobacco as carcinogenic to humans (Group 1) on the basis of sufficient evidence of an increased risk of cancer of the oral cavity, pharynx and oesophagus. The working group reviewed epidemiological studies of human cancer, mainly studies from India, Pakistan and Taiwan (China). Studies on betel quid with tobacco and areca nut with tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. The working group also evaluated betel quid without tobacco as carcinogenic to humans (Group 1), on the basis of sufficient evidence of an increased risk of oral cancer. Studies on betel quid without tobacco and areca nut without tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. Areca nut, a common ingredient of betel quid and many different chewing preparations, including those available commercially, has been observed to cause oral submucous fibrosis

Book excerpt: This report, which was developed by an expert committee of the Institute of Medicine, reviews the first three services listed above. It is intended to assist policymakers by providing syntheses of the best evidence available about the effectiveness of these services and by estimating the cost to Medicare of covering them. For each service or condition examined, the committee commissioned a review of the scientific literature that was presented and discussed at a public workshop. As requested by Congress, this report includes explicit estimates only of costs to Medicare, not costs to beneficiaries, their families, or others. It also does not include cost-effectiveness analyses. That is, the extent of the benefits relative to the costs to Medicareâ€"or to society generallyâ€"is not evaluated for the services examined. The method for estimating Medicare costs follows the generic estimation practices of the Congressional Budget Office (CBO). The objective was to provide Congress with estimates that were based on familiar procedures and could be compared readily with earlier and later CBO estimates. For each condition or service, the estimates are intended to suggest the order of magnitude of the costs to Medicare of extending coverage, but the estimates could be considerably higher or lower than what Medicare might actually spend were coverage policies changed. The estimates cover the five-year period 2000-2004. In addition to the conclusions about specific coverage issues, the report examines some broader concerns about the processes for making coverage decisions and about the research and organizational infrastructure for these decisions. It also briefly examines the limits of coverage as a means of improving health services and outcomes and the limits of evidence as a means of resolving policy and ethical questions.

This book was released on 2017-06 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: This colposcopy manual was developed in the context of the cervical cancer screening research studies of the International Agency for Research on Cancer (IARC) and the related technical support provided to national programs. It is thus a highly comprehensive manual, both for the training of new colposcopists and for the continuing education and reorientation of those who are more experienced. This manual offers a valuable learning resource, incorporating recent developments in the understanding of the etiology and pathogenesis of cervical intraepithelial neoplasia (CIN), as well as in colposcopy and cervical pathology. Expertise in performing satisfactory, safe, and accurate colposcopic examinations requires high competence in the technical, interpretive, and cognitive aspects, and the capability to develop pragmatic and effective management plans and treatment. This comprehensive and concise manual covers all these aspects and serves as a useful handbook for acquiring the necessary skills for the visual recognition and interpretation of colposcopic findings and for developing the personal and professional attributes required for competence in colposcopy.
