Book excerpt: L'infection par le VIH des femmes immigrées en provenance d'Afrique subsaharienne sur le sol français était en constante augmentation jusqu'en 2002. Si l'on constate un infléchissement important depuis 2003 confirmé en 2005, il reste que les femmes d'Afrique subsaharienne représentent 48% des femmes dépistées positives contre 19,2% des hommes. Il nous est apparu à la consultation du SMIT de Saint Denis que ces séropositivités ne sont pas le fait d'une maladie simplement d'importation mais que sur les femmes dépistées certaines se sont contaminées en France, puisqu'elles ont présentées un test antérieur négatif. A l'aide d'un questionnaire rempli par 26 nouvelles dépistées sur une période de 6 mois et qui avaient toutes présentées un test antérieur négatif, nous avons définis deux typologies de patientes plus à risque dont 80% provenaient d'Afrique subsaharienne et 20% des Caraïbes : les femmes mariées qui se font contaminer dans l'intervalle de deux grossesses, l'utilisation du préservatif étant absent au sein du couple; Des femmes jeunes, célibataires, sans famille en France, avec des partenaires qui refusent test et préservatif. Nous avons élaborés notre réflexion sur les déterminants des ses contaminations par des entretiens d'acteurs de la prévention. Il apparaît que le rapport de domination homme/femme, la pauvreté et la représentation sociale du SIDA sont les trois obstacles à la prévention du VIH chez les femmes migrantes. Afin d'améliorer encore l'impact des campagnes publiques, une prévention de proximité notamment axée sur la spécificité de genre destinée aux hommes et aux rapports homme/femme, la promotion du préservatif féminin, une amélioration des conditions de vie des migrantes et une prise de position des instances religieuses pourraient être des remparts aux conduites à risque
