Au travers de 5 cas cliniques, il ressort que l'orthodontie apporte de réels bénéfices dans l'aménagement des édentements. Les prothèses complexes chez l'adulte n'en sont qu'améliorées. La réussite n'est obtenue qu'au prix d'une collaboration pluridisciplinaire ouverte dans laquelle orthodontiste, implantologiste, chirurgien orthognatique, parodontologiste, prothésiste et omnipraticien doivent s'entendre sur des objectifs consensuels. La réalisation de prothèse apporte aussi de réelles garanties de stabilité dans le domaine de la chirurgie orthognatique

L'objectif de cette étude était d'évaluer les modifications du parodonte après renfort parodontal minéralisé (RPM) (corticotomies + biomatériau de substitution osseuse) réalisés selon le protocole spécifique du Dr Straub. Matériel et méthode : Notre protocole associe corticotomies par piézochirurgie, biomatériau de substitution osseuse NovaBone Dental Putty® et membrane de PRF (Protein Rich Fibrin). Notre échantillon était composé de 7 femmes et 1 homme, les 35 dents étudiées étaient toutes mandibulaires. Sur les images cone beam réalisées avant RPM et après la fin du traitement orthodontique, l'épaisseur d'os en regard des dents et à mi-distance entre les dents a été mesurée à l'apex et à mi-hauteur radiculaire. Le parodonte superficiel a été évalué cliniquement. Résultats: Au niveau des cones beams, les valeurs significatives étaient l'épaisseur osseuse apex - corticale vestibulaire, qui a augmenté de 0,49 mm (± 1,08) (p=0,012), et l'épaisseur osseuse vestibulo-linguale entre les dents au niveau des apex, qui a diminué de 0,46 mm(± 1,04) (p=0,033). Au niveau clinique, aucune récession n' est apparue, ni ne s'est aggravée. Certaines récessions ont même été totalement ou partiellement recouvertes. Les reliefs radiculaires qui étaient visibles par transparence se sont estompés. La hauteur de gencive kératinisée a augmenté. Conclusion : Le RPM a permis un épaississement du parodonte cliniquement visible. Au niveau osseux, les analyses cone beam montrent un léger épaississement de l'os alvéolaire en vestibulaire à l'apex. Cependant, d'autres études sont encore nécessaires afin de valider la pérennité sur le long terme des résultats obtenus."

Le concept de contentions fixes à vie était largement prédominant avec une forte tendance au fil collé mandibulaire et aux attelles en U d'Abjean au maxillaire.
