Cette maladie chronique implique de nombreuses conséquences, non seulement sur la santé générale des individus (cardiovasculaires, endocrinologiques, gastro-intestinales, neurologiques, orthopédiques, rénales), mais également sur leur santé bucco-dentaire. Cependant, l’enfance est une période clef dans de développement bucco-dentaire, il est donc essentiel de pouvoir prévenir et prendre en charge de manière efficace l’obésité le plus tôt possible afin d’éviter ses répercussions à l’âge adulte. De ce fait, une prise en charge multidisciplinaire doit être mise en place pour permettre l’exercice d’une prévention plus efficace et plus poussée.

Le dentiste a tout d'abord un rôle de prévention, il doit expliquer au patient, en cas d'obésité, comment son régime alimentaire peut influer sur sa santé bucco-dentaire mais également il doit adapter sa prise en charge.

Book excerpt: Objectifs : Décrire l'état de santé bucco-dentaire d'enfants et d'adolescents hospitalisés pour la prise en charge de leur obésité, évaluer leurs habitudes en matière de prévention bucco-dentaire et décrire l'impact des paramètres bucco-dentaires sur leur qualité de vie. Méthodes : Cette étude transversale a inclus les patients de moins de 18 ans hospitalisés entre novembre 2020 et janvier 2022 au centre spécialisé de l'obésité du CHU de Bordeaux et ayant eu un bilan bucco-dentaire standardisé incluant un examen clinique et un questionnaire administré à l'enfant avec l'aide d'au moins un parent. Résultats : Parmi les 68 enfants inclus (3-17 ans, 53 % de filles), 13 % n'avaient jamais consulté un chirurgien-dentiste et 51 % se brossaient les dents moins de 2 fois par jour. Une gingivite a été diagnostiquée chez 53 % des jeunes, 31 % présentaient au moins une dent cariée (principalement des molaires permanentes) et 15 % rapportaient des douleurs et/ou des difficultés à mastiquer. Les garçons rapportaient plus souvent brosser leurs dents moins de 2 fois par jour (62 % vs. 42 %, p=0,032), avaient plus souvent du tartre (59 % vs. 33 %, p=0,031) et nécessitaient plus souvent une consultation avec un orthodontiste (44 % vs. 17 %, p=0,030). Les adolescents étaient plus susceptibles d'être gênés par leur sourire que les enfants (29 % vs. 6 %, p=0,015), d'avoir du tartre (60 % vs. 30 %, p=0,014) et une gingivite (66 % vs. 39 %, p=0,030). Conclusion : Cette étude suggère l'intérêt d'inclure une prise en charge et une éducation aux bonnes pratiques bucco-dentaires dans le parcours de soins des enfants hospitalisés pour le traitement de leur obésité.

Méthode: nous avions identifié les articles par une recherche électronique et manuelle basée sur des références d’articles publiés de 2008 à 2013. Tous les articles scientifiques originaux traitant cette association chez les enfants âgés de dix ans et moins ont été inclus dans la revue. Résultats : Les résultats ont moniré qu’il y a association entre surpoids/obésitt intntile et carie dentaire, également entre sous poids et carie dentaire mais le nombre d’études longitudinales de qualité a augmenté et semble se porter en fàveur d’une relation. Le groupe d’age de 5-6 ans semble être un âge de référence pour cette association. De plus, la revue a montré une possible association entre poids et éruption dentaire précoce. Des cocteurs tels que la situation socio-économique, l’alimentation riche en sucreries semblent aussi être impliqués. Conclusion: l’inconsistance de la relation entre carie dentaire et surpoids/obésité est peut-être due en partie à des méthodes d’études différentes et des indices de mesures divergents. Nous reconunandons, pour les études à venir, de chercher à déterminer les rôles des cofacteurs dans cette association et une mise en oeuvre d’une approche pluridisciplinaire.

In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.
