La diabetes gestacional es aquella que se diagnostica o se conoce por primera vez durante el embarazo. Podría existir previa al embarazo, pero se desconocía. Se produce porque el embarazo es una sobrecarga importante para el metabolismo de una mujer a la que el organismo se debe adaptar. Cuando esta adaptación no ocurre adecuadamente se desarrolla la diabetes. Es, por lo tanto, la disminución de la tolerancia a los hidratos de carbono (HC) que se diagnostica por primera vez durante la gestación, independientemente de la necesidad de tratamiento insulínico, grado del trastorno metabólico o su persistencia una vez finalizado el embarazo. Este diagnóstico obliga a una reclasificación de la alteración metabólica en el postparto (2). La DM constituye la alteración metabólica que más frecuentemente se asocia al embarazo, afectando al pronóstico de la madre y al de su futuro hijo. Aproximadamente un 1% de todas las mujeres embarazadas presentan DM pregestacional (DMPG) y hasta un 12% de los casos, dependiendo de la estrategia diagnóstica empleada, presentará diabetes gestacional (DMG) en el transcurso del embarazo (3). El incremento de los malos hábitos alimentarios y la inactividad física que prevalece hoy día en la población general, específicamente en la población en edad reproductiva, ha aumentado la prevalencia de obesidad y trastornos del metabolismo de los carbohidratos; sin duda, en las mujeres embarazadas estos eventos están ligados con el incremento de la prevalencia de la DMG (5). Como veremos a lo largo del capítulo, la aparición de diabetes en el embarazo constituye un factor predictivo de aparición posterior de DM tipo 2 y síndrome metabólico (dislipiemia, obesidad e hipertensión asociada). Ocasionalmente la DMG está manifestando una disminución de reserva pancreática secundaria a destrucción autoinmune de la célula beta (DM tipo1 latente), dando lugar posteriormente a una DM tipo1.
