The decline in several indicators of welfare for the sample population that occurred after the beginning of the civil conflict in Yemen is also traced. Overall, the program decreased the share of children diagnosed with moderate or severe malnutrition and improved anthropometric indicators of nutritional status in children in the poorest third of households. The Cash for Nutrition program was funded by the World Bank through the United Nations Development Programme as part of the Yemen Emergency Crisis Response Project.

Cash gives people choices and thereby preserves their dignity. Commodity distribution may pose logistical problems, takes time, and in the case of food aid, may disrupt local markets if food is actually available within the affected country or region. But among humanitarian agencies there are fears that cash transfers will pose security risks, create inflation, and fail to be used to meet basic needs. In this guide, the first of its kind, Oxfam staff members present the rationale behind cash-transfer programs, considering the arguments for and against cash as an alternative to commodity distribution. They also give guidance on when cash is the most appropriate intervention and how to assess this. Different types of cash intervention are compared--cash grants, vouchers, and cash-for-work--and the guide uses checklists to explain the practical steps involved in implementing them. They draw on the experience of Oxfam and other agencies of operating such programs, including responses to the devastation caused by the Indian Ocean tsunami in December 2004. The guidelines are primarily intended for NGO personnel: humanitarian program managers, food-security specialists, public-health engineers, finance staff, and logisticians. Policymakers in donor organizations and international agencies will also find them relevant. The sixteen cards contain key elements from the book to explain how to assess whether cash is the most appropriate response to any particular emergency. That is, the government only transfers the money to persons who meet certain criteria. These criteria may include enrolling children into public schools, getting regular check-ups at the doctor's office, receiving vaccinations, or the like. They have been hailed as a way of reducing inequality and helping households break out of a vicious cycle whereby poverty is transmitted from one generation to another. Do these and other claims make sense? Are they supported by the available empirical evidence? This volume seeks to answer these and other related questions. Specifically, it lays out a conceptual framework for thinking about the economic rationale for CCTs; it reviews the very rich evidence that has accumulated on CCTs; it discusses how the conceptual framework and the evidence on impacts should inform the design of CCT programs in practice; and it discusses how CCTs fit in the context of broader social policies. The authors show that there is considerable evidence that CCTs have improved the lives of poor people and argue that conditional cash transfers have been an effective way of redistributing income to the poor. They also recognize that even the best-designed and managed CCT cannot fulfill all of the needs of a comprehensive social protection system. They therefore need to be complemented with other interventions, such as workfare or employment programs, and social pensions.

However, little is known about how doing so affects household economic well-being. We study a program providing cash or food transfers, with or without nutrition behavior change communication (BCC), to poor women in rural Bangladesh. We find that adding BCC to cash or food transfers leads to larger impacts on both consumption and assets - an apparent puzzle, given the transfer value is unchanged. Evidence suggests this occurs through the BCC inducing increases in income generation - plausibly by improving households’ social capital and empowerment.

Without proper gender considerations, there is a concern that cash transfers may fail to reach those left furthest behind, potentially limiting rather than generating opportunity for greater gender-transformative change. This paper begins by presenting an overview of the latest research on cash transfers, gender protection, and empowerment outcomes. It continues by discussing some of the programme design features to consider when seeking to improve gender outcomes. Finally, the paper concludes with a set of research questions that can help shape future research and practice in this area.

Global evidence suggests that cash transfer programs can reduce poverty and food insecurity and can build resilience for the poor. When cash transfer programs are com-bined with nutrition interventions, they also have the potential to accelerate improvements in child nutrition, especially when targeted to the critical window of opportunity for nutrition, the first one thousand days of a child’s life (Ruel et al., 2013). In West Africa, many cash transfer programs are combined with accompanying measures such as promotion sessions that aim to improve knowledge and increase adoption of recommended behaviors—including those related to child nutrition (Beegle et al., 2018, see Box 1). However, the extent to which such multi-component programs lead to changes in behavior and improve-ments in outcomes related to children’s nutrition and health is still not well-understood.

Book excerpt: The importance of children’s nutritional status for subsequent human capital formation, the limited evidence of the effectiveness of social protection interventions on child nutrition, and the absence of knowledge on the intra-household impacts of cash and food transfers or how they are shaped by complementary programming motivate this paper. We implemented two, linked randomized control trials in rural Bangladesh, with treatment arms including cash transfers, a food ration, or a mixed food and cash transfer, as well as treatments where cash and nutrition behavior change communication (BCC) or where food and nutrition BCC were provided. Only cash plus nutrition BCC had a significant impact on nutritional status, but its effect on height-forage z scores (HAZ) was large, 0.25SD. We explore the mechanisms underlying this impact. Improved diets – including increased intake of animal source foods – along with reductions in illness in the cash plus BCC treatment arm are consistent with the improvement we observe in children’s HAZ.

Book excerpt: Many development programs that aim to alleviate poverty and improve investments in human capital consider womens empowerment a key pathway by which to achieve impact and often target women as their main beneficiaries. Despite this, womens empowerment dimensions are often not rigorously measured and are at times merely assumed. This paper starts by reflecting on the concept and measurement of womens empowerment and then reviews some of the structural interventions that aim to influence underlying gender norms in society and eradicate gender discrimination. It then proceeds to review the evidence of the impact of three types of interventionscash transfer programs, agricultural interventions, and microfinance programson womens empowerment, nutrition, or both. Qualitative evidence on conditional cash transfer (CCT) programs generally points to positive impacts on womens empowerment, although quantitative research findings are more heterogenous. CCT programs produce mixed results on long-term nutritional status, and very limited evidence exists of their impacts on micronutrient status. The little evidence available on unconditional cash transters (UCT) indicates mixed impacts on womens empowerment and positive impacts on nutrition; however, recent reviews comparing CCT and UCT programs have found little difference in terms of their effects on stunting and they have found that conditionality is less important than other factors, such as access to healthcare and child age and sex. Evidence of cash transfer program impacts depending on the gender of the transfer recipient or on the conditionality is also mixed, although CCTs with non-health conditionalities seem to have negative impacts on nutritional status. The impacts of programs based on the gender of the transfer recipient show mixed results, but almost no experimental evidence exists of testing gender-differentiated impacts of a single program. Agricultural interventionsspecifically home gardening and dairy projectsshow mixed impacts on womens empowerment measures such as time, workload, and control over income; but they demonstrate very little impact on nutrition. Implementation modalities are shown to determine differential impacts in terms of empowerment and nutrition outcomes. With regard to the impact of microfinance on womens empowerment, evidence is also mixed, although more recent reviews do not find any impact on womens empowerment. The impact of microfinance on nutritional status is mixed, with no evidence of impact on micronutrient status. Across all three types of programs (cash transfer programs, agricultural interventions, and microfinance programs), very little evidence exists on pathways of impact, and evidence is often biased toward a particular region. The paper ends with a discussion of the findings and remaining evidence gaps and an outline of recommendations for research.

Children with low birth weight do not only include premature babies, but also those with intrauterine growth restrictions who consequently have a very high risk of developing metabolic syndrome in the future. Epidemiology, epigenetic programming, the correct nutrition strategy and monitoring of outcomes are thus looked at carefully in this book. More specifically, two important nutritional issues are dealt with in depth: The first being the prevention of low birth weight, starting with the health of adolescent girls, through the pre-pregnancy and pregnancy stages and ending with lactation. The second point of focus concerns the nutritional follow-up and feeding opportunities in relation to dietary requirements of children with low birth weight.

Book excerpt: For many Americans who live at or below the poverty threshold, access to healthy foods at a reasonable price is a challenge that often places a strain on already limited resources and may compel them to make food choices that are contrary to current nutritional guidance. To help alleviate this problem, the U.S. Department of Agriculture (USDA) administers a number of nutrition assistance programs designed to improve access to healthy foods for low-income individuals and households. The largest of these programs is the Supplemental Nutrition Assistance Program (SNAP), formerly called the Food Stamp Program, which today serves more than 46 million Americans with a program cost in excess of $75 billion annually. The goals of SNAP include raising the level of nutrition among low-income households and maintaining adequate levels of nutrition by increasing the food purchasing power of low-income families. In response to questions about whether there are different ways to define the adequacy of SNAP allotments consistent with the program goals of improving food security and access to a healthy diet, USDA's Food and Nutrition Service (FNS) asked the Institute of Medicine (IOM) to conduct a study to examine the feasibility of defining the adequacy of SNAP allotments, specifically: the feasibility of establishing an objective, evidence-based, science-driven definition of the adequacy of SNAP allotments consistent with the program goals of improving food security and access to a healthy diet, as well as other relevant dimensions of adequacy; and data and analyses needed to support an evidence-based assessment of the adequacy of SNAP allotments. Supplemental Nutrition Assistance Program: Examining the Evidence to Define Benefit Adequacy reviews the current evidence, including the peer-reviewed published literature and peer-reviewed government reports. Although not given equal weight with peer-reviewed publications, some non-peer-reviewed publications from nongovernmental organizations and stakeholder groups also were considered because they provided additional insight into the behavioral aspects of participation in nutrition assistance programs. In addition to its evidence review, the committee held a data gathering workshop that tapped a range of expertise relevant to its task.
