When nurses check on patients, their focus is to address the 4 Ps, positioning, potty (elimination needs), pain, and possessions (proximity of personal belongings). (Halm, 2009) At the time of admission, patients and their family members will be informed of the hourly rounding process in order to set realistic expectations of the plan of care. Research performed in 14 hospitals has found that by setting these goals, the use of call lights have decreased by 37%, patient falls decreased by 52%, and patient satisfaction scores increased by 12%. But nurses attempt to be proactive on avoiding these issues are impaired by factors like high patient ratios, lack of time , frequent call lights and stress from meeting the patients demands. Planned hourly rounding is a proactive approach to patient care. Hourly rounding reduces patient fall, skin break downs, call light use, noise level and also improve patient satisfaction. Studies on hourly rounding to increase the patient safety and patient satisfaction showed a 52% decline in call light use after implementing hourly rounding and unit was quiet and nurses could spend more time for patient care and logs (Ford, 2010). Intentional rounding researches showed significant reduction in fall rate with one hour rounding. With the implementation of hourly rounding on the stroke and cardiac inpatient unit of the Texas Health Resources Hospital, the purpose is to prevent unwanted events and to improve patient satisfaction and safety.

The staff can also benefit by reinforcing the importance of hourly rounding through the yearly house side mandatory review session accessed through the hospital's intranet website. This proposal will include an implementation plan that shows the positive benefits implementing hourly rounding re-education given to nurses and nursing assistants has improved patient outcomes, specifically improved patient safety as seen by a decrease in falls, and improved patient satisfaction. In addition, evaluation of the findings as well as disseminating them to all unit department directors and the nursing community will be discussed.

Hourly rounding has been shown to decrease call light usage, falls and pressure sores as well as increase patient satisfaction. 'Margo A. Halm, RN, PhD, CNS-BC, director of nursing research and quality at Salem Hospital in Oregon, reviewed 11 hourly rounding studies and reported in a 2009 American Journal of Critical Care article that 83 percent of studies examined showed a reduction in call lights, 77 percent a decrease in fall rates and 88 percent an improvement in patient satisfaction' (Wood, 2014, para. 8). 'Swanson's Nursing Care Theory supports the purposed use of hourly rounding as it centers on practices that promote patient satisfaction and safety by focusing on the connection between caring processes and patient well-being and outcomes' (Tongues, Ray, 2011, pages374). Hourly rounding was introduced previously at this facility but the implementation was poorly received by nurses. In an effort to reintroduce hourly rounding, a team will be formed to champion the process, a presentation to the stakeholders will be done outlining the plan as well as the costs involved, and ongoing education will be provided for nursing as well as tools to aid the nurse in incorporating hourly rounding into their daily routines. Weekly meetings with a specific agenda will be held encouraging staff feedback on how the hourly rounding trial is proceeding. At the end of the trial period an assessment will be given to each nurse seeking their ideas and any new problems that need to be addressed. The team will evaluate data from the meetings, patient's comments and any occurrence reports concerning falls and pressure sores and call light usage information obtained from the facilities call light system. This information will be shared with the stake holders as well as nursing staff. As the nursing shortage increases, hospitals as well as nursing leaders are searching for ways to provide quality care, enhance patient safety and satisfaction and to increase nurse's satisfaction. 'Hourly patient rounding proposes to be one answer to both patient and nurse satisfaction' (Wood, 2014, para.6).

This proposal will include an implementation plan and give a clear picture of the resources and tools needed to make this plan a success. Once this plan has been implemented for the time of one year there will be a re-evaluation on the comfort levels of the staff with hourly rounding and data will be collected on the number of documented falls that have or have not occurred during this time. The goal of this presentation is to improve patient safety and improve overall staff satisfaction.

Nurse hourly rounding system will include hourly rounding logs or forms placed in each patient's room that require a signature from then nurse, the time entered and spent in the room as well as the care provided. Locator badges will be worn by both aide and nurse and will be used to monitor and accurately track movement in and out of patients' rooms with the time of each occurrence as this is shown to deliver consistent and valuable information for better patient care outcomes. Reports will be generated to measure patient satisfaction and patient care. Another benefit will be the reduction of noise from the call light system. These measures will be taken so that data and documentation can be used to monitor the positive impact on risk management and patient/staff satisfaction.

Book excerpt: In the United States, it is estimated that 700,000 to 2,000,000 individuals have a fall in the acute hospital setting with one in three individuals aged 65 years and older. Yearly, 20-30 percent of hospitalized patients suffer an injury that could have been prevented (Ganz, Saliba, Huang and Shier, 2013). Based on documented studies, implementing purposeful rounding or intentional rounding decreases call lights, alarms, falls, increases patient and employee satisfaction, supports a more productive shift for nursing staff, and most importantly increases patient safety (Olrich, Kalman, and Nigolian, 2012). Several documented studies have reported a decrease in fall rates post implementation of intentional rounding, one hospital reported a decrease in falls by 23 percent post implementation (Olrich, Kalman, and Nigolian, 2012). Falls in the acute hospital setting are a serious complex problem, addressing and implementing purposeful or intentional hourly rounding while better serving patients will decrease the fall rate in the acute hospital setting. Based on several journal readings successful implementation of intentional rounding or purposeful rounding that included the nursing staff in the implementation process yielded the most rewarding results. Identifying the problem or issue, developing an implementation plan and evaluation plan, along with finding the best avenues to disseminate the evidence are key to successful implementation of any project.
