Bedside reporting (bedside handover/handoff) has proven to be direct and practical method to achieve high quality safe care. Joint Commission announced that communication breakdowns are the rooted cause to sentinel events. Therefore, the impact of poor shift reporting can result in unsafe patient outcomes as well as in the work environment. In some studies, nurses perceived poor communication in report methods such as traditional reporting due to receiving impertinent information and wasted time. Time is limited with patients due to the length of stays. Lewin's theory of change can be utilized to help transform to bedside reporting. Through many research studies, bedside reporting exhibited evidence-based practices that cultivate patient involvement by fostering patient/nurse trust/bonds, integrates patient in the care team, and promotes accountability for all and prioritizing care. The purpose of this project is to guide the nurse leader/nurse manager and staff nurses an educational plan consisting of visuals on bedside reporting. The implementation plan includes scripts to aid nurses for the optimal report. By visualizing the patient during report, nurses will be able to address patient's needs, perform safety scans and involve patient/family in the plan of care. In a study, there were increased nurses' satisfactions, accountability, patient involvement as well as decrease in 35% in falls and 50% decrease in medication errors. Pre/post implementation surveys are given to nurses in order to pinpoint barriers and revise the process. Nurse managers, administration and staff nurses are the recipients of disseminating evidence to enhance bedside reporting and continue ongoing evaluations.

It is important for nurses to explore the patient's perception on being isolated from report and develop a new way of engaging them in their care. Bedside reporting offers a feasible solution for patients to engage in their plan of care and to become partners in their care. An oncology nursing unit at Banner Gateway Medical Center is currently giving shift report away from the patient, causing a decrease in satisfaction scores. The purpose of this project proposal is to examine bedside reporting compared to traditional reporting at the nurses' station and the impact on patient satisfaction. This project will provide a description of the problem, proposed solution, and a detailed implementation, evaluation, and dissemination plan. Evidence suggests that bedside reporting is more patientcentered and comprehensive ensuring that quality care is delivered from shift to shift. Furthermore, this reporting method improves the patient's experience of care, promotes nurse and patient engagement, increases time spent providing direct care, improves communication, and enhances nursing practice. Furthermore, research shows that bedside shift report allows the oncoming nurse to visualize the patient, perform a quick assessment, answer patient questions, confirms the plan of care, and establishes patient goals for the shift. This new method of reporting will improve nursing practice, outcomes, and patient satisfaction.

Book excerpt: Transfer of care report is one of the most frequent hand-off communication opportunities in healthcare for nurses. The goal of this report is to provide information in a standardized format that allows accurate data to be transferred to the oncoming nurse in a clear, concise manner. Literature reveals this report occurring at the patients is the best nursing practice; however, it does come with obstacles, competing priorities, and distractions. A breakdown in communication can cause disruptions to care, inaccurate information, or even harm and death to the patient in certain circumstances. Identifying a standardized approach to bedside report is a priority for patient safety and the hospital’s safety culture. The Nurse Knowledge Exchange Plus (NKE plus) created by Kaiser Permanente was designed with a multifaceted team and front-line nursing staff to engage the team in helping break down the obstacles and distractions. NKE plus was chosen as the standardized tool for beside reporting for this project because of the engagement of staff, ability to work through barriers and distractions, and provide a comprehensive tool that uses the electronic health record, safety checks, and inclusion of the patient and family (Lin, Heisler, Fahey, McGinnis, & Whiffen, 2015). This evidence-based quality improvement project looked at the nurse’s perceptions of bedside reports and Press Ganey’s communication with nurse's scores for improvements.The results of this Doctorate of Nursing Practice (DNP) project did not support the evidence that NKE plus improved nurses’ perception of bedside reports or improved communication with nurses’ scores. However, several areas did show statistical significance. Those areas included: (a) the nurse I follow from a previous shift completes job responsibilities; (b) interpersonal relationships between shifts are good; (c) the patient’s condition matches what I get in the report; and (d) the current system of report fosters a partnership between the nurse, patient, and families. The results were skewed due to a lack of surveys for both the nursing questionnaire and patient surveys. Further dissemination of this project includes completing NKE plus throughout the other three units within the hospital because of the evidence supporting including patient and family during bedside report.

Book excerpt: Nurse to nurse handoff has been known as a sacred time in nursing communication and has had a long standing in performing this task without the patient. Recent research has shown an increase in patient safety, nurse satisfaction and patient satisfaction when nurse-to-nurse handoff occurs at bedside and includes the patient. Bedside report has been shown to increase the perception of safety in the patient, identify potential medication errors before they occur, and influence a sense of satisfaction among nurses. The nurses that practice this style of reporting are facilitating a connection between the patient and the nursing staff. A review of the literature seeking studies reflective of a change to bedside report has been conducted. With the use of Lewin09́s Change Theory an implementation, evaluation and dissemination plan has been established to support an Adult Medical Unit09́s change to bedside shift report. Each phase of the plan utilized evidence based best practice to support the change to bedside report. In implementing a standardized report process, this unit09́s goal was to improve patient safety, patient satisfaction, and nurse satisfaction. Surveys were conducted before and after implementation of the change along with real time audits to provide feedback to staff. Additional tools were provided to staff to further support their transition to bedside report. Through the literature review conducted, other near miss situations were noted that further supported the implementation of bedside shift report. These included medication errors, infiltrated intravenous sites, and incorrect laterality of scheduled surgeries. It is also noted that bedside report is supported by the Institute of Medicine and The Joint Commission who aim to support health care to be provided in the safest manner.

Nurses were better prepared and were able to better assess patient needs after implementation of bedside shift report. Nurses were held more accountable and patients became more involved in their care. This new process gave the oncoming nurse the opportunity to visualize the patient and ask questions. Finding of this report confirmed how important having a standardized report system is in reducing errors. Accurate and consistent communication was improved after the implementation of bedside reporting. Effective communication at shift change is a fundamental component of nursing and the culture of safety. This study was conducted within a large hospital on medical and surgical units. A survey was obtained before the implementation of bedside report and several times over the first year post. Along with the use of surveys visual observations were done as well as in person interviews. Results of this study show that bedside report increased nurse's satisfaction, helped in the prioritization of patients which increased work flow, and decreased the amount of time for handoff.

Bedside handoff is more beneficial for patients and nurses rather than the current practice happening in hospitals. Through a literature review of articles it has been determined that there is no standard for nursing shift handoffs. The Joint Commission has made a patient safety goal and proposed that there be a standardized handoff communication in hospitals. Performing shift change at the bedside with a standard template to be used at the change can be the new standard and will increase patient care and help nurses feel more empowered with the communication received and given. The results showed that the information being reported previously to bedside shift change was often poor, and that much of the information was easily accessed in the chart and did not need to be part of report. They also found that the inefficiencies in communication is a factor associated with sentinel events. Making the switch to bedside shift reporting and making it the standard in hospitals around the world can make a safer, smarter tomorrow. This project will outline the problem of the current shift change process, how to implement the change, the theory that goes into the change, how to evaluate the implementation, and how dissemination can be done. There is a literature review, an appendix and a complete reference list that are all included.

Book excerpt: One of the fundamental elements of nursing practice is the transfer of patient information from one provider to another known as a bedside report. However, an unstructured bedside report that occurs away from the bedside, places patient safety, quality of patient care, and patient satisfaction in jeopardy. The purpose of this quality improvement (QI) project is to determine if the implementation of bedside reporting in an acute care pediatric unit will: decrease preventable errors reported through an event reporting system known as Safety Tracker, increase patient satisfaction in provider communication, and improve nurse accountability. The study included a convenience sample of 65 inpatient bedside nurses on a 30-bed, acute care pediatric unit in a large urban hospital. The participants were recruited on a voluntarily basis and informed consent was obtained prior to the study. Data were collected through pre-and post-questionnaires. Each questionnaire included, demographic data and two open-ended questions relating to the new Situation, Background, Assessment, Recommendation (SBAR) bedside handoff reporting tool. Education regarding the pre-and post-questionnaires and the new SBAR handoff reporting tool was provided by the primary investigator. Fifty-five participants, or (85%) completed both questionnaires for comparison. Results of this QI project indicated participants liked the consistency and structure of the new SBAR reporting tool; however, barriers identified were arousing and discussing sensitive information at the patient’s bedside. Patient satisfaction scores increased from 68.4% to 75%.

Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.
