Book excerpt: In Nigeria, the scale-up of free antiretroviral treatment (ART), proactive anti-discrimination laws, and social awareness campaigns have contributed to bringing Nigeria closer to ending AIDS by achieving "Zero new infections, Zero AIDS related deaths and Zero discrimination by 2021." In the fight to end maternal and pediatric HIV/AIDS, however, multiple, intersecting stigmas have made stagnant any further gains in Prevention of Mother to Child Transmission of HIV (PMTCT) service delivery. As in the rest of Sub-Saharan Africa, Nigeria still carries a high overall burden of HIV. Maternal/pediatric HIV are troubling consequences of inadequate health services for HIV+ women in the region. We explored the interconnecting factors that perpetuate continued vertical transmission of HIV from mother to child in rural northwestern Nigeria. We conducted in-depth, semi-structured ethnographic interviews with 27 HIV+ pregnant women, aged 17 to 32 years, in four government-run rurally located secondary healthcare hospitals offering PMTCT services. Based on field notes and transcripts, we used grounded theory and thematic analysis and found that intersectional stigmas continue to create significant barriers to women living with HIV's ante- and post-natal ART adherence. Key strategies women employed to access needed care by keeping appointments, collecting medications and taking medications on time included peer support groups, spousal disclosure, child survival and economic autonomy. For optimal maternal/pediatric HIV services, providers should support established models these women already employ. If an AIDS-free generation is to be realized by 2030, Nigeria must also strengthen and enforce anti-HIV stigma and discrimination laws.

It is imperative that midwives, nurses, and other health care providers are therefore equipped with the research and advocacy tools necessary to respond and account for such needs.Purpose: To share the preliminary findings of a case study which explored resiliency and capacity building within the context of collaborative partnerships between maternal-child health researchers in Canada and Nigeria.Methods: Stakeu2019s (1995) principles of qualitative case study guided this research. Following ethical approval, data was collected using individual face to face and skype interviews. Interviews were conducted with key stakeholders located in Nigeria and Canada. Interviews were transcribed verbatim and thematic analysis was used for analysis. Findings: Findings will be presented as they relate to the ongoing collaborative relationships between maternal-child health researchers in Nigeria and Canada. Presented findings will address; strengths of the relationships, challenges of conducting global maternal-child health research, and ideas to enhance the capacity of maternal-child health research in LMICs.Conclusions: Ongoing global partnerships and mentoring are crucial for building research capacity in LMICs. Resiliency and leadership are key to the maintenance of such relationships and mentoring. Implications for practice: These findings have the potential to inform policies that aim to strengthen maternal-child health research globally.

There is therefore a need for greater clarity about what exactly implementation research is, and what it can offer. This Guide is designed to provide that clarity. Intended to support those conducting implementation research, those with responsibility for implementing programs, and those who have an interest in both, the Guide provides an introduction to basic implementation research concepts and language, briefly outlines what it involves, and describes the many opportunities that it presents. The main aim of the Guide is to boost implementation research capacity as well as demand for implementation research that is aligned with need, and that is of particular relevance to health systems in low- and middle-income countries (LMICs). Research on implementation requires the engagement of diverse stakeholders and multiple disciplines in order to address the complex implementation challenges they face. For this reason, the Guide is intended for a variety of actors who contribute to and/or are impacted by implementation research. UNICEF's flagship publication, The State of the World's Children 2009, addresses maternal mortality, one of the most intractable problems for development work.The difference in pregnancy risk between women in developing countries and their peers in the industrialised world is often termed the greatest health divide in the world. A woman in Niger has a one in seven chance of dying during the course of her lifetime from complications during pregnancy or delivery. That's in stark contrast to the risk for mothers in America, where it's one in 4,800 or in Ireland, where it's just one in 48,000. Addressing that gap is a multidisciplinary challenge, requiring an emphasis on education, human resources, community involvement and social equality. At a minimum, women must be guaranteed antenatal care, skilled birth attendants and emergency obstetrics, and postpartum care. These essential interventions will only be guaranteed within the context of improved education and the abolition of discrimination.

Book excerpt: Mother To Child Transmission (MTCT) of HIV is a major challenge in Sub-Saharan Africa due to a variety of socio-economic and political factors. In South Africa, for example, there was element of denialism by government on the fact that treatment could assist in reducing the likelihood of transmitting the virus to the baby. It was only in 2001 after the Treatment Action Campaign took the South African government to court that they were ordered to develop a programme to prevent the transmission of HIV from mother to child. The South African government established a PMTCT programme that aligned itself with the United Nations' Millennium Goal of ensuring that all HIV positive pregnant women receive treatment in an effort to eliminate babies born with HIV. This study, therefore, explored the experiences of HIV positive mothers who enrolled on PMTCT programme at Tonga hospital. The researcher explored the participants' understanding of the programme prior to enrolment, the extent to which they experienced the programme, the nature of support and services they received while on the programme as well as challenges they faced. All this was done with the intention and commitment to strengthening the intervention strategies for the HIV pregnant women, thereby ensuring that they receive top quality services from a group of multi skilled professionals. To achieve this, the study applied a collective case study within a qualitative approach. The population for the study was HIV positive mothers who enrolled on the PMTCT programme between June 2011 and July 2012. The sample consisted of 12 HIV positive mothers who took part in the PMTCT programme at Tonga hospital. For data collection purposes, the researcher applied semi-structured interview. Informed by the findings, the study concluded that there is lack of knowledge and understanding of the programme in the community. It further established that the clinic is situated under the ward where HIV positive patients are treated and that psycho social services are not offered to the HIV positive pregnant women. It also concluded that the women's husbands or partners are not encouraged to be part of the programme. Consistent with the above, the study recommended that the PMTCT programme should be provided in a holistic and well integrated manner, where all health care disciplines contribute as required to make the participants' experiences on the programme more comfortable both socially and emotionally. There should be a way to get the male counterparts of the participants more involved in the programme so as to afford them the opportunity to get first-hand information about pregnancy and what is expected of them as partners. Each health worker's role should be clearly defined and a referral system be developed so that all services are easily accessible.
