Common pathogens, once believed to be under control, acquire increased virulence and resistance to drugs, while exotic microorganisms emerged from hidden reservoirs to cause yet incurable diseases in humans. These changes, together with epidemic outbreaks related to political and socio-economic instabilities, increase the needs for the development of new, advanced vaccines. In this volume, devoted to the proceedings of the 39th OHOLO Conference, we present some of the recent strategies for the design and production of novel vaccines. The advent of recombinant DNA technology has stimulated the production of several subunit vaccines. In spite of the obvious advantages to this approach, the limited immuno genicity of many subunit candidates has hindered their development. Strategies to enhance the immunogenicity of subunit vaccines is therefore critical. Several approaches toward this goal, including design of novel adjuvants and delivery systems as well as design of advantageous carriers, are presented here. Among the carriers evaluated here are polypep tides (flagellin, HBV core antigen, J3-galactosidase), attenuated virions (Vaccinia, Sindbis), and nonpathogenic licensed bacteria (Salmonella).

Book excerpt: When my interest was first drawn to the phenomenon of vaccination for virus diseases in the late 1930s, the state of the art and the science of vaccine design was not far advanced beyond the time of Jenner at the end of the 18th century and of Pasteur a century later. In the 1930s it was still believed that for the induction of immunity to a virus-caused disease the experience of infection was required, but not for a toxin-caused disease such as diphtheria or tetanus, for which a chemically detoxified antigen was effective for immu nization. This prompted the question as to whether it might be possible to produce a similar effect for virus diseases using nonreplicating antigens. When in the 1930s and 1940s it was found possible to propagate influenza viruses in the chick embryo, protective effects could be induced without the need to experience infection by the use of a sufficient dose of a noninfectious influenza virus preparation. Later in the 1940s, it became possible to propagate polio and other viruses in cultures of human and monkey tissue and to immunize against other virus diseases in the same way. Later, with the advent of the era of molecular biology and genetic engineering, antigens and vaccines could be produced in new and creative ways, using either replicating or nonreplicating forms of the appropriate antigens for inducing a dose-related protective state.

However, that progress has stagnated in recent years, leaving an estimated 20 million children worldwide either undervaccinated or completely unvaccinated. The determinants of vaccination uptake are complex, mutable, and context specific. A primary driver is vaccine hesitancy - defined as a "delay in acceptance or refusal of vaccines despite availability of vaccination services". The majority of vaccine-hesitant people fall somewhere on a spectrum from vaccine acceptance to vaccine denial. Vaccine uptake is also hampered by socioeconomic or structural barriers to access. On August 17-20, 2020, the Forum on Microbial Threats at the National Academies of Sciences, Engineering, and Medicine held a 4-day virtual workshop titled The Critical Public Health Value of Vaccines: Tackling Issues of Access and Hesitancy. The workshop focused on two main areas (vaccine access and vaccine confidence) and gave particular consideration to health systems, research opportunities, communication strategies, and policies that could be considered to address access, perception, attitudes, and behaviors toward vaccination. This publication summarizes the presentations and discussion of the workshop.

Book excerpt: Emerging and re-emerging infections, in particular those caused by viruses, are expected to rise in correlation, among other factors, with climate changes. Antibiotic resistance is another issue that will limit the therapeutic arsenal against bacterial and parasitic infections. Therefore, one must be adequately prepared to overcome and prevent current and novel infections, and vaccination remains the optimal way to fight infectious diseases in humans. This is also true for many impacting diseases of livestock and companion animals, to which there are no available vaccines. First (attenuated and inactivated) and second (subunit) generation vaccines have their limitations. To overcome this, efforts have been made in recent years to develop novel vaccine types such as virus-like particle (VLP)-, viral vector-, and DNA-based vaccines in humans and animals. The recent success in utilizing RNA technology based-vaccines to fight SARS-CoV-2 to limit the severity of clinical symptoms and mortality constitutes a promising way to develop such vaccines against animal diseases. Development of novel molecules or materials with adjuvant or immunostimulatory activity to adequately stimulate the innate immune response, and, therefore the pathogen-specific adaptative immune response, as well as the identification of suitable antigenic material that would bring broad protection against infectious agent variants are other key issues to be considered in developing safe and innovative immunization strategies.
