Néanmoins, l’efficacité de la thrombolyse est influencée par la composition du thrombus : lorsqu’il est riche en plaquettes, il est résistant au rtPA. En modélisant in vivo 2 étiologies (embolique et thrombose in situ) de l’AVC ischémique, l’objectif de ce travail a été d’étudier l’efficacité de la thrombolyse et les mécanismes de la thrombose. Dans les conditions ischémiques, la limitation des dommages cérébraux dépend du rétablissement rapide de la perfusion sanguine cérébrale mais également de l’intégrité de la barrière hémato-encéphalique (BHE). La métalloprotéinase ADAMTS-4 étant impliquée dans la dégradation des composants de la matrice extracellulaire, nous avons étudié son rôle dans la perméabilisation de la BHE. Nos résultats démontrent que la thrombose est un processus en 2 étapes : les récepteurs aux glycoprotéines (Gp) IIb/IIIa sont impliqués dans la phase initiale de l’agrégation plaquettaire alors que la fermeture vasculaire dépend de l’interaction du facteur von Willebrand (VWF) avec les récepteurs GpIbα. Inhiber l’interaction VWF-GpIbα permet de lyser le caillot sanguin, de rétablir la perfusion sanguine cérébrale et par conséquent de réduire les dommages cérébraux. Nos résultats suggèrent également qu’ADAMTS-4 contribuerait dans les conditions ischémiques au maintien de l’intégrité de la BHE. L’interaction VWF-GPIbα et ADAMTS-4 sont donc de potentielles cibles d’intervention thérapeutiques de l’AVC ischémique.
