Graduates of the Faculty of Medicine of mcgill University will recognize that - the views enunciated closely follow the teaching of the former Professor Of Dermatology, Dr. Francis J. Shepherd, whose almost uncanny astuteness in diagnosis and simple methods of treatment the writer had the advantage of following for twenty years as his assistant in the Montreal General Hospital clinic. In choosing the diseases which Should be included in a work of this scope, the writer has based his selection on the statistics of the hospital clinic with which he is connected, thereby, possibly, admitting some not properly classed as common, because of the large proportion of aliens in his clientele. One theory that has been handed down to us from a very early age, is that of the ba mors which assumes that the majority of diseases originate in a morbid condition of the humors of the body, and especially of the blood. This is the foundation of the Humoral Pathology, and the disciples of this doctrine are the Humoralists. Humoralism was early adopted as an explanation of cutaneous disease; and the practice which has been founded upon that theory has been most successful in the treatment of these diseases. Theory assumes a morbid alteration of the blood; practice exerts all its powers to remove the offending constituents of the blood, to improve the condition of the blood; in fact, to purify the blood. I am sorry this delay should have occurred, as Mr. Davies more than once informed me that several members of the profession had applied for copies; but I can only express my regret, as I still consider it better that it should not have appeared than have come before the world in an imperfect form. When read before the Medico Chirurgical Society, it was too condensed to bear reprinting except as a pamphlet. The parasitic nature of some diseases of the skin could not be ignored in a work devoted to treatment. Some supporters of the vegetable theory contend that in certain of these affec tions there is no necessity for general treatment of an active kind, often for no general treatment at all, and that a speedy and lasting cure can be effected by local means alone; whereas when these means are neglected, or made merely auxiliary to constitutional measures, progress must ever be slow and success uncertain. On the other hand, such eminent writers as Wilson, Carpenter, and Erichsen, deny the vegetable nature of these diseases; Wilson even considering local remedies as of secondary importance, and, when not conjoined with means calculated to improve the health, as too often powerless. The relative preponderance of foreigners over natives is thereby considerably reduced, and the proportion of serious over trivial affections correspondingly increased, as the number of the latter seek ing relief from a distance is naturally small. The nature of the cases, however, is as. Diverse as is usually presented in such lists, representing, as they do, such various relations of life for it is by no means the poor alone who avail themselves of the gratuitous advice of the department. Most notable differences, however, will be discovered in the' character of the diseases by comparing the list below with those drawn from the private case-book of the dermatologist; the extremes being, on the one hand, the representatives of poverty and filth, on the other of luxury and the refinement of bodily care. But no one without the experience derived from the study of both classes can form an intelligent opinion of the nature of the cutaneous diseases of any community.
