Book excerpt: Excerpt from The Medical Journal of North Carolina, 1860, Vol. 3 The cases referred to by Dr. Manson are evidently instances of central pneumonia in which there is certainly nothing peculiar to the "malarial" form. Indeed, it is only another of the many points of resemblance between Dr. Manson's "malarial pneumonia" and ordinary simple pneumonia, accidentally impressed by a malarious influence. "It is now well ascertained," says Fournet, "contrary to the opinion of Lænnec, that, in central pneumonia, the most careful auscultation cannot recognize any of the usual local signs of the disease. During the year 1837, there were three patients under the care of M. Andral, at la Charite, who were affected with central pneumonia, characterized by the sputa and the general symptoms, in all of whom the above was true. Some patients, in whom I have traced, with the utmost care, and, as I may say, from hour to hour, the progress of a pneumonia that I had seen arise under my own eyes, have had no primary crepitating rhonchus at the commencement of the disease, and no rhonchus redux at the period of resolution. Perhaps this usually occurs when the primary rhonchus is wanting." (a). Other authors, also, have long ago called attention to these cases, particularly Andral (b), Chomel (c), Williams d), &c. Indeed, it would be impossible, we presume, at the present time, to find any respectable authorities maintaining different grounds; and yet, as it seems to us, Dr. Manson speaks of such instances as if-they were encountered only in the "malarial" form of pneumonia! Subsequently, however, in alluding to them, he says: "In these cases the pneumonia is either central, or the secretion in the air-cells and finer branches of the bronchi may not be present in quantity sufficient to offer any obstacle to the transmission of air."
