Epidemiological studies are one part of well-conducted investigations while the other part is microbiological investigation of suspected food items. However, in most of the food borne outbreaks, all food items are consumed or thrown away and the only way to reveal the causative food item is through conducting an epidemiological study. Epidemiological studies are also used in research to identify risk foods and/or risks behaviour in sporadic cases in order to gain more knowledge about transmission routes. This Nordic workshop dealt with methodological issues concerning case-control studies and food borne infections, both in the outbreak situation and with sporadic cases. The invited speakers were asked to take up methodological difficulties relevant to case-control studies. The participants in the workshop concluded that some "success stories" in food borne outbreak investigations would serve as good examples when planning how to deal with outbreak investigations. It was also concluded that this kind of workshop is most useful in terms of learning as well as giving support to each other

Book excerpt: Globalization of the food supply has created conditions favorable for the emergence, reemergence, and spread of food-borne pathogens-compounding the challenge of anticipating, detecting, and effectively responding to food-borne threats to health. In the United States, food-borne agents affect 1 out of 6 individuals and cause approximately 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year. This figure likely represents just the tip of the iceberg, because it fails to account for the broad array of food-borne illnesses or for their wide-ranging repercussions for consumers, government, and the food industry-both domestically and internationally. A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. Participants discussed the globalization of the U.S. food supply and the burden of illness associated with foodborne threats to health; considered the spectrum of food-borne threats as well as illustrative case studies; reviewed existing research, policies, and practices to prevent and mitigate foodborne threats; and, identified opportunities to reduce future threats to the nation's food supply through the use of a "One Health" approach to food safety. Book excerpt: Legionnaires' disease, a pneumonia caused by the Legionella bacterium, is the leading cause of reported waterborne disease outbreaks in the United States. Legionella occur naturally in water from many different environmental sources, but grow rapidly in the warm, stagnant conditions that can be found in engineered water systems such as cooling towers, building plumbing, and hot tubs. Humans are primarily exposed to Legionella through inhalation of contaminated aerosols into the respiratory system. Legionnaires' disease can be fatal, with between 3 and 33 percent of Legionella infections leading to death, and studies show the incidence of Legionnaires' disease in the United States increased five-fold from 2000 to 2017. Management of Legionella in Water Systems reviews the state of science on Legionella contamination of water systems, specifically the ecology and diagnosis. This report explores the process of transmission via water systems, quantification, prevention and control, and policy and training issues that affect the incidence of Legionnaires' disease. It also analyzes existing knowledge gaps and recommends research priorities moving forward.

The work is the output of the disease reference group on zoonoses and marginalized infectious diseases (DRG6), which is part of an independent think tank of international experts, established and funded by the Special Programme for Research and Training in Tropical Diseases (TDR), to identify key research priorities through the review of research evidence and input from stakeholder consultations. The report covers a diverse range of diseases including zoonotic helminth protozoa, viral and bacterial infections considered to be neglected and associated with poverty. Disease-specific research issues were elaborated under individual disease sections and many common priorities were readily identified among the disease such as need for new and/or improved drugs and regimens, diagnostics and, where appropriate, vaccines. The disease specific priorities are described as micro priorities compared with the macro level priorities which will drive such policies as the need for improved surveillance; the need for inter-sectoral interaction between health, livestock, agriculture, natural resources and wildlife in tackling the zoonotic diseases; and the need for a true assessment of the burden of the zoonoses. This is one of ten disease and thematic reference group reports that have come out of the TDR Think Tank, all of which have contributed to the development of the Global Report for Research on Infectious Diseases of Poverty.

The Institute of Medicine's Forum on Neuroscience and Nervous System Disorders, in response to a request from the U.S. Secretary of Health and Human Services, hosted a workshop called "Autism and the Environment: Challenges and Opportunities for Research." The focus was on improving the understanding of the ways in which environmental factors such as chemicals, infectious agents, or physiological or psychological stress can affect the development of the brain. Autism and the Environment documents the concerted effort which brought together the key public and private stakeholders to discuss potential ways to improve the understanding of the ways that environmental factors may affect ASD. The presentations and discussions from the workshop that are described in this book identify a number of promising directions for research on the possible role of different environmental agents in the etiology of autism.
