Book Synopsis Care Without Coverage by : Institute of Medicine
Download or read book Care Without Coverage written by Institute of Medicine and published by National Academies Press. This book was released on 2002-06-20 with total page 213 pages. Available in PDF, EPUB and Kindle. Book excerpt: Many Americans believe that people who lack health insurance somehow get the care they really need. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Available in PDF, EPUB and Kindle. Book excerpt: Emergency department (ED) use is on the rise as availability of operational EDs has decreased. EDs in urban areas are overcrowded and contend with large amounts of patients with non-urgent complaints. To identify reasons for seeking care, a convenience sample of 100 non-urgent patients (ESI triage acuity 4 and 5) presenting to one Midwestern, urban ED was used. This quantitative, descriptive study utilized a 14 item self-administered questionnaire to collect demographic data and to ask a series of closed and open-ended questions about ED use. Specifically, the questions inquired about a patient's usual sources of care, decision-making, and perceived urgency of illness. The Andersen framework of health services utilization was employed to analyze patients' predisposing factors, enabling/impeding factors, and needs for care. Understanding reasons for non-urgent ED use is important to future hospital planning for optimal patient outcomes and effective resource utilization. The results of this study showed patient's seeking care in the ED for non-urgent complaints did so because of a perceived sense of urgency about their condition, a desire for the best care, and the convenience/accessibility of the facility. While a large portion of the sample did not have a primary care provider (PCP), insurance/PCP status were not found to be a significant factor.

Download or read book Hospital-Based Emergency Care written by Institute of Medicine and published by National Academies Press. This book was released on 2007-05-03 with total page 424 pages. Available in PDF, EPUB and Kindle. Book excerpt: Today our emergency care system faces an epidemic of crowded emergency departments, patients boarding in hallways waiting to be admitted, and daily ambulance diversions. Hospital-Based Emergency Care addresses the difficulty of balancing the roles of hospital-based emergency and trauma care, not simply urgent and lifesaving care, but also safety net care for uninsured patients, public health surveillance, disaster preparation, and adjunct care in the face of increasing patient volume and limited resources. This new book considers the multiple aspects to the emergency care system in the United States by exploring its strengths, limitations, and future challenges. The wide range of issues covered includes: • The role and impact of the emergency department within the larger hospital and health care system. • Patient flow and information technology. • Workforce issues across multiple disciplines. • Patient safety and the quality and efficiency of emergency care services. • Basic, clinical, and health services research relevant to emergency care. • Special challenges of emergency care in rural settings. Hospital-Based Emergency Care is one of three books in the Future of Emergency Care series. This book will be of particular interest to emergency care providers, professional organizations, and policy makers looking to address the deficiencies in emergency care systems.

Book excerpt: In the last ten years use of Emergency Departments went from 89.8 million to 110 million visits (Weber, Showstack, Hunt, Colby and Callaham, 2005). The problem of ED overcrowding is truly a multifaceted one, even definition of "overcrowding" has multiple interpretation depending on the authors. Whatever the correct definition the issue at hand is that truly emergent patients lives are at risk, if no intervention is made within one hour of onset of symptoms, those patients will die. Appropriate triage, patient education and redirecting non-urgent complaint to different areas of Emergency Department staffed with Nurse Practitioners or Family Medicine Physicians may be an answer to long term ED overcrowding. Having a fast track area in or next to Emergency Department may be financially sound as well. Unnecessary medical testing and occupancy of ED beds by non-emergent patients increase healthcare spending (Adesara, Spencer and Bost, 2011). By reorganizing patient flow after triage and directing level 4 and 5 patients to separate area of the ED would decrease main ED wait times, allow ED nurses and physicians to concentrate their efforts on patients who would have adverse outcome if their respective health issue is not addressed during first "golden" hour. It will also allow for appropriate patient education by case managers and social workers providing health teaching and follow up referrals resulting in continuity of care for those non03́emergent patients using ED for primary care concerns.

Book excerpt: Once considered reserved for life-threatening disease or illness, emergency departments in the United States are now described as the primary care clinic and the social work department for many Americans (Grumback, Keane & Bindman, 1993). A 1992 study found 2 million children in the United States routinely used the emergency department as their primary source of care (Halfon, Newacheck, Wood & Peter, 1996). Studies have shown 50-90% of all patients presenting to emergency departments have minor complaints which could easily be treated in primary care settings (Dowling & Dudley, 1995; Grumbach et al., 1993). The non-urgent use of emergency departments has led to overcrowding, long waiting times and patient dissatisfaction (Grumbach et al., 1993; Derlet & Nisho, 1990, Liggins, 1993). Overcrowding and inappropriate use of the emergency department also interferes with the care of critically ill patients (Dowling & Dudley, 1995). Civilian hospitals have developed various strategies to address patients with minor complaints varying from outright refusal of care, to triaging to off-site clinics, developing non-urgent care clinics and establishing Fast Track Clinics also known as Minor Emergency Areas (MEAs) within existing emergency departments. These Fast Track clinics have been staffed with a variety of non-physician, mid-level health care providers including nurse practitioners.

Available in PDF, EPUB and Kindle. Book excerpt: ABSTRACT: Utilization of the emergency department (ED) for non-urgent visits has become an important topic among healthcare providers and administrators. Non-urgent visits to the ED result in financial and health related problems for those who utilize the ED in this manner, as well as managed care organizations. There are many influences included in patients' decisions to utilize the ED for non-urgent reasons. The purpose of this research was three fold. The first purpose was to identify reasons for utilizing a southwest Florida Emergency Department rather than other healthcare services for non-urgent care. The second purpose was to identify common factors among patients seeking non-urgent care in the emergency department in a southwest Florida rural emergency department. The third purpose was to determine if patients who visit a southwest Florida Emergency Department for non-urgent reasons receive less preventative care than national standards.

Available in PDF, EPUB and Kindle. Book excerpt: A major reason behind crowding in emergency departments (ED) is non-urgent patients' visits to ED. In this paper we investigate how non-urgent ED visits are influenced by patients' imperfect perception of their urgency and their self-interested choice, and we explore interventions that can reduce non-urgent ED visits and social cost. We consider a healthcare setting where heterogeneous patients with imperfect perceptions choose between an expensive/congested ED and less expensive general practitioners (GP) who refer urgent patients to the ED. We model patients' choice problem as a network queueing game, and analytically characterize equilibrium patient flows and their variation with patients' perception errors. We find that non-urgent ED visits and social cost may increase by reducing perception errors. We also characterize socially optimum flows and show that, if optimum flow can be induced at equilibrium then reducing perception errors can guarantee to reduce the social cost. We further explore how to align equilibrium patient flows with optimum flows, and identify scenarios under which perception-improvement measures can achieve this alignment. For other scenarios we design incentive mechanisms to achieve the alignment, which include a novel differential pricing mechanism that leverages the special referral feature of the ED-GP network.
