Book excerpt: Hardly any other part of the human body is of such interdisciplinary interest as the anal, perianal and rectal region. Gastroenterologists, dermatologists, urologists, general practitioners and surgeons specialising in proctology, phlebology, and coloproctological surgery are involved in this region between the ectoderm, transitional zone, and entoderm. Diagnostic procedures, such as endoscopy, radiology, sonography, manometry, electromyography, and histopathology are even more diverse, in particular where the differential diagnosis of non-neoplastic conditions of the anorectal region is concerned. The Falk Symposium No. 118, held in Freiburg, Germany, on October 1-2, 2000, focused on the morphology and function of the pelvic floor and its dysfunction, radiation damage in proctology, haemorrhoidal complaints, and chronic inflammatory rectal diseases, as well as conditions of the anal and perianal region. These proceedings will be of interest to all research physicians and colleagues working in hospitals or in their own practice who are involved in the diagnosis and treatment of anorectal pathologies.

Book excerpt: Management of benign anorectal disorders is challenging for the treating surgeons as the patients are highly distressed, the symptoms are always alarming, and surgical intervention may be associated with lifelong disabilities such as loss of sphincter control. Written by highly experienced colorectal surgeons, this book provides detailed notes on optimal management of these disorders including pre- and post-operative management. Chapters cover the entire range of benign disorders such as hemorrhoids, fissure, fistula-in-ano, anorectal injuries, anal incontinence, rectal prolapse, pelvic floor disorders, benign tumors and ulcers, and strictures. Chapters on surgical anatomy and physiology equip the surgeons with the information necessary to understand the complex anatomy and functioning of this region which makes surgery all the more difficult. Benign Anorectal Disorders is an essential reading for colorectal surgeons, general surgeons, fellows-in-training and the post-graduate students.

Available in PDF, EPUB and Kindle. Book excerpt: This book covers the entire spectrum of non-neoplastic diseases within the gastrointestinal (GI) tract including new entities, recent developments, and questions that arise in the practice of GI pathology. The text is organized by specific organ site, with related disorders of each organ site subgrouped into several chapters based on their common features. Pathologists can quickly find the disorders they are interested and review similar disorders in the same chapter to help them make a correct diagnosis. Each major and common entity is described in detail with its definition, clinical features, pathological features (covering both the gross and microscopic details), differential diagnosis and treatment/prognosis. All chapters also highlight the use of special/immunohistochemical stains and other supporting studies as needed with a focus on providing a practical differential diagnosis rather than just a list of potential associations. This book is extensively illustrated with both gross and microscopic images that act as an integral part of the information provided in the text. Written by practicing GI/liver pathologists and gastroenterologists from the Mayo Clinic, Surgical Pathology of Non-neoplastic Gastrointestinal Diseases serves as a comprehensive yet practical guide for diagnostic surgical pathologists with a passion for GI pathology.

Book excerpt: "Prior to the development of the first fibre-optic endoscopes in the 1960's, gastroenterology, in common with other traditional medical specialties, relied on biochemical and radiological techniques in order to investigate the gastrointestinal tract. Histological confirmation of a disease process usually required the presence of a surgeon, was invasive, and carried inherent risks. Because of a rapid improvement in technology, we are now able to sample the entire gastrointestinal tract from the mouth and anus through to the ileal / jejunal junction"--

Available in PDF, EPUB and Kindle. Book excerpt: The prevalence of anorectal disorders in the general population is probably much higher than that seen in clinical practice. Anorectal diseases have for a long time been considered of little interest, with their treatment considered of little prestige despite the social impact they cause to patients. Proctology was initiated late and developed slowly over the years. However, in the last 20 years, a renewed interest has begun, and today, we can say that proctology is a specialized branch of general surgery. This book "Proctological Diseases in Surgical Practice" provides a practical introduction to proctology, with a particular attention to the topics that have not yet been investigated. This book may be useful for the general physician as well as for the specialist.

This book was released on 2002-09-30 with total page 304 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book is the proceedings of the Falk Symposium No. 127 on `Autoimmune Diseases in Paediatric Gastroenterology' (IV International Falk Symposium on Paediatric Gastroenterology), held in Basel, Switzerland, on November 8-9, 2001. The symposium focused on the role of the immune system, both the acquired and the innate systems, in inflammatory bowel disease (IBD) in children and adolescents. The innate system has an important fundamental role in host defence by initiating immune responses against potentially deleterious matter. However, a mutation within the innate system may elicit an immune response against the host: hence, an autoimmune response. Chronic autoimmune hepatitis occurs predominantly in young people, and especially in women. Immunological changes are conspicuous. Tissue antibodies are found in a large number of patients. This is a disease of disordered immunoregulation marked by a deficit in suppressor T cells causing the production of autoantibodies against specific hepatocyte surface antigen. Liver membrane protein is found in the sera of patients with autoimmune chronic acute hepatitis and with primary biliary cirrhosis (PBC). The latter condition of progressive granulomatous destruction of intrahepatic bile ducts is, in many respects, analogous to the graft-versus-host syndrome where the immune system has become sensitized to foreign HLA-molecules. Primary sclerosing cholangitis (PSC) is another condition of unknown origin. All parts of the biliary tree can be involved in a chronic, fibrosing, inflammatory process that results in obliteration of the biliary tree and ultimately in biliary cirrhosis. About half of the patients also suffer from ulcerative colitis and rarely from Crohn's disease. Circulating antibodies to some antigens are found in obstructed portal tracts, as well as increased concentrations of biliary immune complexes in patients with PBC. In all three previous Falk symposia on paediatric gastroenterology, attention was focused on the role of the innate immune system in the aetiology of IBD. It has become increasingly clear in recent years that the innate system has a much more important and fundamental role in host defence. The decision to initiate an immune response is one of the major roles of the innate system. Mutations within this system could transform it into becoming constitutively active, resulting in an inflammatory reaction and thus eliciting an autoimmune response. Following an introduction to the basic phenomena of autoimmunity, the proceedings discuss clinical aspects of autoimmune diseases. In particular, current knowledge and the state of the art about the diagnosis and treatment of the autoimmune diseases of the gastrointestinal tract are described by world-renowned experts. The book also contains the short presentations on selected topics, as well as abstracts of the mini-posters read by title, which were included in the symposium.

Book excerpt: In recent years, our knowledge about the pathogenesis, pathophysiology and treatment of hepatobiliary diseases has increased considerably. The molecular basis of cholestatic disorders as well as of gallstone disease is increasingly recognized. This has resulted in improved diagnosis, for instance in hereditary forms of intrahepatic cholestasis, and advances in treatment, for example in primary biliary cirrhosis and other chronic cholestatic disorders. This book, the proceedings of a Falk Workshop held in Cluj-Napoca, Romania, on June 9-10, 2000, brings together contributions from scientists and clinicians to highlight the most recent advances in molecular biology, pathophysiology, diagnosis and therapy of diseases of the hepatobiliary system. World experts cover a broad spectrum of topics from genetic studies to endoscopy and from medical treatment to liver transplantation.

Available in PDF, EPUB and Kindle. Book excerpt: Pregnancy represents an allograft in so far that half of the antigens are foreign to the mother. Therefore, a bidirectional interaction between the maternal and the fetal immune systems must exist, which could be disturbed by preexisting autoimmune liver diseases. Since nowadays autoimmune liver diseases are increasingly detected as early stage diseases due to modern diagnostic procedures, e.g. in women of childbearing age, and since the number of posttransplant pregnancies has become an issue due to improved survival, questions concerning regnancy, autoimmune liver disease and treatment options with bile acids and/or immunosuppressants are of an ever-increasing interest. This book, the proceedings of a Falk Workshop held in Freiburg, Germany, on June 2, 2002, is certainly unable to give any definite answers to any of the hundreds of still remaining questions in this fascinating field, but hopefully it will help to stimulate and initiate cooperation among immunologists, bile acid researchers, gynaecologists and internists.
