Book excerpt: This study utilizes a national survey of physicians in the United States, administered four times between 1996 and 2005, to examine the use of non-financial performance measures in physician compensation contracts. Consistent with agency theory, we find that non-financial measures are used more frequently when the measures are more informative, when alternative control mechanisms are complements rather than substitutes, and when external pressures for quality of care and cost-containment are greater. Further, we find that contractual relationships in the healthcare value chain are inter-related; non-financial measures are more likely to be used to evaluate physician performance when the physicians' practice is compensated based on fixed rate payments (i.e., capitation). We also find that physicians' compensation contracts are more likely to incorporate non-financial performance measures when productivity in revenue generation is also used to evaluate performance. Taken together, the results suggest that non-financial performance measures play a significant role in physician compensation, acting to balance incentives tied to individual physician productivity.

Book excerpt: Medicaid plays a vital role in the U.S. health care system and the importance of office-based physician practices to the care of Americans served cannot be overstated. Despite increases in program enrollment, a number of sources report that the proportion of U.S. office-based physicians willing to accept new Medicaid patients is decreasing. This discovery is significant. This managerial accounting study utilizes a national survey of practicing physicians to explore this conundrum from the physicians' perspective. Specifically, the impact of organizational-related determinants including (1) practice characteristics, (2) compensation and (3) performance measures is examined on physicians' propensity to accept new Medicaid patients. With an empirical analysis using the cumulative odds ordinal logit model, this study breaks new ground in several significant venues. Only a limited number of studies have yet examined physician practice characteristics relative to physician acceptance of Medicaid, but these were far more limited in scope. In addition, this study offers a first look at how financial and non-financial incentives collectively impact patients' access to Medicaid. Finally, it is important to point out that this study provides for a comprehensive look at understanding what organizational characteristics help to determine the probability of physician acceptance of new Medicaid patients.The most significant factors found to be associated with diminished propensity for Medicaid access include: operating as a primary care doctor, physician ownership in the practice, practice size, low relative geographic reimbursement, supportive clinical IT and gender. Knowing what factors are associated with physicians' decisions should provide opportunities for potential public policy solutions.

Book excerpt: Most executive compensation contracts (specifically bonuses) are based on accounting-related measures such as earnings per share (EPS), growth in revenue, return on stockholders' equity. However, the recent trend of account manipulation and earnings management documented in the literature may lead to inequitable bonus payments to the managers or executives. Alternative performance measures that may be used to motivate managers are stock prices or market returns and non-financial measures. In this paper, I examine firms in the Samp;P 500 index and examine the type of performance measures they use in their short-term compensation. I expect that firms that use non-financial performance measures will have a lower prevalence of earnings management, since these measures are harder to manipulate. The findings are opposite to expectations. The firms that use both financial and non-financial performance measures actually have higher discretionary accruals, specifically accounts receivable accruals, than the firms that only use financial performance measures. The interpretation of these results is that these firms suffer from low earning s quality which makes the accounting numbers less meaningful.

The fate of P4P is not yet certain, but we can learn a number of lessons from experiences with P4P to date, and ways to improve the designs of P4P programs are becoming apparent. We anticipate that a “second generation” of P4P programs can now be developed that can have greater impact and be better integrated with other interventions to improve the quality of care and reduce costs.

Indeed, the accounting earnings have been the subject of several studies. A few of them have attempted to check whether to introduce the Non-Financial Performance Measures (NFPM) as a constraint on earnings management. The aim of this paper is to empirically test the impact of NFPM on earnings management behavior. Our sample consists of 89 Canadian listed firms pertaining to three sectors for the period, 1997-2006. The results are conclusive and showed that there is a negative relationship between Discretionary Accruals (DA) as a proxy for earning management and the relative importance of NFPM in managers' compensation contracts, suggesting that NFPM acts as an additional earnings management constraint.
