May it Please Your Honour: In presenting its ninth annual report the Provincial Board of Health is gratified to be able to record the fact, that during the past year there has not been a single outbreak of smallpox. The Province has not been equally fortunate in escaping the visitation of other zymotic diseases. Thus in Toronto, with an estimated population of 178,000, typhoid fever caused 117 deaths, diphtheria 71, measles 18, and scarlet fever 14. The total deaths for the year were 2,919, the ratio per 1,000 of population 16.39, and the zymotic death-rate 3.43. Now the zymotic death-rate, or in other words, the mortality from preventable diseases is a measure whereby one may test the purity of a municipality's water supply, its freedom from nuisances, and the fidelity with which notification, isolation and disinfection are attended to in resisting the attacks of contagious disease within its borders. The postulate of hygiene is: given pure air, particularly in inhabited places, purity of water supply, prompt action in giving notice of the existence of zymotic disease, immediate isolation of patients and exposed persons, with the necessary disinfection of persons and things, and zymotic disease will soon cease to appear in any locality. The nearer a municipality approaches to the conditions of the postulate, the freer will it be from diseases of this class. Measured by this standard, Toronto with its zymotic mortality of 3.43 maybe considered one of the healthiest cities in the Dominion, the zymotic death-rate of Quebec for 1890 being 9.88 per 1,000, Montreal 7.08, Halifax 5.82, Ottawa 5.36, Winnipeg 4.89, St, John, N.B., 3.49, Hamilton 3.49, and London 2.09. And yet if we compare even the best of these, that of London, Ontario, with 1.5 the zymotic death rate of Kensington, a London parish having a population of 178,000, we gain some idea of the extent of our own shortcomings, and also of the influence which hygiene can and does exert even in densely populated cities, in preventing the spread of contagious diseases, and consequently preserving the lives of the people. It is evident, therefore, that if we would attain to the highest degree of sanitary well-being, the people of Toronto as well as other Ontario municipalities must strengthen the hands of their Local Boards of Health, and see to the appointment of competent medical health officers whose duty it will be to look narrowly into existing defects, and by advising their removal, accomplish much actual good, while at the same time raising the standard of hygienic endeavor among the people. In this Province, as in other countries, about 85 percent, of all deaths from diphtheria are of children under ten years of age. Among grown people the mortality is much less. This disease, though not one which ordinarily appears in epidemic form, nevertheless, though endemically present in most populous districts, has appeared in localized outbreaks, and, owing to its great fatality and painful symptoms which it exhibits, is one to which the attention of bacteriologists was early directed. So early as 1884, Carle and Cattani discovered virus in tetanus that could be inoculated from men to animals but it was not until 1889 that the. Tetanus bacillus was obtained in pure cultures by Kitasato, and als by Tizzoni and Cattani. As an anaerobe, which was spore-bearing. These spores are exceedingly resistant even to moist heat; but, fortunately, the germs do not develop in free air. Its presence, however, in the earth of gar dens and streets in various localities, made it' very desirable that investiga tions be carried on to discover if possible its antitoxin. Behring, Kitasato, Ehrlich and Tizzoni labored at this for some years, and by 1890 the fact of an immunity being established against the disease through the injection of a toxin of graduated strength, was confirmed. Its utility has hitherto been limited by the fact that the quantity of antitoxic serum required for a suc cessful result is at least two thousand times as great after the symptoms of tetanus have appeared, as compared with that required when an injection has been made antecedent to inoculation with the disease. This fact, asso ciated with the further fact that the tetanus germ goes on developing in an ordinary punctured wound without any sign of its presence till the onset of the spasms caused by it, makes any chance of treating the disease suc cessfully by the antitoxin relatively slight. The very complete knowledge which we now possess of the cause of tetanus, and the. Conditions under which it develops have, however. Enabled the surgeon when called upon to treat punctured wounds, to take such steps at the time the wound occurs as will preclude in large measure any danger of its development. Book excerpt: Excerpt from Second Annual Report of the Provincial Board of Health of Ontario, Being for the Year 1883 In this way, many of the lives yearly sacrificed to other filth diseases have been saved. It is here worthy of remark that, throughout our Province, as in most other countries, more deaths are caused yearly by the want of systems for securing thorough cleanliness of streets, lanes, and premises, good ventilation, effective drainage and ventilation of drains, good water supply, isolation of infected persons and careful disinfection than have ever been superadded to the ordinary mortality by any epidemic of cholera that has occurred. We would express the hope that Ontario will not be amongst the last to do away with this unsatisfactory state of affairs. Any person sufficiently interested in the saving of life, health and wealth, may obtain a fair idea of what may be gained by increased attention to sanitary organization and sanitary reforms generally, by calculating from the figures given in the "Statesman's Year Book," the ratio of deaths to population during the six years before the passage of the English Public Health Act of 1875 and during the six years following that date. He will find on adding the columns of population and deaths as given on page 243 of the volume for 1883, that for the first mentioned period the death rate was 22.09 per 1,000, and for the last mentioned, 20.63 per 1,000.Continuing his calculations, he will find the total saving thus effected in six years to amount to 219,118 lives. In England and Wales alone there were saved a number more than twice as great as the standing army of Great Britian for the same period. We would be horrified if the British army was destroyed by hostile armies every three years, and yet the people have been carelessly allowing to lurk in their midst invisible foes whose ravages have been no less destructive to life. And we may here remark that these figures are taken from one of the most accurate records in the world, and that they cover in each case a period of six years. We must also remember, that as years have rolled on, the population has been growing more dense. The Imperial Act of 1875 was the outcome of the formation of the Local Government Board of England, and one of its chief features is that provision is made for the establishment of a Board of Health in every municipal district, apart from the Boards of Aldermen and Councillors already in existence. It was expected that the formation of a Board in every municipality, whose sole duty it would be to care for the health of that municipality, and composed of men elected on account of their interest in sanitary matters, would be the means of saving many lives. How far this expectation has been realized the above figures have partially shown; but the saving, so far effected, will be enormously increased when the various schemes recently instituted for the stamping out of infectious diseases have been fully adopted, as they ultimately must be. People will not forever listen to erroneous nonsense about infringments of personal liberty and interference with business, whilst their children are being needlessly slaughtered by preventable disease. The people are now realizing more fully that life and money can be saved by more efficient sanitary organization. The remarks which have been made will apply with greater force to the Province on Ontario than they do to England. Many of the sanitary reforms and sanitary organizations which this Board desires to see introduced here had been in existence in many parts of England prior to the passing of the Act of 1875, to a far greater extent than is the case in this country, and it is the extension of them to every district, and the perfecting of the organizations referred to, that caused the saving of life before described. But let us for the present anticipate no more than that by the adoption of similar legislation similar results will follow. This means t

In the report of the Secretary to the Board, following this introduction, you will find the few small outbreaks which have occurred referred to in detail; and in regard to the lessons which we may learn from the record of these outbreaks I would respectfully draw the attention of Your Honour to two needs to which they point. The first is the desirability of having impressed upon the public, in our urban and village municipalities especially, the necessity there is for erecting what has been found to be economically quite possible, Isolation Hospitals or "Houses of Recovery" to which cases of contagious disease can be removed, along with the mother or nurse if desirable, and there-be attended by the family physician. Conclusive evidence is adduced from cities in this Province, as also from abundant experience in England, to show that the safety of the patient is thereby best guaranteed, the freedom from exposure of the rest of the family fully accomplished, and the cost of such outbreaks very materially lessened both to the affected family and the municipality. Book excerpt: Excerpt from Fifth Annual Report of the Provincial Board of Health of Ontario: Being for the Year, 1886 The progress of Sanitation in our Province during the last year, we may fairly consider to have been satisfactory, and the good work done by the numerous Local Boards most encouraging, affording conclusive evidence of an increase of interest in the minds of the people on matters connected with public health. On the routine work of our Board it will be unnecessary to dilate, as the Secretary, in his report, will enter fully on the discussion of all matters that at the various quarterly and special meetings have occupied the attention of the members; but on a few points of great importance to the community at large, I purpose briefly commenting: - 1st. On the establishment of a vaccine farm at Palmerston, under the supervision of the Board and subsidized by the Government, Dr. Stewart, of that town, being the Manager. The occasional but somewhat rare occurrence of death from smallpox after vaccination, erysipelas and cases of blood poisoning from the use of impure virus, have led to a distrust in the minds of many to the practise of vaccination, as the Medical Officers, during the epidemic of last year in the Province of Quebec, had, too frequently for success in stamping out the disease, occasion for observing. It therefore became the imperative duty of all State and Provincial Boards of Health ta endeavour to provide for the area of country under their charge vaccine virus to which no possible suspicion could attach. In view of the great importance of convincing the public that every possible precaution against impurity should be insisted on, it was deemed by our Board that the frequent supervision by the Secretary and one of our members resident in the neighbourhood, and the quasi Government imprimatur, would be the most effectual step towards antagonizing a most disastrous prejudice. Entertaining this belief at a meeting of the Board, February 2nd, 1886, the following resolution, moved by Dr. Yeomans, seconded by Dr. Cassidy, was carried: That this Board having received notification from Dr. Stewart, of Palmerston, of the establishment by him of a vaccine farm, that he has supplied medical men throughout the Province, and further has asked continued inspection of his farm and methods of procedure, therefore this Board recommend that the Government should provide for the continued, frequent and thorough inspection of this vaccine farm, or other vaccine farms that may hereafter be established, with the object of securing a supply of pure and reliable vaccine lymph, procurable within this Province; and that a copy of this resolution be transmitted to the Attorney-General. At the quarterly meeting in May, Dr. Bryce read his report on the inspection made of the Ontario Vaccine Farm at Palmerston, which report was received and adopted. On the following day, on motion of Dr. Yeomans, seconded by Dr. Cassidy, it was resolved, That inasmuch as Dr. Stewart had fulfilled some of the conditions required by this Board for the supply of reliable vaccine virus for the Province, this Board would recommend the payment of 8250 out of the Government grant, also that the remainder be paid in quarterly instalments. Dr. Stewart continuing to fulfil the conditions laid down by this Board. Since the establishment of this vaccine farm the supply of virus required in our Province has been largely obtained from Dr. Stewart to the complete satisfaction at least from any communications to our Board to the contrary of all who have made use of it. By this establishment of a vaccine institution under Government surveillance, the members of the Board entertain the hope that the prejudice existing in the minds of too many for safety against the dreaded disease. Smallpox, by vaccination, may in time be dispelled and the use of the only effectual preventive universally had recourse to. At a special meeting of the Board convened on the 21 st of April .
