Book excerpt: The management of cerebral aneurysms is still the subject of controversy in spite of recent dramatic advances in surgical techniques and neuro-intensive care. Currently, two main topics in this field have totally different aspects: 1. the management of unruptured cerebral aneurysms as a preventive medicine and 2. neurocrutical management of severe subarachnoid hemorrhage (SAH) after the rupture. These topics were discussed during the Swiss-Japanese Joint Conference in Zurich, Switzerland, in May 2001. Selected papers from this meeting are included in this volume presenting new clinical experiences in order to find out updated and proper ways to focus the treatment and providing updated information on neurocritical care aspects as well as endovascular and surgical treatment modalities carried out in daily practice in Zurich and Japan.

Book excerpt: Every so often a gathering of minds and experience occurs that results in an all encompassing overview in depth of such a vast subject as Cerebro-Vascular Malformations, as occurred in Verona in June 1992 and which warrants publication. Professors Da Pian and Pasqualin deserve high compliment and it is a measure of the respect in which they are held that virtually all those most knowledgeable around the world attended, presented their work and thoughts and contributed to intense discussion. Ljunggren's opening historical survey set the stage and must be the most comprehensive yet published. Subarachnoid hemorrhage from aneurysm rupture still constitutes, I dare say, the most difficult problem for neurosurgeons, in relief of the brain injury and arterial reaction and the technical perfection of aneurysm obliteration, even for small, as well as large and giant sacs. Very large high flow A VMs can be as demanding too. The bulk of the conference was devoted to subarachnoid hemorrhage, aneurysms and A VMs which were discussed under about 14 headings each. But vein of Galen malformations, dural A VMs, cavernous angiomas and venous angiomas (renamed developmental venous anomalies) came under scrutiny, not always with consensus. Trends are perceptible such as fibrinolysis of subarachnoid clot, non surgery for Galenic and dural malforma tions, the benignity of venous angioma, but there is still much variation in approach, pharmacologically, technically and with such as the evolving endovascular and radiosurgical stories, used alone or in conjunction.

Further chapters deal with the management of unruptured aneurysms and with subarachnoid hemorrhage. Practical guidelines for vasospasm treatment are given. Together with contributions about arteriovenous malformations and fistulas, cerebral revascularization techniques and surgery related to the intracranial venous system a comprehensive overview about stroke surgery is given with an interdisciplinary approach. The book will be of interest for all specialists involved in therapy of cerebrovascular disease.

Available in PDF, EPUB and Kindle. Book excerpt: Seminal resource on state-of-the-art flow diversion techniques from leading neurointerventional experts From detachable balloons and GDC coils to the recent advent of flow diversion, practitioners of endovascular neurosurgery have been fortunate to work in an era of rapid and exciting advances. The first commercially available flow diverter in the U.S. was approved specifically for a small subset of cerebral aneurysms. Recent experience has demonstrated its utility in treating challenging or otherwise untreatable aneurysms, safely and efficaciously. The design of these devices requires learning radically different methods than those used in the deployment of other, non-braided stents. Flow Diversion of Cerebral Aneurysms by Min Park, Philipp Taussky, Felipe Albuquerque, and Cameron McDougall provides step-by-step guidance on utilization of flow diversion technology in clinical practice. Reflecting the combined experience and knowledge of pioneers in neurointerventional surgery, this comprehensive book fills a gap in available resources. Twenty-one chapters cover fundamentals to advanced concepts – historical perspective to future developments. Key Features More than 100 high quality graphics and illustrative case studies reinforce key concepts Techniques and nuances of Pipeline, Silk (Balt Extrusion), Surpass Streamline, and Flow-Redirection Endoluminal Device (FRED) deployment An overview of current flow diversion devices, discussion of coil embolization versus flow diversion, off-label uses, adjuvant approaches, and hemodynamic modifications Pharmacology, flow diversion grading scales, and post-procedure radiographic imaging Clinical pearls on ruptured aneurysms, intraprocedural/postprocedural complications, and management of aneurysm residuals The ultimate goal of incorporating cutting-edge flow diversion techniques into the aneurysm treatment paradigm is improved efficacy and patient outcome. The knowledge gleaned from this outstanding resource will help residents and fellows learn to deploy flow diverters for the first time and enable seasoned clinicians to expand their neurointerventional radiology skills.

While open microvascular technique has dominated Western surgical practice, surgeons in the former USSR have developed endovascular techniques that have gained acceptance among surgeons here. This text demonstrates both surgical and endovascular approaches, written and illustrated by surgeons with vast experience in both, in a comparative context. The work is extensively illustrated with full-color surgical illustrations, line drawings, and radiographs.

This book was released on 2012-12-06 with total page 196 pages. Available in PDF, EPUB and Kindle. Book excerpt: The small neck of the aneurysm afforded an easy surgical attack. An ordinary flat silver clip was placed over the sac and tightly compressed obliterated it completely. The clip was flush with the wall of the carotid artery. The sac, lateral to the silver clip, was then picked up with the forceps and thrombosed by the electocautery. Walter Dandy reporting his successful operation of a posterior communicating aneurysm on March 23, 1937. Walter Dandy's patient left the hospital in good health 2 weeks later, and from his report one may gain the impression that the operation was an easy task. Despite continuous developments during the following decades, it was not until the introduction of the operating microscope and microsurgical techniques that surgical treatment was generally accepted. During the microsurgical era surgical results have continued to improve due to diagnostical, neuroanaesthesi ological, and microsurgical refinements, and improved neurointensive care. Endovascular obliteration has become an important treatment alternative but this has not been included in this particular volume. The purpose of the present supplement of the ACTA NEUROCHIRURGICA is to review some of the elements in the neurosurgical management of patients with aneurysmal subarachnoid haemorrhage that are important for a successful outcome. Professor Helge Nornes has been a major force in the development of new techniques and research strategies in this area for a number of years and has recently retired from the National Hospital in Oslo.
